The overall research Question for the integrative study is: What is the evidence on interventions to initiate or enhance individuals’ self-care activities across population groups in the Cochrane repository of systematic reviews? Criteria for considering studies for this review: Types of participants: This integrative study will consider systematic reviews that focus on individuals who engage in or have the potential to engage in self-care activities. For example, individuals who have the potential to engage in a new (perhaps specific) self-care activity would be individuals recently diagnosed with a chronic disease such as asthma or diabetes mellitus. Individuals who currently engage in self-care activities (but may require ongoing support or encouragement) would be individuals who currently perform self-care activities to address chronic disease, injury or disability. Changes in life, such as a decrease in visual acuity with aging, may require an adaptation of self-care activities. Types of interventions)/phenomena of interest: This integrative study will consider systematic reviews that describe intervention strategies focused on initiating or enhancing self-care activities. Types of outcomes: Outcomes of interest for this study include: the successful engagement of the individual in self-care activities` the sustainment of self-care activities` or health outcomes. The successful engagement in self-care activities would be indicated by the individual actively performing all (or some aspects) of a new self-care behaviour. For example, a child recently diagnosed with diabetes mellitus may begin to check their own blood sugar levels but rely on a parent to inject the insulin. The sustainment of self-care activities would be indicated by the individual actively continuing to perform a self-care behaviour that was already part of their repertoire. For example, an adolescent diagnosed with asthma as a child and who continues to use an inhaler to prevent asthmatic attacks. These outcomes would generally be measured by observation or self-report through Questionnaires or participant diaries. {"title":"A systematic review on the effectiveness of nurse-led cardiac clinics for adult patients with coronary heart disease.","authors":"Verena Schadewaldt, T. Schultz","doi":"10.11124/01938924-201008020-00001","DOIUrl":"https://doi.org/10.11124/01938924-201008020-00001","url":null,"abstract":"Aim Nurse‐led clinics offer the possibility of patient education, support and continuity of care for patients with Coronary Heart Disease (CHD) to address and reduce risk factors. The objective of this review is to present the best available evidence related to nurse‐led cardiac clinics for adults with CHD by updating an existing systematic review. Methods Twenty databases for published literature and 11 databases or websites for unpublished literature were searched for the period September 2002 to March 2008. Additionally, reference lists of retrieved articles were reviewed and relevant journals were hand searched. Assessment for methodological quality, data extraction and synthesis was undertaken using a systematic review management tool (JBI‐SUMARI). Results Seven RCTs formed the basis for this systematic review, however, in many cases meta‐analysis was not possible because of a lack of relevant data. The major nurse‐led intervention applied in the clinics consisted of health promotion through educating and counselling the patients. There were no harmful effects on patients with CHD identified when exposed to a nurse‐led clinic. Few risk factors were significantly reduced by attending nurse‐led clinic, but particular in the long‐term no success in risk factor reduction was achieved. Difficulties were obvious in modifying behaviour changes concerning smoking cessation and diet adherence. Nurse‐led clinics may positively influence perceived quality of life and general health status, especially physical functioning. Conclusion The results indicated that care was equivalent to non‐nurse led clinics, and there was no greater risk of poorer outcomes in the nurse‐led clinics. The effectiveness of clinics might be dependent on the intensity of the nursing support. The combination of counselling and regular assessment of risk factors and health status delivered at nurse‐led clinics is supported by the available research, and given that outcomes were in general equivalent between nurse‐led and other clinics, further research should investigate the cost effectiveness of the different models of care. Implication for practice Before establishing a nurse‐led clinic appropriate qualification and responsibilities, as well as the particular structure of the health care system and funding possibilities have to be considered. Implication for research Additional research on the effectiveness of selected interventions in nurse‐led clinics is necessary. Anche se la contenzione fisica è spesso considerata una soluzione \"semplice\", secondo molti autori i benefici sono dubbi in quanto può aumentare l'agitazione e ci possono essere devastanti effetti fisici e psicologici sui pazienti. Search Strategy E' stata fatta una ricerca della letteratura utilizzando i seguenti database: MEDLINE, CINAHL, EMBASE, COHCRANE Library. Sono stati cercati anche studi non pubblicati, non sono stati posti limiti temporali né linguistici alla ricerca. Methods E' stata condotta una valutazione critica degli studi non pubblicati usando gli strumenti del software JBI-MAStARI. Main results (before data extraction) Sono stati inclusi in questa revisione 6 studi osservazionali; sebbene l'ambito di interesse fosse l'autorimozione di qualsiasi presidio nei pazienti adulti critici sottoposti a contenzione fisica, gli studi inclusi hanno trattato solo l'estubazione non pianificata.
