By the 3 rd of May 2020, a total of 28 patients have been admitted to the Turku University Hospital. We explore baseline characteristics in order to identify risk for severe disease and critical care admission. Methods: For this retrospective cohort study, data were derived from hospital records. Basic descriptive statistics were used to characterise patients, including medians, percentiles and frequencies. Differences were tested with Mann Whitney U-test and Pearson’s chi-square test. Results: Pre-existent obstructive sleep apnea (OSA) was present in 29% of patients admitted in the hospital for Covid-19. Overall, other findings on admission were comparable with those reported elsewhere. C-reactive protein and procalcitonin were higher in patients who were eventually transferred to critical care in comparison to in those who were not (median CRP 187 mg/L versus 52 mg/L, p<0.005 and median PCT 0.46 versus 0.12, p=0.047). Conclusion: OSA was pre-existent in a disproportional large group of patients, which suggests that it is an important risk factor for severe Covid-19. It was declared as public Health Emergency of international concern (PHEIC) by the World Health Organization on 30th Jan 2020,1 and a pandemic on 11th March 2020.2 As of time of writing this report, there are >20 million confirmed cases of COVID-19, and >750,000 confirmed deaths globally reported by WHO. As per CDC USA data reported on August 18, 2020, there are 5,422,242 cases reported in United States with 169,870 deaths. It has long been known that incidence of mental health problems including depression, anxiety, posttraumatic stress, and sleep disturbances rises during disease pandemics/epidemics.3 Given the uniquely high impact of the current pandemic, it should hardly be surprising if a similar, or indeed worse, pattern be observed during the COVID-19 pandemic. Moreover, these mental health problems likely have long-lasting affect, beyond the duration of the pandemic.4 Beyond the obvious cost in terms of mortality and morbidity due to the infection itself, the unprecedented spread of the pandemic has caused immense socioeconomic strain due to mandatory lockdowns across the developed and the developing world. Any change in lifestyle can lead to changes in this cycle and, consequently, in sleep.1 The National Cancer Institute José Alencar Gomes da Silva (INCA) describes cancer as a disorderly growth of cells that invade tissues and organs. These cells tend to be very aggressive and uncontrollable, being able to generate tumors and spread to other regions of the body.2 According to INCA, an estimated 625 thousand new cases of cancer in Brazil, in the years 2020/2022. It was alarming for mental health service when it started being used as addictive agent by patients for its hypnotic properties. Methods: We report two cases from Community Mental Health Team who started abusing their prescribed Pregabalin. They were closely observed in community and on inpatient unit for their pattern and extent of drug abuse and dependence criteria. Results: Both the patients developed dependence syndrome indicated by tolerance, craving and presented with withdrawal symptoms on stopping Pregabalin. They needed replacement medication to manage withdrawal symptoms. Conclusion: Pregabalin has strong addictive potential and mostly abused by people for its hypnotic or sleep-inducing properties. {"title":"Heart rate variability in children with obstructive sleep apnea: a systematic review","authors":"Vanessa Cristina Cunha Sequeira","doi":"10.15406/smdij.2020.04.00067","DOIUrl":"https://doi.org/10.15406/smdij.2020.04.00067","url":null,"abstract":"Obstructive Sleep Apnea (OSA) is a common respiratory disorder characterized by recurrent nocturnal episodes of breathing interruption due to the total or partial collapse of upper airway1 and affects 1.25.7% of children.2 The peak of incidence occurs at 2-8 years and the most common cause of OSA in children is enlarged tonsils and adenoids.2,3 Moreover, OSA is associated with morbidity that involves central nervous system (CNS), cardiovascular and metabolic systems.4 The recurrent hypoxia is suggested as the main cause of altered autonomic nervous system (ANS) in those patients.5,6 The obstructive episodes lead not only to hypoxia, but also intermittent hypercapnia, which can modulate the autonomic nervous system of OSA patients. Main consequences of hypoxia and hypercapnia include increased of sympathetic tone and sympathetic responsiveness as well as sympathetic-parasympathetic imbalance.4 There is also evidence that OSA alters heart rate response and systemic blood pressure, which can contribute to cardiovascular morbidities when adults.5 To evaluate the integrity of ANS is commonly used a non-invasive method named heart rate variability (HRV). HRV represents the oscilations of heart rate (HR) and it’s considered a measure of neurocardiac function that reflects ANS dynamics.7 Some pathologies, such as respiratory and cardiovascular diseases, shows lower HRV pattern, which is consistent with and abnormal adaptability of ANS, whereas healthy individuals exhibit higher HRV indices meaning an ANS adaptable and dynamically responsive to change.8 To address this issue, the aim of this study was to review whether heart rate variability was altered in children with obstructive sleep apnea. It is also a quite common underlying factor in resistant HTN (RHTN). The main etiological factor of OSA is obesity, which is a rapidly growing global epidemic. To control obesity, patients should be encouraged by health care professionals to lose weight and be educated about weight loss strategies such as lifestyle modifications, which include regular exercise, low-calorie diet, low sodium intake, smoking cessation, and decreased alcohol consumption. This review also emphasizes the importance of screening for OSA as the major underlying cause of essential, and RHTN, which can lead to CVD and can cause end-organ damage. It also stresses the importance of using continuous positive airway pressure (CPAP) and its beneficial effects, along with other antihypertensive regimens, in treating HTN, and RHTN. It may affect children, especially boys who also have a diagnosis of attention deficit hyperactivity disorder (ADHD). It is also often diagnosed through adolescence and into early adulthood. Among adults, it affects women and men equally. It occurs in 0.5-1% of people in a given year. OCD is characterized by two features: obsessions and compulsions. Obsessions in OCDs are persistent thoughts, images, or impulses that seem intrusive or inappropriate by others. These ideas are often associated with anxiety or distress. One common obsession relates to cleanliness and a belief that objects are contaminated with germs. Other compulsive thoughts in people with OCD are that they think that a task was not completed or done inappropriately. For example, there may be anxiety about turning off the stove or locking the door, even though these were in fact safely secured. Apparently an important goal of deliberation on that topic is to recognize the beliefs and motives for action of the healthy and unhealthy in the areas of life, which considerably influence the health. According to experts, people’s attitudes in respect of health care are determined by a number of factors. Individual choice of the kind of adopted behaviour largely depends on the level of general knowledge, particularly knowledge on health protection, as well as experience, motivation, recognized values and set objectives.
