Passengers traveling from meningitis belt countries and arriving in Jeddah, Saudi Arabia should receive one dose of ciprofloxacin upon their arrival if they are not vaccinated with the quadrivalent (ACYW) conjugate meningococcal vaccine. This study aims to assess the knowledge and attitude of those passengers regarding the preventive measures taken at the point of entry toward meningococcal disease. A cross-sectional study was conducted among 394 passengers who arrived from meningitis belt countries in Jeddah, Saudi Arabia from April to June 2022.\u0000The study shows that 51% of the participants were aware of the purpose of ciprofloxacin use, 64.7% of them were vaccinated with the meningococcal vaccine. About 77.2 % of the passengers were not informed about the preventive measures regarding meningococcal disease and the obligation of receiving ciprofloxacin if they are not vaccinated with conjugate type. {"title":"Predictive models of health expenditure via regularization: Do low and upper middle income economies share common predictors?","authors":"Emmanuel Thompson, Faustine Williams","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Countries around the world are presently confronted with gargantuan health care challenges and huge variability in health spending. In the literature, income has been recognized as a crucial predictor of health expenditure. However, there is no agreement on which other variables may be connected to the remaining largely unexplained variation in health expenditure. Therefore, the aim of the present study was to investigate the link between health expenditure and some important predictors among low-income and lower middle-income economies. Regularized regression methods including the Lasso and the Elastic net, and the 2013 World Bank data were used to identify key predictors of health expenditure. The present study showed that the Elastic net algorithm produced a model with a better predictive power than the Lasso in the case of low-income economies. However, the Lasso produced a slightly superior predictive power compared to the Elastic net in the case of lower middle-income economies. Also, income remains a common predictor of health expenditure in both economies. Methods : Pawpaw fruits with lesions or rots were obtained from three different markets in three different states in South Southern Nigeria. Five genera of moulds (Rhizopus, Aspergillus, Fusarium, Curvularia and Trichoderma) were isolated and used for inoculation of sound pawpaw samples at two wound depths of 5 and 7mm with either single or paired cultures. The resultant isolates were evaluated for mycotoxin production using thin layer chromatography and the mycotoxigenic potential of isolates was assessed by animal feeding trial using albino rats of the wistar strain fed orally with different concentrations of the mycotoxin extracts Results : Mycotoxins were detected from pawpaw samples inoculated with Rhizopus, Aspergillus and Fusarium, before and after autoclaving for 15 min at 121 oC. Wistar rats fed on mycotoxin extracts developed symptoms of neurotoxicity characterized by ascending paralysis, convulsion and respiratory arrest. Conclusion : Most of the fungal isolates in this study showed a great potential for mycotoxin production with associated neurotoxicity which is of concern in public health. Methods: The phytochemical analysis of Psidium guajava was carried out by using a standard procedure. Ash, fat, protein, carbohydrate and fibre contents were determined using proximate analysis while the metal contents were determined using Pearson’s method. Results: The phytochemical analysis revealed the presence of saponins, glycosides, terpenoids, anthraquinones, tannins, flavonoids and alkaloids, the proximate analysis showed a low ash value of 2.80%, protein content 2.80%, fibre 2.70% and fat 1.80% but a relatively high content of carbohydrate 88.90%. The metal content analysis revealed the presence of seven metals Calcium 1.34 mg/kg, Magnesium 0.64 mg/kg, Potassium 0.76 mg/kg, Sodium 0.05 mg/kg, Iron 16.18 mg/kg, at concentrations below the tolerable upper intake level except for Manganese 29.23 mg/kg and Zinc 56.49 mg/kg Conclusion: The powdered leaves of Psidium guajava contain more of organic components and levels of manganese and zinc is above the tolerable upper intake established by the overseeing body. Methods: This study was carried out among 254 smokers and 144 nonsmokers from suburban and urban residential areas in Bangkok, Thailand. All anthropometric variables, blood pressures, resting heart rate and biochemical parameters in each subject were measured. Results: The anthropometric variables, biochemical parameters, blood pressures and resting heart rate were not significantly different between smokers and nonsmokers, except for white blood cell count (WBC). Cigarette smoking was associated with increased risk for metabolic syndrome (OR =1.97; 95% CI=1.11-3.42) and the percentages of metabolic syndrome in smoker and nonsmoker Thais were 22.8% and 13.2%, respectively. Moreover, the number of cigarette smoking per day showed significant association with metabolic syndrome (p=0.047). Logistic regression analysis revealed that cigarette pack-years, resting heart rate, body mass index (BMI) and total cholesterol were significantly increased risk factors for metabolic syndrome. Conclusion: The current findings suggest that cigarette smoking is associated with the increased risk of metabolic syndrome by using the modified NCEP/ATP III criteria in Thais. Methods: Ibuprofen (90 g) and the lipid (30 g) were prepared by melt dispersion technique. Conventional granules of ibuprofen were prepared with starch mucilage, 20% w/v. Resulting lipospheres were characterized with respect to sizes, flow property, bulk and tap densities, encapsulated in hard gelatin capsules and evaluated for drug release profiles. Results: Dissolution profile for lipospheres were a maximum drug release of 97% in 1 hr (conventional granules), 23% in 4 hr (GF), 60% in 2 hr (CW) and 40% in admixtures of fats (GC). Admixing the fats enhanced flow properties of the lipospheres. Inclusion of a surfactant enhanced the release profiles from the lipospheres. Conclusion: Formulation of ibuprofen into lipospheres modified the release profile, which has implications in the formulation of sustained release multiunit dosage forms. The bimonthly drug requisition forms and the amount supplied by medical supply department (MSD) were collected and reviewed. Results: Total number of prescriptions issued for the drugs under examination was 29113. 26551 prescriptions were dispensed while the remaining 2562 (9%) were undispensed due to drug unavailability. The difference between quantity of drugs requested by the pharmacy and quantity issued from MSD was 47-52%. Conclusion: Mismatch between drug demand and supply in the facility studied is the main cause of shortage. Methods: The survey took place at the Yaounde University Teaching Hospital from May to June 2008. Serum samples were collected from 110 pregnant women attending the ante natal clinic using aseptic techniques after obtaining informed consent. The samples were analysed using toxo-lgG immunocomb and toxo-IgM “capture” ELISA. A structured questionnaire was used to collect information on predisposing risk factors for toxoplamosis from each patient. Data was analysed on Epi-Info using confidence intervals and chi-square statistic test. Results: The average age of the women was 27.9+5.8years and the mean gestational age was 4.1+0.2months. The overall IgG seroprevalence was 65.5% {95% CI: 53.7-71.7%} and that of IgG and IgM co-infection was 2.7%. The seroprevalence was 75% in the first trimester, 60.6% in the second trimester and 50% in the third trimester. No statistically significant relationships were established between anti-toxoplasma IgG and IgM antibodies and abortion history, meat consumption, potable water sources, cat ownership and age. Conclusion: The prevalence of IgG antibodies to Toxoplasma gondii is high and the first trimester in pregnancy carries the highest risk. All pregnant women should be screened for toxoplasmosis and educated on predisposing risk factors during antenatal visits.
