Avulsion fractures result when the fracture piece is pulled from its original bone by powerful contraction of a tendon or ligament. We report a case of boy patient with isolated avulsion fracture of the lesser trochanter confirmed by radiological investigation and patient underwent non-operative treatment. This is the most common benign tumor of the peripheral nerve sheath, representing the 8% of all soft tissue tumors, affecting the sciatic nerve in less than 1% of cases. Due to its location in the nerve sheath, these types of tumors produce symptoms related to nerve function and compression of adjacent structures. Diagnosis is made by magnetic resonance and is confirmed by pathological anatomy, often after screening of radicular pathology with normal magnetic resonance and symptomatology compatible with clinical sciatica. We report the case of a 53-year-old woman with a tumor in the posterior area of the right thigh of one year of evolution. After the examination and the appropriate complementary tests, she was diagnosed with schwannoma of the peripheral sheath of the right sciatic nerve. Pinho","doi":"10.15406/mojor.2020.12.00531","DOIUrl":"https://doi.org/10.15406/mojor.2020.12.00531","url":null,"abstract":"First described in 1957 by Dr Geoffrey Osborne, the Osborne ligament is defined as a band of tissue that connects the two heads of the flexor carpi ulnaris, which forms the roof of the cubital tunnel, between the medial epicondyle and the olecranon. We retrieved data from 16 elbows of 8 cadavers, measuring the length of the Osborne’s ligament in both maximum extension and in 90º flexion. Using this data, we managed to compare the differences between the left and right arm and also between male and female specimens. Our goal was to find statistically significant differences between the variables: males vs females; right vs left elbow; flexion vs extension. {"title":"Indications for operative repair of the traumatic skull base CSF fistula","authors":"B. Meningitis development while treating the CSF leakage with a conservative treatment method, failure of the conservative treatment, pneumocephalus and persistent leakage of the CSF, delayed CSF leakage, CSF leakage in penetrating trauma, recurrent CSF leakage and severe defect in the anterior skull base with brain herniation through the defect of the skull, are some indications for operation. In case there would be CSF leakage in spite of lumbar drainage in more than seven to ten days after the time of the injury, conservative therapy would not be effective anymore and there should be operative repair to be done for the CSF leakage.4 Having a high risk for developing meningitis, delayed CSF leakage, pneumocephalus and large dural tear, external brain herniation and penetrating injuries are the indications for early surgery which conservative therapy should not be considered in such cases. The best time for surgical repair depends on various factors like ongoing risk for meningitis development and risk of frontal lobe retraction during skull base exposure when there would be a swelling in the brain tissue due to the trauma. The aim of this study was to evaluate the efficacy and safety of humeral lengthening using Ilizarov fixator. We reviewed the literature on humeral lengthening using different fixators with regard to indications, operative technique, results and complications. We also retrospectively reviewed 12 patients treated with humeral lengthening using Ilizarov external fixation. The etiology was epiphyseal injury (7 cases), infection (5 cases). The average age at surgery was 14,5 years (range 12,5-23,5) with post septic humeral shortening. The patients were assessed clinically and radio-graphically. Follow up ranged from 1-10-years.The magnitude of lengthening achieved ranged from 5-15cm with an average of 7, 5cm. Functionally all the patients returned to their preoperative jobs and daily activities including sports. Complications included pin track infection in 8 patients, radial nerve palsy which recovered completely in one patient. Conclusion; Humeral lengthening is a valid method that improves the outcome following arm shortening and deformity correction including angulation and rotation. Between 2013 and 2017, 32 patients were operated, with an average age of 31years. Functional assessment was performed using the abbreviated DASH functional scale and pain was classified according to the VAS scale. All the fractures healed successfully, except for two cases that presented in union delay. The radial nerve injury prior to the intervention was recovered progressively without requiring transfers. The functional results were satisfactory in the patients who completed more than one year of follow-up. The posterior approach of the minimally invasive surgery in the treatment of this particular fracture pattern seems to show a biological advantage by allowing complete union rates in all patients.
