Jindal","doi":"10.1093/jsprm/snac018","DOIUrl":"https://doi.org/10.1093/jsprm/snac018","url":null,"abstract":"\u0000 We conceptualized Surgical Accredited and Trained Healthcare Initiative (SATHI) as a community-based worker through which patients who are unaware or undecided of surgical conditions would be linked to service providers. These personnel will help patients to attain the benefits of health schemes and will offer counseling through trust-building and elimination of fear of surgical procedures to reduce the surgical unmet needs. Our baseline study identified five common surgical problems among the study population: extremity injuries/wounds; kidney stones; cataracts; abdominal pain, including external hernias, and menstrual disorders. Based on these findings, we designed a modular, skills-focused curriculum and recruited six full-time staff who have undergone didactic and practical training. Upon finding any suspected health condition, SATHI staff are trained to encourage the patient and their family to seek medical advice. They are also equipped with the basic information about the surgical management of these conditions and will offer preoperative and post-operative surgical care and psychosocial support. This publication describes the process of development of training material, methods and process. Each SATHI worker has been assigned 1000 households (~5000 population). Based on our estimates, he/she will have to cater to 64 cases of unmet surgical needs if the unmet need is 40%. This means a SATHI worker would have approximately five to six cases of unmet surgical needs per month. Though rare, a subset of TSCIs occurs in children, which can lead to long-term comorbidities if not managed within the optimal time frame. However, the lack of pre-hospital care, infrastructure and specialist manpower in LMICs may pose a challenge for health practitioners to provide quality and consistent standard of care to the children. The variety in practice and lack of clarity on management and outcomes of TSCIs in LMICs necessitates an evaluation of the literature. This scoping review protocol outlines how the authors will address the said topic of interest.\u0000 \u0000 \u0000 \u0000 MEDLINE, Embase and Global Index Medicus will be searched from database inception to date in order to identify the relevant studies. Paediatric patients (ages 18 or below) with a TSCI managed in an LMIC country will be included. Surgical and conservative management of TSCIs will be considered. Original research, reviews, commentaries, editorials and case reports will be included.\u0000 \u0000 \u0000 \u0000 Primary outcomes will include TSCI epidemiology, presentation, management, morbidity, mortality and long-term complications. Despite the many recent advancements in increasing neurosurgical access in Africa, published reports have shown that the vast majority of undergraduate students have little or no exposure to neurosurgery. The lack of exposure may pose a challenge in reducing the neurosurgical workforce deficit, which is one of the long-term strategies of tackling the unmet burden of disease. Students may also miss the opportunity to appreciate the specialty and its demands as well as nurture their interest in the field. This study aims to assess the impact of a neurosurgical rotation during medical school in shaping the perception and interest of students towards a career in neurosurgery. Methods The cross-sectional study will be conducted through the dissemination of a self-administered e-survey hosted on Google Forms from 21st February 2021 to 20th March 2021. The survey will contain five-point Likert scale, multiple-choice and free-text questions. The structured questionnaire will have four sections with 27 items: (i) socio-demographic background, (ii) neurosurgical experience, (iii) perception towards a neurosurgical career and (iv) interest in a neurosurgical career. All consenting medical students in African medical schools who are in their clinical years (defined as fourth to sixth years or higher years of study) will be eligible. Odds ratios and their 95% confidence intervals, Wilcoxon rank-sum test, Welch t-test and adjusted logistic regression models will be used to test for associations between independent and dependent variables. Research to date has mainly focused on the extent to which medical schools prepare students for clinical work as junior doctors. However, there remains a need to ascertain how students prepare for their career and what facilitates or hinders learning regarding careers in medicine. The purpose of the XTRA study is to examine career readiness of medical students at UK universities and the support they receive during their studies regarding career planning. Methods: The eXploring medical sTudents caReer reAdiness (XTRA) study is a national cross sectional study of all medical students enrolled at a UK medical school. Data collection will occur via a secure online survey designed as a training need analysis based on the principles of Supers theory (Super, 1953) of career development. A snowball sampling strategy will be used to recruit participants via social media and networks. Results will be analysed using quantitative analysis and thematic analysis to identify themes in qualitative responses. The primary outcome is to understand the perspective of current medical students on how well prepared they are about entering their careers in healthcare. Conclusions: We anticipate that findings from this study will help identify career readiness of medical students to facilitate the development of career development programmes and resources to ensure medical students are well equipped for their future careers. The quality of care patients receive in the prehospital setting and emergency department settings are highly variable and often dependent on computed tomography (CT) availability. The implementation of a handheld device that is able to reliably diagnose intracranial hemorrhage (ICrH) in TBI patients is needed in these settings where CT is unavailable. The Infrascanner has shown promise to detect intracranial bleeds in patients with moderate TBI. We aim to determine the correlation between the coefficient provided by the Infrascanner compared to the post-traumatic supratentorial hemorrhagic collections volume in patients with surgical indication in differente trauma centers. This is a multi-center prospective observational study that will be carried out in three trauma centers in Colombia, Guatemala and Pakistan. We calculated a sample size of eighty-six patients with mild, moderate or severe TBI ≥ 15 years old who have a supratentorial hemorrhage abnormality that requires neurosurgical intervention. The Infrascanner’s ability to detect IChR in patients with surgical indications associated to hemorrhage volume when compared with CT is the primary outcome measure. Secondary outcome measure is the correlation of the mean of the six Infrascanner measurement values compared with volume of ICrH as measured from the six manual head CT segmentation measurements.
