Methods: Forty-seven, non-diabetic female participants aged 19 and older with FMS completed the Mental Clutter Scale, the Food Craving Inventory, the Profile of Mood States and the Fibromyalgia Impact Questionnaire-Revised. Results: Carbohydrate cravings were found to be a unique correlate of Fibrofog compared to other specific cravings. Results from hierarchal models revealed that carbohydrate cravings independently predicted Fibrofog, albeit modestly, over and above the symptoms of physical impact and mood (even after controlling for BMI). An effective screening tool to assess GJH may influence our understanding and choice of management. Diagnosis is clinical, using tools such as the Beighton Hypermobility Score and the Contompasis Scoring System. The comparable reliability of these tools has not been previously reported. The aim of the present study was to compare the intra- and the inter-rater reliability of the Beighton Score to the Contompasis Score to assess GJH. Methods: This was an observational study assessing 36 pain-free participants; 27 females and nine males; aged 18–32 years. Participants were assessed in random order, by two researchers over two sessions to determine intra- and inter-rater analyses. Intraclass Correlation Coefficient [ICC] and weighted Kappa statistics were used to calculate the level of agreement. Results: The intra- [ICC: 0.71–0.82] and the inter- [ICC: 0.72–0.80] rater reliability of the Beighton Score was substantial to almost perfect. The Contompasis Score displayed substantial to almost perfect intra-rater [ICC: 0.73–0.82] reliability and moderate to substantial inter-rater [ICC: 0.58–0.62] reliability. Conclusions: The present study provides an indication of the measurement capabilities of the Beighton and Contompasis Scores. Methods: C-reactive protein obtained before the start of immunosuppressive therapy was analyzed retrospectively in 61 therapy naïve patients [PM n = 27, DM n = 15, Jo-1 n = 5, sIBM n = 14]. All patients had a diagnosis of idiopathic inflammatory myopathy confirmed by muscle biopsy. Results: C-reactive protein was significantly higher in DM and Jo-1 compared to PM and sIBM [ANOVA p < 0.001]. The CRP levels less than twice the upper limit were found in 79% of sIBM patients, 85% of PM patients, 20% Jo-1 of patients, 53% of DM patients. C-reactive protein showed no correlation with creatine kinase and erythrocyte sedimentation rate. Material and method: A total of 36 patients and 31 healthy controls were enrolled. Demographic characteristics were recorded and the patients were evaluated according to the Beighton score and Brighton criteria for BJHS. The assessment of pain was performed based on the visual analogue scale [VAS 0–10 cm]. Function and QoL were assessed using the Stanford Health Assessment Questionnaire [HAQ] and Nottingham Health Profile [NHP], respectively. Results: The mean age was 29.94 ± 6.04 years in the patient group and 30.16 ± 5.64 years in the control group. When the two groups were compared, there were significant differences in the HAQ scores. QoL was also significantly worse in the patient group in all dimensions of the NHP except for social isolation. There was a positive correlation between the Beighton score and the NHP-emotional reaction score. We have also observed a positive correlation between the HAQ scores and the pain [VAS], NHP-pain and NHP-Physical Mobility scores in the patient group. No correlation was observed among the other parameters. Conclusion: This study has demonstrated that function and QoL are significantly impaired in the patients with BJHS compared to healthy controls. Function and QoL as well as the pain should be assessed in the patients with BJHS. Findings: Nineteen randomized controlled trials [N = 1077 women] satisfied the inclusion criteria. Methodological quality of papers was assessed using the PEDro scores. Data on the study, subject, and exercise program characteristics as well as assessment of changes in depression, fatigue, global well-being [overall feeling of well-being and health-related quality of life], pain, sleep, and stiffness were extracted. The studies were grouped according to the intervention program: land interventions [aerobic, combined [aerobic endurance, strength, and flexibility], vibrations, and alternative programs], and aquatic interventions. Nineteen studies were selected for systematic review, but clinical heterogeneity limited the meta-analysis to two aerobic, three combined, two alternative, and five aquatic studies. Conclusions: In general, exercise programs have a positive effect on the symptoms of fibromyalgia in women in peri-menopausal age. The meta-analysis indicates that programs based on combined exercise and aquatic exercises have, respectively, a moderate [d = −0.63; I2 = 0%] and small effect [d = −0.41; I2 = 30%] on functional global well-being [assessed using the Fibromyalgia Impact Questionnaire total score]. Methods: We performed a cross-sectional analysis in the community-based Johnston County Osteoarthritis Project, a cohort that is representative of the US population, utilizing data from 1672 participants with a mean age of 68 years; 69% were white and 68% were women. Trained staff obtained data on participant-reported: symptoms, comorbidities, depression and functional status; and performance-based functional assessments. Regression models of neck and shoulder symptoms and functional measures were adjusted for age, sex, race and body mass index, and additionally for other joint symptoms and comorbidities. Results: Symptoms of neck [8%], shoulder [13%] or both [13%] were reported by participants. Neck symptoms were most frequently reported by White women; shoulder symptoms were evenly distributed among race and gender subgroups. Neck and shoulder symptoms were associated with cancer, diabetes mellitus [DM], depression and lung, cardiovascular and other musculoskeletal problems, as well as pain, aching or stiffness at other sites, and independently with self-reported and performance-based functional measures. Nijs","doi":"10.3109/10582452.2015.1132027","DOIUrl":"https://doi.org/10.3109/10582452.2015.1132027","url":null,"abstract":"Abstract Objective: Illness perceptions, i.e. how patients think about their illness in terms of identity, cause and consequences, are important, as negative illness perceptions are associated with maladaptive illness behavior, more dysfunctioning, poor treatment adherence and treatment outcome. As illness perceptions are influenced by cultural background, former experiences and provided information, the aim of this study was to compare illness perceptions of patients with fibromyalgia syndrome [FMS] living in different countries. Methods: Dutch speaking participants from two countries were recruited in this international comparative study. In total, 114 Belgian [Flemish] and 283 Dutch FMS patients completed the Illness Perception Questionnaire for Fibromyalgia-Dutch Language version [IPQ-R FM-Dlv]. The Mann–Whitney U test for non-parametric data was used to compare data between the patient groups. Pearson’s chi-squared test was used to compare proportions. Results: Significant differences in illness perceptions were observed. Belgian patients reported a higher variety of symptoms and believed more strongly that these symptoms are related with their illness. They had more negative beliefs concerning the consequences of their illness and reported more external attributed factors to be the cause of their illness. Dutch patients demonstrated a better understanding and controllability. They indicated more internal attributed factors as a cause. Conclusions: These findings suggest that illness perceptions in patients with FMS are prone to local influences, including small differences in the health care system.
