The Effect of Mindfulness Intervention on Job Stress in Nurses of Intensive Care Units 正念干预对重症监护护士工作压力的影响

One of the appropriate methods for decreasing stress is mindfulness training, therefore, this study aimed at assessing the effect of mindfulness training on nurses’ job stress level. Methods: The present study was conducted in intensive care unit of Imam Hussein hospital (SA), a subspecialty hospital in Kermanshah in 2016. This was a preand posttest study with a control group in which 60 nurses were randomly and equally placed into 2 groups (control and experimental). Eight 90minute mindfulness training sessions, one session per week, were provided to the experimental group. After the training sessions, the scale-revised questionnaire on nursing job stress was given to both control and experimental groups; and then, the effect of intervention was measured and compared between the 2 groups. Data analysis was done through descriptive and inferential statistical tests using SPSS 16 software. Results: A significant difference was found between average± standard deviation of stress scores in the control group before (159.57 ± 20.56) and after (171.63± 21.39) mindfulness intervention (P = 0.001). Also, a significant difference was obtained between average ± standard deviation of stress scores in the experimental group before (165.28 ± 21.35) and after (118.20 ± 17.52) mindfulness intervention (P = 0.001). However, no significant difference was detected between the control (159.57± 20.56) and experimental (165.28 ± 21.35) groups in average ± standard deviation of stress scores before mindfulness intervention (P = 0.31). A significant statistical difference was found between the experimental (118.20 ± 17.52) and control (171.63 ± 21.39) groups after training (P = 0.001). Conclusions: Mindfulness intervention can decrease job stress of the nurses who work in intensive care units. Therefore, the present study aimed to evaluate the effect of Burn’s wean assessment program on the success rate of weaning from ventilator in patients admitted to the Intensive care unit (ICU). Methods: The present experimental study was carried out on 100 patients undergoing MV for more than 72 hours. The patients were selected by convenience sampling and randomly divided into two groups of 50 subjects. Burn’s wean assessment program was applied to the intervention group, while weaning was assessed by a physician in the control group. Results: 35 patients in the intervention group were male and 15 were female. In the control group, 36 were male and 14 were female. Chi-square test showed no statistically significant difference between the two groups (P < 0.826). In the intervention group, 46 patients had successful weaning (92%) and 4 patients had unsuccessful weaning (8%). In the control group, 35 patients had successful weaning (70%) and 15 patients had unsuccessful weaning (30%). Chi-square test showed a statistically significant difference between the two groups (P < 0.005). Therefore, poor nutrition leads to decreased life quality and increased complications of hemodialysis as well as mortality in these patients. This study aimed at investigating the effect of nutrition education program (NEP) on biochemical parameters among patients undergoing hemodialysis. Methods: This quasi-experimental study was conducted on 30 patients who referred to hemodialysis unit at Ayatollah Taleghani hospital in Urmia province, Iran. The sampling method was purposive. Data were collected through interviews and data collection instruments consisted of demographic questionnaires and checklist. In the pre-education stage, the demographic questionnaire was completed and a 2month average of biochemical parameters (sodium, potassium, calcium, phosphor, blood urea nitrogen, and creatinine) were measured and recorded in the checklist by the researcher. Then, the patients received four 30minute educational sessions on nutrition using learning assistant tools. Two months after the education program, the laboratory results were measured and recorded again by the researcher. Data analysis was performed using SPSS 14 and paired t test, and significance level was set at 0.05 (P < 0.5). Results: The findings revealed a significant decrease in sodium, potassium, calcium, blood urea nitrogen (P < 0.05), but no significant difference was found in the laboratory results of phosphor and creatinine (P < 0.05). Conclusions: The implementation of nutrition education program (NEP) for patients under hemodialysis can improve biochemical parameters as well as complications resulting from their imbalance. Objectives: The aims of the present study were to determine the prevalence of suicide attempt in psychiatric patients, and determine its associations with clinical and sociodemographic factors, and gender differences. Methods: Participants in this descriptive-analytic study were 941 patients who were hospitalized for at least 2 days in Razi center in Tehran in 2010. Univariate and multivariate logistic regressions were used to analyze the factors related to suicide by SPSS software Version 17. Results: Almost 23.4% of the participants had a history of suicide attempt. Of the 250 females, 42 and of the 691 males, 178 attempted suicide. Suicide attempt in divorced, separated, and widowed females was much higher than in single females (P = 0.032). Also, those females who were drug abusers attempted suicide more often compared to non-users (P = 0.047). Suicide attempt was more prevalent in young males (P < 0.001). In addition, bipolar disorder (P = 0.011), depression (P = 0.27), drug abuse (P < 0.001), and alcohol abuse (P = 0.017) were risk factors of suicide attempt among males. Interactions between gender and marital status (P = 0.002) and between gender and the type of psychiatric disorder (P = 0.004) were statistically significant. Documentation of such a pattern may contribute to identify new pathophysiological factors in the occurrence of MI. Therefore, the current study aimed at determining the association between MI and the admission season and time. Methods: The current cross sectional, retrospective study was conducted on 286 patients diagnosed with MI admitted to Imam Sajjad (a.s) hospital in Ramsar, Iran, in a 5-year period from 01 July, 2012 to 31 July, 2016. Data of patients including age, gender, and admission hours, months, and seasons were collected from their records. Information on temperature and humidity was collected from Ramsar weather forecast center (Iran). Data analysis was done by SPSS version 18 through statistical tests. Results: The mean age of the study subjects was 68.12 ± 6.44 years. The highest admission rate was in summer (30.1%) in terms of season, and it was 35% at 12:00 P.M to 6:00 P.M in terms of time. Admission peak hours were at 10:00 AM, 11:00 AM, 1:00 P.M, and 2:00 P.M, respectively. There was significant statistical difference between MI in terms of season, and temperature and humidity average (P < 0.00). Conclusions: There was a seasonal rhythm in admissions for acute MI in the study with an increase in summer and a decrease in winter. In most of the studies regarding the effect of seasonal variations on the occurrence of acute MI, only the effect of temperature was studied. Maggot debridement therapy (MDT) offers important advantages for the management of chronic and infected wounds and is used in hundreds of clinics worldwide. This study aimed to evaluate the benefit of maggot debridement therapy (MDT) in the treatment of a patient with a diabetic foot ulcer (DFU). Case Presentation: A 46-year-old male patient with a 6-year-old diabetic foot wound in the right leg. Maggot larvae were placed on the wound. The patient was followed for 6 weeks and luckily had a good response to treatment. Conclusions: This study concluded that MDT is as powerful as traditional debridement in the treatment of diabetic foot ulcers. It would be an achievable different option for diabetic foot ulcer treatment. The use of evidence in nursing practice is not only a duty, but also a professional responsibility and practice. Objectives: The current study aimed at assessing the effects of an evidence-based practice training workshop on the information literacy of critical care nurses. Methods: The current 2-group, pretest-posttest, quasi-experimental study was conducted in 2016. A convenient sample of 90 critical care nurses was selected. Nurses were non-randomly allocated to a control and an experimental group 45 subject each group. An 8hour evidence-based practice training workshop was held in 2 consecutive days for nurses in the experimental group, while nurses in the control group received no evidence-based practice training. A researcher-made valid and reliable questionnaire was used to measure the information literacy in nurses. The collected data were analyzed using the SPSS software version 19, and via running the Fisher exact, the pairedand independent-sample t tests. Results: At baseline, the mean score of information literacy in the control group did not differ significantly from that of the experimental group (9.668 ± 5.11 vs. 8.84 ± 4.37; P value = 0.40). After the study, these values increased to 10.51 ± 5.03 and 23.75 ± 3.07, respectively. Intergroup differences regarding the posttest information literacy score was statistically significant (P value < 0.001). Objectives: The present study aimed at determining the rate of time management in Iranian critical care nurses and its related factors. Methods: This descriptive-analytical study was conducted on critical care nurses of hospitals affiliated to Mazandaran University of Medical Sciences in 2016. Of 500 nurses from all the selected hospitals, 410 ones accepted to participate in the study, with response rate of 91.1%. The instruments for data collection were demographic characteristics and time management questionnaires. Data were analyzed by descriptive and inferential statistic tests (Pearson correlation, one-way ANOVA, independent sample t test, and General Linear Model). Results: In this study, 100 male (24.39%) and 310 female nurses (75.6%) took part, with the average age of 37.58 ± 7.37 (CI95: 36.82 38.33). The rate of time management in the nurses under the study was 107.68 ± 18.011 (CI95: 105.83 109.54). Moreover, 35 (8.5%), 275 (67.07%), and 100 nurses (24.39%) had low, medium and good levels of time management, respectively. A significant relationship was observed between time management and age (r = 0.506, P < 0.001), educational level (t = 2.06, P = 0.003), and gender (t = 1.583, P = 0.03). Conclusions: Time management is necessary for medical workers, especially nurses. Lactose, composed of 50% glucose and 50% galactose is the unique means Nature offers to the newborn, who in absence of modern care suffers from severe bioenergetic impairment, similarly to adults resuscitated after cardiac arrest. Aim of this study is to review these issues, to understand how we may improve outcomes in intensive care units. Evidence Acquisition: A review study was conducted in 2017 through searching on Science Direct, PubMed, Wiley, and Blackwell databases. The search was performed using bioenergetics, cardiac arrest, critical illness, hyperglycemia, glucose, galactose, newborn and resuscitation, as key words. Finally, 24 articles in English were assessed in this study, thereby comprised 2 guidelines (2015 American heart association and guidelines for both Adult and Neonatal Resuscitation. Results: Correct glycemic control strategy in extreme ATP deficit conditions, such as after resuscitation from a cardiac arrest or a complicated or preterm birth can improve outcome. 2015 American heart association (AHA) guidelines do not recommend glucose infusion after cardiac arrest. Data on glucose administration following brain insult in newborn are limited. Outcomes of applying the hypothesis allowed to assess that glucose, an excellent substrate, turns to a harmful one, able to worsen brain outcome, likely due to its needing phosphorylation prior to be utilized. Patients with end-stage diseases usually have painful experiences at the end of life. Provision of adequate supportive and palliative care is a major issue in nursing. The integration of religious concepts in new nursing principles is an interesting phenomenon, which can be effective (1). In this regard, Bahar et al. noted that “nurses should be closely acquainted with the prevailing traditions and religious beliefs in the communities where they serve” (2). Here, the authors, as a holistic care team, aimed to discuss the practice in a rural area in northeastern region of Thailand, a tropical country in Southeast Asia. Although Buddhism is deeply rooted in Thailand, a Buddhist approach to spiritual care has been only applied for a few years and it is considered as a new phenomenon. As a Buddhist country, local patients usually have faith in Buddhist principles and hold religious amulets for psychological support at hospitals. The patients’ relatives usually bring different amulets to the patient’s bed and pray for healing through superstitious powers. Accordingly, local hospitals integrate the local belief in the concept of spiritual care to support patients at the end of life. At hospitals, specialized nursing teams are appointed to pray for the patients. The patients respond to the prayers as they start to feel more relaxed, and the relatives feel content to hear the prayers. In fact, this confirms the concept proposed by Fournier on “creating a sacred place in the intensive care unit at the end of life” (3). Praying for patients at the end of life has been shown to be an effective nursing practice, supporting both patients and their relatives. Masa’Deh et al. noted that “most family members want this option for some religious and cultural reasons such as praying and supplicating to support their loved one” (4). There are various “dying care” interventions (5), some of which are complex and require additional instruments in need of systematic evaluation (5). In comparison with other techniques, praying can be simple and easy-to-practice. It requires no complex tools and imposes no costs on anyone. It is also understandable and involves the patient’s family and relatives, as an important neglected part in “dying care” interventions (6). This process can help support both patients and their relatives. In addition, it is a way to create a “sacred space” (3) in the care unit, and at the same time, nurses can practice some good deed in line with their religious beliefs.
