In countries with limited resources, quality improvement in healthcare and patient safety are very challenging. This study aims to identify which areas of the patient safety culture (PSC) need prompt intervention.</p><p><strong>Methods: </strong>A cross-sectional study was conducted in three Moldovan healthcare settings, using the Romanian translation of the US Hospital Survey on Patient Safety Culture HSOPSC. Descriptive statistics were carried out, based on the responses from n. 929 staff. Percentages of positive responses (PPRs) by item (41 items) and composite (12 PSC areas) were computed.</p><p><strong>Results: </strong>Most respondents were nurses (53%), followed by doctors (35%). The main work areas were: primary care (27%), medical specialties (20%), gynecology and obstetrics (16%), and general surgery (11%). The highest composite PPRs were for: teamwork within units (80%), feedback & communication about error, organizational learning-continuous improvement and supervisor/manager expectations & actions promoting patient safety (78%), and management support for patient safety (75%). The lowest composites were for: frequency of events reported (57%), non-punitive response to errors (53%), communication openness (51%) and staffing (37%).</p><p><strong>Conclusion: </strong>Our results suggest that staffing issues should be tackled to provide safe care. Staff avoid to openly report adverse events and/or discuss errors, likely because a poor understanding of the potential of these events for learning and because of fear of blame or punitive actions. TNF-alpha, one of the targeted molecule in psoriasis therapy, seems to be also involved in thyroid disorders etiopathogenesis. The aim of this study was to evaluate the relationship between anti TNF-alpha therapy and thyroid parameters: serum level of triiodothyronine (T3), free thyroxine (FT4), thyroid-stimulating hormone (TSH) and antithyroidperoxidase antibody (AbTPO) in psoriasis treated population.</p><p><strong>Methods: </strong>The study was performed on 44 patients with psoriasis vulgaris (20 patients under antiTNF-alpha treatment (etanercept), 24 patients with no previous systemic therapy). Serum concentrations of hormones, AbAntiTPO and TNF-alpha were measured and a thyroid ultrasonographic evaluation was performed for each patient.</p><p><strong>Results: </strong>The mean serum level of FT4 was significantly higher in patients with no systemic treatment (p<0.05). The patients treated with etanercept had a significantly higher level of TNF-alpha (p<0.05). No significant difference was observed for the other evaluated parameters. Also, we found a significant negative correlation between TNF-alpha and TSH levels (r=-0.366, p=0.015).</p><p><strong>Conclusions: </strong>We only found that the mean level of FT4 was significantly higher in patients with no systemic treatment. Also, a negative strong correlation was seen between serum level of TSH and TNF-alpha. They constitute a heterogeneous group of tumors that present differently in terms of both behavior and prognosis.</p><p><strong>Case report: </strong>We present a case of thymic carcinoma in a 55-year-old male patient known to suffer from Myasthenia gravis, whose chief complaints were fatigability, ptosis and coughing. All electromyography studies were done along with excision biopsy to reach the definitive diagnosis.</p><p><strong>Results and conclusion: </strong>The histopathological diagnosis of Thymic carcinoma-Lymphoepithelial variant was established. Though no definite staging system exists for the thymic carcinoma, prognosis of the patient was ascertained by using Weissferdt-Moran system, Masaoka and Tsuchiya TNM staging systems. On coronary angiography, epicardial coronary arteries are either normal or exhibit minimal atherosclerotic changes. Primary Takotsubo syndrome usually occurs in postmenopausal women in whom symptoms are triggered by emotional or physical stress, associated with catecholamine surges. Secondary Takotsubo syndrome may have multiple causes, including an increased catecholamine release due to pheochromocytoma. Despite considerable developments in treatment modalities, a subset of patients with advanced-stage breast carcinoma display poor prognosis. Breast cancer heterogeneity and risk of recurrence could be explained with the help of cancer stem cell hypothesis. Stem cells have the capacity to self-renew and differentiate into multiple cell types. Aldehyde dehydrogenase-1 (ALDH1), an enzyme responsible for the oxidation of intracellular aldehydes, contributes to normal and tumor stem cell differentiation. Invasion and metastasis in breast cancer are found to be mediated by a subpopulation of tumor cells which exhibit stem cell-like features and express ALDH1.<b>The aim</b> was to document ALDH1 expression in breast carcinoma and find its association with other clinico-pathologic prognostic parameters.</p><p><strong>Study design: </strong>This was a cross-sectional observational study.</p><p><strong>Methods: </strong>A total of 62 patients with breast carcinoma undergoing mastectomy were included in this study. The tumors were classified into molecular subtypes by assessing immunohistochemical (IHC) expression of ER, PgR, HER2 and Ki-67 according to St. Gallen Consensus Conference 2013. ALDH1 expression was studied by IHC and correlated with clinicoathological parameters.</p><p><strong>Statistical analysis: </strong>Statistical analysis was done using Graph Pad software (Prism 5 version) for Windows 7. A p-value <0.05 was considered statistically significant.</p><p><strong>Results and analysis: </strong>Out of 62 tumors, 35 tumors (56.4%) showed ALDH1 positivity. ALDH1 expression was significantly associated with larger size, lymph node involvement, higher grade, higher stage and HER2+ or triple negative tumors.</p><p><strong>Conclusion: </strong>This study suggests that ALDH1 expression is associated with poor prognostic parameters and aggressive tumor behavior. The specimens were further divided into two groups (n=12) and were immersed in two coffee solutions (Bio Organic Coffee Bellarom, 100% Arabica, and Iulius Meinl Coffee), for 24 hours. Between the staining sessions, the specimens were stored in sterile water, at 37°C. Each group was further divided into three (n=4), in order to be bleached, as follows: Group 1 - Beyond 35% in office, for 4 applications of 15 minutes each, Group 2 - Zoom Day White 6% in office, for 4 applications of 15 minutes each, Group 3 - Control Group, stored in sterile water. Color values were measured with a dental spectrophotometer Vita EasyShade 4.0 and five measurements were recorded for each sample at a time. Lightness L*, color coordinates a* and b* were recorded, at baseline, after staining in coffee and after bleaching. Whiteness index (WID) of the three composite resins (A3D, A3B, A3E) in the three moments were calculated, as well as the color difference Delta E* correspondent to the staining and bleaching process. Data were analyzed using one-way repeated measures ANOVA and the WID index was calculated WID (p<0.05). Univariate analysis of variance was performed for assessing the influence of staining solution upon composite resins, as well as for testing the effect of bleaching agents. The significance level was set at α=0.05 and pairwise comparisons were adjusted by the Least Significant Difference method.</p><p><strong>Results: </strong>The pairwise comparisons showed no significant difference between the effects of the two bleaching agents upon the WID, meaning that they induce almost similar color changes. The results of the univariate ANOVA test indicated a significant effect of the composite resin and the staining solution upon the WID (p<0.05). However, no significant interaction effect was found between the composite resin and the staining solution (p=0.095). There was a significant difference in the staining effect of the two coffee solutions only for A3B and A3E composite resins (p<0.05).</p><p><strong>Conclusions: </strong>The chromatic changes of the nanocomposite resin could be evaluated by the variation of the whiteness index. The staining effect induced by the two types of coffee was similar. {"title":"Prevalence of deleterious mutations among patients with breast cancer referred for multigene panel testing in a Romanian population.","authors":"Iulian Gabriel Goidescu,&nbsp;Gabriela Caracostea,&nbsp;Dan Tudor Eniu,&nbsp;Florin Vasile Stamatian","doi":"10.15386/cjmed-894","DOIUrl":"https://doi.org/10.15386/cjmed-894","url":null,"abstract":"<p><strong>Aim: </strong>Multigene panel testing for Hereditary Breast and Ovarian Cancer (HBOC) using next generation sequencing is becoming more common in medical care.We report our experience regarding deleterious mutations of high and moderate-risk breast cancer genes (BRCA1/2, TP53, STK11, CDH1, PTEN, PALB2, CHEK2, ATM), as well as more recently identified cancer genes, many of which have increased risk but less well-defined penetrance.</p><p><strong>Methods: </strong>Genetic testing was performed in 130 consecutive cases with breast cancer referred to our clinic for surgical evaluation and who met the 2016 National Comprehensive Cancer Network (NCCN) criteria for genetic testing.</p><p><strong>Results: </strong>82 patients had pathogenic/likely pathogenic mutations and VUS mutations, and 48 were negative; 36 of the pathogenic mutations were in the high-risk genes and 16 were in the moderate risk genes and only 5 cases in the intermediary risk group.From the VUS mutation group 21 cases were in the intermediary risk group, 9 cases were in the moderate risk group and only 7 cases in high risk group.The most frequent BRCA1 variant was c.3607C>T (7 cases) followed by c.5266dupC and c.4035delA (each in 4 cases). Regarding BRCA-2 mutations we identified c.9371A>T and c.8755-1G>A in 6 cases and we diagnosed VUS mutations in 3 cases.</p><p><strong>Conclusion: </strong>Our study identified 2 mutations in the BRCA1 gene that are less common in the Romanian population, c.3607C>T and c.4035delA. {"title":"The influence of occlusal stabilization appliances on cervical dystonia symptoms.","authors":"Camelia Navrotchi,&nbsp;Mîndra Eugenia Badea","doi":"10.15386/cjmed-824","DOIUrl":"https://doi.org/10.15386/cjmed-824","url":null,"abstract":"<p><strong>Objectives: </strong>The aim of this study was to assess the therapeutic efficiency of the occlusal stabilization appliance (OSA) in patients with cervical dystonia (CD).</p><p><strong>Methods: </strong>The study included 11 patients aged between 29 and 80 years, 7 women and 4 men, diagnosed with primary CD. The patients underwent an extra- and intra-oral clinical examination, followed by para-clinical examinations, necessary for the specifications of the OSA. The following data were recorded: demographic parameters, CD duration, management of the disease, dental impression, recording of the centric relationship, recording of the position of the upper jaw with the facial bow. A standardized OSA was manufactured in a private dental laboratory. Patients received instructions for wearing the OSA for 24 hours. Patients filled a questionnaire designed by us, which evaluated the effects of wearing the OSA over a 24-hours period on the symptoms of CD: muscles contraction, pain, discomfort while walking, sleep quality, tremor. The patients kept the dental appliances, and after three months they completed the questionnaire one more time.</p><p><strong>Results: </strong>The OSA was applied on the lower arch in 3 (27.3%) patients and on the upper arch in 8 (72.7%) patients. The OSA wearing time for the first 24 h was on average 19.2±6 hours. Total relaxation of dystonic muscles was reported by 9 (81.8%) patients, while 2 (18.2%) patients related partial muscle relaxation. Seven (63.6%) patients reported a pain decrease. Increased comfort while walking was observed by 8 (72.7%) patients. Two (18.2%) patients described an increase of sleep quality. In two (18.2%) patients the tremor disappeared. All patients reported difficulties while eating and removed the OSA during meals. Patients who wore the OSA for more hours, experienced a pain decrease (p=0.08), an increase in sleep quality (p=0.1), the disappearance of the tremor (p=0.1). After three months, only seven patients continued to use the OSA. In particular the are no data about patients listed for liver transplantation (LT), or affected by acute alcoholic hepatitis. HRS is a common complication of end stage liver disease (ESLD) with a high three-month mortality rate (about 90%). HRS type I (HRS-I) is an acute decline in renal function with creatinine levels above 2.5 mg / dL in less than two weeks. The average survival time is two to four weeks. HRS-II is characterized by a slow and progressive deterioration of renal function, and it underlies refractory ascites. HRS is a frequent complication of alcoholic liver disease (ALD) and can be the onset of severe acute alcoholic hepatitis (AAH) [1,2]. LT is an effective therapy. When waiting for LT, TIPS may be considered an excellent “bridge” procedure. It is known, in fact, that TIPS results in decreased levels of plasmatic aldosterone, renin and noradrenaline within four to six months after placement. This corresponds to a reduction in portal hypertension [2,3]. However, TIPS can be complicated by encephalopathy and liver failure [4]. For some authors [5,6,7,8,9] the use of TIPS shall not be recommended in HRS-1 patients. Of course this consideration in relation to the serious clinical condition of these patients is shareable, although in our experience for patients already on a list for liver transplantation or in a particularly severe clinical condition, TIPS, in view of the high mortality rate in the short term, can be a useful ‘bridge’ therapy. We have considered three studies concerning cases that were followed prospectively during the period 19982006 and re-evaluated retrospectively. In the first retrospective study [10], in 18 patients waiting for LT with HRS-II and RA, and were notresponding to medical therapy, TIPS placement led to a significant improvement in the clinical-laboratory parameters. After 12 weeks we witnessed a total resolution of the ascites in 44.5% of cases and a partial remission (compensated ascites) in 55.5%. One of the consequences was the exacerbation of tuberculosis. In order to control this situation, there was a need to increase the efficiency of diagnosis. This optimization can be reached by a better detection of mycobacterium infection, optimal isolation of strains and identification of the resistance of strains to antituberculous drugs.</p><p><strong>Methods: </strong>In order to identify the best diagnostic modality, we compared the efficacy of the classical bacteriological diagnosis, still performed in the field, to the modern methods of molecular biology. The study included two groups, one represented by 213 patients who were investigated using the classical bacteriological methods, and 49 who were diagnosed using the PCR method.</p><p><strong>Results: </strong>The tuberculosis patients who have been evaluated only with the classical bacteriological methods where diagnosed as TB positive and treated according to the national guidelines, which are in agreement with the international guidelines. The PCR diagnostic methods had a superior diagnostic value compared to the traditional bacteriological method.</p><p><strong>Conclusions: </strong>The results revealed the superiority of the modern molecular biology methods based on PCR.
