Methods: Between January 2000 and January 2018, 28 patients (20 males and 8 females) with big defects following infection and post traumatic injury of the tibia were treated. The mean age of the patients at the time of surgery was 15.3 years (4-24 years). The fibula was mobilized medially to fill the defect and was fixed with Ilizarov fixator. The average size of the defects reconstructed was 7.35 cm (6-10cm). Patients were evaluated functionally using the ASAMI scoring system. Results: The mean follow-up period was 29.03 months (range, 15-60). The average time for complete union was 6.8 months (range, 5-9 months). At final follow-up all patients had fully united. We found leg length discrepancy in 22 patients and that was corrected by re- lengthening of the solid new regenerate bone. Conclusion: Ipsilateral fibular centralization or medialization is an excellent reconstruction for big tibial defects after wide debridement with very good acceptable functional outcome. {"title":"Surgical treatment of thalamic fractures treated by anatomic plate with survevent","authors":"L. K., S. B, S. S, D. They represent 65% of tarsal trauma and 2% of all fractures. The thalamic fractures constitute a particular entity by their mechanism of occurrence, by their treatment and by their prognosis much less favorable than the extra-articular fractures.1 Materials and methods: Our series spanning a period of 7 years from January 2011 to November 2018 focused on 12 patients with a thalamic fracture of the calcaneus treated by anatomical plate with bearing, collected in the Orthopedic Trauma Department A of the Hassan II Fez University Hospital. All adult patients with a thalamic calcaneal fracture who had undergone an osteosynthesis using an anatomical plate were included and extra-thalamic fractures and thalamic fractures treated by other therapeutic means or other types of screw plates were excluded. All patients underwent an urgent general systematic examination and a regional examination. On the neurological level, the majority of patients were admitted with a GCS at 15. Results: Our focused on 12 operated patients between the ages of 17 and 53 with an average of 35 years. Our series includes 12 patients including 17 calacneum fractures which are divided into 10 men and 2 women; the male sex represents 83.3%. The preoperative delay was on average 9.25 days with extremes between 03 and 20 days. No thromboembolic complications or algodystrophy were noted in our series. According to Kitaoka's rating, the average score was 73.5%. The functional results were excellent in 47.06% of the cases, good in 23.53% of the cases, average in 23.53% of the cases and poor in 5.88% of the cases. In post-operative the average Bohler angle was 26.5° (15° to 35°) against a pre-operative Bohler angle varying between -7° to 15°. On revision, the average recoil from Bohler's angle finds it to be 21.5°. The association with an authentic rheumatoid arthritis (RA) remains however exceptional and unusual. The chronology of the association is variable, but the concomitant occurrence of these two diseases remains exceptional: to the best of our knowledge, only three observations of synchronous occurrence of RA and Sheehan syndrome have been found in the medical literature. Method: Retrospective case series study (level of evidence IV) based on their view of medical records of 20 patients who participated in a 25-week postoperative follow-up, in which they were assessed using the UCLA score and submitted to radiological control. This follow-up was due to surgical treatment for acromioclavicular dislocation using the modified Weaver-Dunn technique, with two 5.5mm-Peek Zip® anchors fixed to the coracoid process, with ties on the clavicle, and transfer of the coracoacromial ligament to the distal clavicle. Results: The patients were followed-up for 25 weeks, and all presented satisfactory functional results, with 70% considered excellent and 30% classified as good. However, a high rate of reduction loss of the acromioclavicular joint was observed, which corresponded to 6 of the 20 cases monitored. The average time before returning to routine activities was 20.7 weeks; the shortest was 16 weeks, and the longest, 30 weeks. Patient underwent for spinal fixation from T1 to T8 on 5thof September 2019. After one month, patient started to have a dry gangrene in the left big toe. CT angiogram showed the pedicle screw was crossed the level of the descending aorta. Other associated abnormalities found in spina bifida are: congenital spine deformity, Sprengel deformity, tethered cord, and neurogenic bladder and Talipes equinovarus. Classically, children patients presenting asymptomatic with a diagnosis that can be confirmed by X-rays with the absence of a central zone portion of the clavicle. Although clinical presentations are variable, which ranges from being asymptomatic to discomfort or sometimes restriction in shoulder range of motion, thus, some cases reported manifestations of thoracic outlet syndromes. Therefore, Management of such cases is individually tailored from conservative to surgical intervention when indicated in overt symptomatic patients or those with significant deformity.
