{"title":"Cases of trauma due to war and violence among children in Gaza.","authors":"Shehreen Latif, Enas Abdelraof A Muaileq, Hani Chaabo, Fatima Rashid, Mairead Kelly, Nida Khatri, Hannah Alice King, Neil M Fournier","doi":"10.26719/2025.31.4.210","DOIUrl":"10.26719/2025.31.4.210","url":null,"abstract":"<p><strong>Background: </strong>The conflict in Gaza has exposed children to continuous cycles of violence and trauma, profoundly affecting their mental health.</p><p><strong>Aim: </strong>To illustrate the psychological burden on Gaza's children and highlight their complex mental health needs amid the conflict.</p><p><strong>Methods: </strong>We report cases of 4 children receiving support from Children Not Numbers, who have lived through the violence in Gaza, illustrating the devastating impact of the conflict on their mental health.</p><p><strong>Results: </strong>The children exhibited symptoms of trauma, including emotional dysregulation, social withdrawal, grief, and worsening of pre-existing conditions. Over 12 000 of them have been injured and many live in unprecedented circumstances with debilitating injuries and disability.</p><p><strong>Aim: </strong>To document the rehabilitation needs of children with debilitating injuries and disability in Gaza.</p><p><strong>Methods: </strong>We reviewed the clinic data of children in need of rehabilitation from the Children Not Numbers database to highlight their disability types and the treatment and care required and received.</p><p><strong>Results: </strong>Until the time of this documentation, 242 children from Gaza had been referred to Children Not Numbers for management, of which 63 (26%) required rehabilitation due to debilitating injuries and disability. The majority (60%) of those requiring rehabilitation presented with traumatic conditions and 32% presented with neurologic conditions. Average age of these children was 6.9 years (range 4 months to 17 years).</p><p><strong>Conclusions: </strong>Rehabilitation and continuous care and support is essential for injured and disabled children who survive the war in Gaza, to give them a better chance of achieving optimal functional outcomes and wellbeing. {"title":"Restoring health and hope to displaced Gaza children with malignant disease at a cancer centre in Jordan.","authors":"Rawad Rihani, Sima Jeha, Zeena Salman, Shrouq Amer, Ibrahim Qaddoumi, Carlos Rodriguez-Galindo, Asem Mansour","doi":"10.26719/2025.31.4.243","DOIUrl":"https://doi.org/10.26719/2025.31.4.243","url":null,"abstract":"<p><strong>Background: </strong>Gaza children with cancer have faced critical delays in diagnosis and treatment due to the conflict, leading to late-stage presentation.</p><p><strong>Aim: </strong>To assess the impact of disruptions on treatment outcome for Gaza children with cancer treated at King Hussein Cancer Center in Amman, Jordan.</p><p><strong>Methods: </strong>We collected and analysed data from Gaza children aged 0-21 years with malignant disease, who were evacuated and received treatment at King Hussein Cancer Center between October 2023 and October 2024. We used the Kaplan-Meier survival estimates to analyse their overall survival rate.</p><p><strong>Results: </strong>Fifty-one Gaza children received treatment for cancer at the centre during the study period; median age 8.2 years, 66.7% male. Of the cases, 25.5% were newly diagnosed, 11.8% were relapsed and 62.7% had been previously treated in Gaza; 39.2% had leukaemia and 21.6% had solid tumours. Their referral had been delayed by 3-4 months, and at presentation, 51% exhibited psychological distress and 27.5% were underweight. After 4 months of follow-up, 96% survived.</p><p><strong>Conclusion: </strong>Most of the children experienced delays in diagnosis, referral and treatment and these impacted their treatment outcomes. {"title":"Challenges in providing ophthalmic care to children in Gaza.","authors":"Amena Masrur, Yousef Atef Yousef Yasin, Eyad Oa Alhalis, Enas Abdelraof A Abu Muaileq","doi":"10.26719/2025.31.4.262","DOIUrl":"https://doi.org/10.26719/2025.31.4.262","url":null,"abstract":"<p><strong>Background: </strong>There are several challenges in providing ophthalmic services to children in Gaza due to the conflict.</p><p><strong>Aim: </strong>To document the challenges in providing ophthalmic care to paediatric patients in Gaza and offer possible solutions.</p><p><strong>Methods: </strong>Between March and September 2024, we interviewed 23 children in 4 hospitals in Gaza, aged < 20 years and diagnosed with traumatic and non-traumatic ophthalmic disorder. We assessed their visual acuity, adnexal, anterior and posterior segments, and ocular motility and analysed the data using SPSS version 20. We discussed their cases during the weekly meetings and devised a management plan for each of them.</p><p><strong>Results: </strong>Eleven (48%) of the children (95% CI 27.42-68.92%) presented with ocular trauma and 12 (52%) with nontraumatic ocular disorders (95% CI 31.08-72.58%). The most common challenge in managing these patients was ineffective communication; it was difficult to obtain relevant clinical information for 52% of the cases. Other challenges included the degraded infrastructure, limited number of trained personnel and inefficient evacuation corridors.</p><p><strong>Conclusion: </strong>The conflict in Gaza and the lack of facilities to adequately cater to their ophthalmic needs expose children to the risk of developing amblyopia and long-term vision loss. However, COVID-19 pandemic disrupted the provision of surgical services in Iranian hospitals and globally.</p><p><strong>Aim: </strong>To investigate the impact of COVID-19 pandemic on emergency and elective surgical care in Iranian hospitals.</p><p><strong>Methods: </strong>Using an interrupted time series analysis, we evaluated the impact of the COVID-19 pandemic on emergency and elective surgeries in western Islamic Republic of Iran between January 2017 and December 2023, using February 2020 as the intervention point. We analysed the data with R software Version 4.3.2 and used a segmented regression model to analyse the pre- and post-COVID-19 trends, ensuring the reliability of results by using Durbin-Watson statistic and autocorrection techniques. P < 0.05 was considered statistically significant.</p><p><strong>Results: </strong>Pre-pandemic, the mean monthly number of emergency surgeries was 258.9. Due to the pandemic there was a significant decrease by 359.6; however, we observed a gradual recovery in surgical activity with an average increase of 15.8 surgeries above the pre-pandemic levels. The mean monthly number of elective surgeries before the pandemic was 199.5. After the onset of the pandemic, we found a significant decrease of 85.37, although there was a gradual recovery over time.</p><p><strong>Conclusion: </strong>The COVID-19 pandemic significantly impacted surgical services in Iranian hospitals, causing a reallocation of resources to COVID-19 care and postponement of non-urgent surgeries. {"title":"Needs assessment and gap analysis for national health technology assessments in Lebanon.","authors":"Christiane Maskineh, Sizar Akoum, Randa Attieh, Reina Alameddine, Alissar Rady, Michèle Kosremelli Asmar","doi":"10.26719/2025.31.3.176","DOIUrl":"10.26719/2025.31.3.176","url":null,"abstract":"<p><strong>Background: </strong>Developing countries are increasingly adopting health technology assessment as a critical tool for evaluating and addressing challenges related to the use of health technologies in support of Universal Health Coverage, health equity and resource allocation.</p><p><strong>Aim: </strong>To map health technology assessment resources and identify barriers to national health technology assessments in Lebanon.</p><p><strong>Methods: </strong>Using a questionnaire survey and focus group discussions, we collected data from key informants who had interest in health technology assessment in Lebanon between May and July 2017 and in July 2018. We analysed the quantitative data descriptively using Microsoft Excel and the qualitative data using NVivo version 12.</p><p><strong>Results: </strong>Twenty participants completed the questionnaire and 34 attended the focus group discussions. Participants mentioned reimbursement for the use of health technologies and production of clinical guidelines as top priorities for health technology assessment and mentioned the lack of collaboration nationally and lack of agreement among relevant stakeholders as the topmost barriers to assessments.
