Despite the burden of disease, there is still no consensus on the underlying pathophysiology leading to sporadic AD. The previously held amyloid-beta (Aβ) hypothesis is giving way to a multifactorial model that involves various pathologic components, including Aβ and tau. In this paradigm, other components including inflammatory processes (i.e. microglia), structural components (blood brain barrier), and the amyloid precursor protein (APP) play significant roles in disease. We present here a review on the emerging role of APP in AD, independent of Aβ, including its significance in normal physiology and disease. We briefly describe the various isoforms of APP, its processing pathways, and how this contributes to function. Furthermore, we present our own observations and discussion on APP pathology in AD human tissue. Finally, we combine this knowledge of APP to suggest an interaction with tau that could be applied to AD and other tauopathies such as chronic traumatic encephalopathy. This study was designed to investigate in Parkinson’s patients, whether time-related changes in striatal dopamine transporter availability are associated to the appearance of dyskinesias. 15 Parkinson’s patients had dopamine transporter-specific SPECT imaging with 123I-FP-CIT twice: at baseline (when they were drug naïve) and at follow-up (6.31 ± 2.29 years from baseline), and were followed up clinically every six months. At the end of the study, patients were divided in two groups according to whether they had developed dyskinesias or not. Semiquantification of 123I-FP-CIT data was performed using the occipital cortex as the reference region. Specific binding ratios were calculated for the putamen and the caudate. During the clinical follow-up, all Parkinson’s patients were treated pharmaceutically. 8 patients developed dyskinesias, while 7 remained nondyskinetic. At baseline, the two groups had similar 123I-FP-CIT specific binding ratio values for the putamen and the caudate (p > 0.05). Also, between-group differences in age, disease duration, and Hoehn & Yahr scores were not statistically significant. Overtime, the putaminal 123I-FP-CIT specific binding ratio values in the dyskinetic group decreased significantly (p < 0.01). The nondyskinetic patients had smaller reductions (p < 0.05) during the same period of time. At follow-up, the dyskinetic patients had significantly higher Hoehn & Yahr scores (p < 0.01) and were taking higher levodopa equivalent doses (p < 0.001), as compared to the nondyskinetic patients. Essential tremor (ET) and Parkinson-related tremor are two of the most common forms encountered. Earlier double blind and placebo-controlled studies had suggested efficacy of BoNT-A injections into the forearm muscles in reducing the hand and forearm tremor in patients with essential tremor (ET). High quality publications over the past two years have established the efficacy of botulinum toxin-A therapy for ET as well as demonstrating probable efficacy of BoNT-A injections in Parkinson disease tremor. He had low intelligence (IQ = 68), schizoid personality traits and first degree relatives with schizophrenia. At the age of 27, he experienced stereotyped spitting behavior, bizarre facial grimacing and left persistent clenched fist posturing. Neurological evaluation revealed left hand posturing, atrophy and nail injuries of the hand flesh with fungal infection and motor apraxia. Mogensen","doi":"10.17756/jnen.2019-059","DOIUrl":"https://doi.org/10.17756/jnen.2019-059","url":null,"abstract":"Background and Objectives: To outline experimental variables of relevance to clinical trials employing active control condition (ACC) designs within cognitive rehabilitation after Acquired Brain Injury (ABI), and to qualitatively review the current methodology of trials within this field. Method: Firstly, experimental variables which evidently impact rehabilitation outcome were outlined, based on the authors’ joint knowledge of the research field. Secondly, clinical trials within cognitive rehabilitation after ABI that employed ACC designs were identified and qualitatively evaluated based on the outlined variables. Due to the breadth of this research field, a focus on computer-based cognitive rehabilitation (CBCR) was chosen. Trials were identified through a systematic search in prespecified scientific databases using prespecified inclusion criteria. Results: 10 experimental variables were identified. Nine trials were included in this review. None of the included trials accounted for all experimental variables in their study design. Conclusion: There are methodological shortcomings in the design of trials using ACC’s within the field of CBCR after ABI. Although this methodological review is limited to a subfield within cognitive rehabilitation research, the conclusions drawn here are believed to generalize to the overall field.
