Chiesa Fuxench","doi":"10.58744/001c.73861","DOIUrl":"https://doi.org/10.58744/001c.73861","url":null,"abstract":"Psoriasis is a chronic, multisystemic, inflammatory disease that exhibits a wide array of clinical presentations and is associated with high disease burden. Advancements in our understanding of the pathophysiology of psoriasis have led to the development of novel therapeutic modalities known as biologics that inhibit the action of pro-inflammatory cytokines, offering a more targeted therapeutic approach and improved safety profile compared to traditional systemic agents. Psoriasis disease severity is usually the main driver when it comes to selecting the appropriate biologic for any given patient; however, with an increasing number of biologics from which to choose, one pressing question that is increasingly more common in clinical practice is, “How do we select the ‘right’ treatment for each patient?” We performed a review of the literature on biologic use in patients with psoriasis with a focus on specific systemic comorbidities. The act of peer reviewing is reading a manuscript critically to evaluate if the work is clear, relevant to the audience, up to date, and of the quality necessary for publication. Physician assistants with years of clinical experience in a field like dermatology have the aptitude to be peer reviewers. HPV is the most common sexually transmitted infection in the United States and genital warts are sexually acquired low-risk strains of HPV. Although genital warts are not a life-threatening condition, they can be a cause of morbidity and psychosocial distress for patients and their sexual partners. Since genital warts are common in clinical practice with men predominantly presenting to the dermatology clinic, providers are given an important window for treatment, compassionate education with the possibility of vaccination discussion. Camisa","doi":"10.58744/001c.73857","DOIUrl":"https://doi.org/10.58744/001c.73857","url":null,"abstract":"Merkel cell carcinoma (also called primary neuroendocrine carcinoma of the skin) is a rare and aggressive skin cancer. Because of its variable clinical appearance, its diagnosis is often delayed. Here we present an atypical case of this tumor in an elderly Black man who experienced diagnostic delay, and eventually, fatal metastases. Primary neuroendocrine carcinoma is difficult to treat and often requires a multi-disciplinary approach. Traditionally, atopic dermatitis is a clinical diagnosis with distinctive history, morphology, and distribution of pruritic eczematous lesions aiding in determining the condition and treatment. However, multiple other conditions may mimic atopic dermatitis, requiring further workup. We review diagnostic mimickers of atopic dermatitis and divide them into four major categories: immunodeficiencies, malignancies, nutritional deficiencies, and systemic disease. In addition, we organize a workup to be considered if there is a high suspicion for something other than atopic dermatitis.
