The epidemiological situation of brucellosis in the Russian Federation is characterized as unfavorable. The problem of brucellosis remains of particular relevance in regions with developed animal husbandry. Many different clinical manifestations and the absence of specific symptoms of brucellosis make it difficult to diagnose this disease. The clinical case presented in this paper illustrates the delayed diagnosis of brucellosis in a 10-year-old patient with fever, enlarged lymph nodes, liver and spleen. The defects in the provision of medical care were based on the lack of alertness of doctors regarding brucellosis, insufficient interpretation of epidemiological history data and incorrect assessment of the totality of clinical syndromes of the disease and laboratory parameters. As a result, brucellosis was diagnosed only six months after the onset of the disease. The decedent was nurse, 34-years-old, pregnant, 20 weeks gestation, and working at the district hospital. The pregnant woman died on June 26, 2021 with positive COVID-19 result. Deceased took one dose of COVID-19 vaccine. Delay to diagnosis COVID-19 infection caused death of the pregnant women. Statistical materials were obtained for the period from 2018 to 2021 from contact cards of pregnant women, outpatient cards of HIVinfected partners from discordant couples planning to conceive naturally, and using methods of assisted reproductive technologies. Statistical data processing was carried out using a software package for statistical processing Excel and an online service //math.semestr.ru //. The characteristics of partners in discordant couples by age, epidemiological risk factors, laboratory tests, pregnancy outcomes are given. Conclusion. Patient Z., 37 years old, arrived from the Central African Republic with a diagnosis of \"New coronavirus infection COVID-19, confirmed (PCR RNA SARS-CoV-2 \"+\" from 27.01.22) mild form.\" During an objective examination, the subictericity of the sclera and skin integuments attracted attention, during thermometry - an increase in body temperature to 39.00C. Consciousness at the level of somnolence. Hemodynamics is unstable, episodes of arterial hypotension. Heart rate 96 per minute. Respiratory rate 24 in 1 minute, SpO2 95%, inspiratory dyspnea. According to a laboratory study, severe thrombocytopenia, pronounced signs of hepatic and renal insufficiency, hyperbilirubinemia, an increase in the concentration of C-reactive protein, procalcitonin were revealed. A differential diagnostic search was carried out between acute viral hepatitis and malaria. Blood microscopy revealed young trophozoites of Plasmodium falciparum (++++) using thick drop method. Antimalarial therapy was prescribed with Malacur, which was then replaced with parenteral Quinine followed by mefloquine. In parallel, extracorporeal detoxification operations were carried out. The disease proceeded in a severe form with the development of ophthalmoplegia, tetraparesis, and respiratory failure. Monovalent antitoxic serums against botulinum toxins A, B, and E were administered to the patient. However, on the third day of the disease, he developed fulminant toxic myocarditis with asystole. Despite successful resuscitation, the patient developed a post-resuscitation disease and anoxic encephalopathy. The disease was complicated by severe aspiration pneumonia caused by Klebsiella pneumoniae (ESBL+). The duration of mechanical ventilation was 12 days. His hemodynamics stabilized on the eighth day of illness, and on the 19th day of illness, physical activity and a state of minimal consciousness appeared. The patient was transferred to the rehabilitation department on the 22nd day of the illness. \u0000Doctors should be informed about the possibility of the development and severity of botulism caused by botulinum toxin type F. Taking into account the accumulated positive experience of using bacteriophages, it is advisable to use them for the treatment of patients after determining the sensitivity of each strain of isolated bacteria to bacteriophages. Since, according to numerous sources, Klebsiella pneumoniae bacteria were most often isolated in complicated forms of COVID-19, it became necessary to quickly determine the sensitivity of these and other gram-negative bacteria to the corresponding bacteriophages. The developed express method for determining the sensitivity of gram-negative bacteria to bacteriophages makes it possible to reduce the study time by 6 times, namely up to 3 hours. The diagnosis and selection of adequate therapeutic strategies for the treatment of these forms of infections presents significant difficulties for physicians of therapeutic, neurological profile, immunologists, infectious disease specialists, due to the polysyndromicity and the presence of many clinical masks of these infections. \u0000Aims: determining the prevalence of AHA-HVI among patients infected with herpes virus infections, as well as studying the features of the etiological structure and clinical manifestations/criterion signs of atypically occurring chronic forms of herpes viral infections. \u0000Materials and methods: Under our supervision at the Medsi CDC in Belorusskaya (Moscow) there were 98 patients (SG) of both sexes aged 23 to 60 years suffering from AHA-HVI. The comparison group consisted of 30 conditionally healthy subjects comparable in sex and age to SG patients. In addition to traditional methods (history collection, physical examination methods, CBC, etc.), serodiagnosis methods with ELISA were used to detect herpes-viral infections; PCR method for detecting the genome of viruses in biomaterials. The study was approved by the ethics committee, and all patients received informed consent to participate in the study. Statistical analysis was performed using adequate methods of statistic studies. \u0000Results: The study of the etiological structure of herpes virus infections in patients with AHA-HVI, mixed HVI was shown to occur in 83.4% of patients, and mono-HVI in 16.6% of cases. It was shown that EBV was the dominant virus among patients with both mono and mixed GVI. A high rate of EBV DNA detection was demonstrated in saliva (84.2%), posterior pharyngeal wall scraping (73.5%) and tonsils (42.9%), urine (12.6%) and blood (8.3%) is a marker of high replicative activity of the virus.
