{"title":"Formulation and evaluation of a water-in-oil cream containing herbal active ingredients and ferulic acid.","authors":"Mirela Moldovan,&nbsp;Abir Lahmar,&nbsp;Cătălina Bogdan,&nbsp;Simona Părăuan,&nbsp;Ioan Tomuţă,&nbsp;Maria Crişan","doi":"10.15386/cjmed-668","DOIUrl":"https://doi.org/10.15386/cjmed-668","url":null,"abstract":"<p><strong>Background and aims: </strong>The main aims of the present study were to formulate an anti-age cream based on vegetal ingredients and ferulic acid and to evaluate the physical characteristics and the efficacy of the cream.</p><p><strong>Methods: </strong>The active ingredients were Centella asiatica oil, Spilanthes acmella oil, Zingiber officinale extract and ferulic acid. Formulation 1 (F1) was prepared using glyceryl stearate and Ceteareth-25® as emulsifiers and Formulation 2 (F2) using glyceryl stearate and potassium cetyl phosphate, all other ingredients remaining the same. The physical characterization of the creams was performed and the following parameters were analyzed: viscosity, oil droplet size, polydispersity index; also, texture analysis was performed. The anti-aging effect of the F2 was evaluated by assessing the cutaneous density before and after cream application using DUB-cutis® scanner.</p><p><strong>Results: </strong>The mean diameter of oil drops was 10.26±4.72 mm (F1) and 22.72±7.93 mm (F2) and the polydispersity index was 35.4% and 45.7%, respectively. The mean values for consistency were 594.7±10.3 g (F1) and 300.5±14.5 g (F2), the average values for adhesion were 15.61±0.8 mJ (F1) and 22.25±4.3 mJ (F2), for firmness were 51.2±0.8 g (F1) and 30.3±4.3 g (F2) and the spreadability had values between 72.63 mm2 (F1) and 73.3 mm2 (F2). In vivo study revealed that the mean values of the cutaneous density increased from 9.21±1.39 % to 12.50±1.44 % after 8 weeks of cream application. The herbal ingredients incorporated in the O/W cream base for the antioxidant activity and anti-wrinkle effect, induced changes of the cutaneous density, an important parameter which quantifies the regeneration process of the skin.</p><p><strong>Conclusions: </strong>An anti-age cream containing herbal active ingredients and ferulic acid with appropriate physical characteristics was obtained. {"title":"Infantile hemangiomas: a 7-year experience of a single-center.","authors":"Mădălina Bota,&nbsp;Gheorghe Popa,&nbsp;Cristina Lucia Blag,&nbsp;Daniel-Corneliu Leucuta,&nbsp;Alexandru Tătaru","doi":"10.15386/cjmed-781","DOIUrl":"https://doi.org/10.15386/cjmed-781","url":null,"abstract":"<p><strong>Objectives: </strong>The aim of the study was to describe the historical and clinical characteristics of hemangiomas in a series of cases of our clinic.</p><p><strong>Methods: </strong>This is a retrospective study of 36 patients with infantile hemangiomas consulted in our clinic.</p><p><strong>Results: </strong>We had 14 multiple hemangiomas, and 1 kaposiform hemangioendothelioma. Almost two-thirds involved the cephalic extremity, and 76% of the cases were treated. Pregnancy risk factors included prematurity, low-birth weight and respiratory distress syndrome. Propranolol was used in 22 cases, followed by prednisone in 3 cases. Vincristine and interferon were used as associated therapies or as second line therapies. Two hemangiomas had complications, one ulceration and a Kasabach-Merritt syndrome. All the patients had a good evolution.</p><p><strong>Conclusions: </strong>Our study results regarding the involvement of pregnancy and birth risk factors in developing infantile hemangiomas is similar to literature data. The majority of patients had at least one risk factor suggesting that at least one trigger to develop an infantile hemangioma is necessary. Our study shows that the cephalic extremity is mostly involved, and because of its potential complications they are most likely to be treated. {"title":"Energy systems efficiency influences the results of 2,000 m race simulation among elite rowers.","authors":"Stefan Adrian Martin,&nbsp;Valeriu Tomescu","doi":"10.15386/cjmed-675","DOIUrl":"https://doi.org/10.15386/cjmed-675","url":null,"abstract":"<p><strong>Hypothesis: </strong>Energy efficiency within an elite group of athletes will ensure metabolic adaptation during training.</p><p><strong>Objectives: </strong>To identify energy system efficiency and contribution according to exercise intensity, and performance obtained during a 2,000 m race simulation in an elite group of rowers.</p><p><strong>Method: </strong>An observational cross-sectional study was conducted in February 2016 in Bucharest, Romania, on a group of 16 elite rowers. Measurements were performed through Cosmed Quark CPET equipment, and Concept 2 ergometer, by conducting a VO2max test over a standard rowing distance of 2,000 m. The analyzed parameters during the test were: HR (bpm), Rf (b/min), VE (l/min), VO2 (ml/min), VCO2 (ml/min), VT (l), O2exp (ml), CO2exp (ml), RER, PaCO2 (mmHg), PaO2 (mmHg), Kcal/min, FAT (g), CHO (g), from which we determined the ventilatory thresholds, and the energy resource used during the specific 2,000 m rowing distance (ATP, ATP+CP, muscle glycogen).</p><p><strong>Results: </strong>We performed an association between HR (180.2±4.80 b/min), and carbohydrate consumption during the sustained effort (41.55±3.99 g) towards determining the energy systems involved: ATP (3.49±1.55%), ATP+CP (18.06±2.99%), muscle glycogen (77.9±3.39%). As a result, completion time (366.3±10.25 s) was significantly correlated with both Rf (p=0.0024), and VO2 (p=0.0166) being also pointed out that ≥5 l VO2 value is associated with an effort time of ≤360 s. (p=0.040, RR=3.50, CI95%=1.02 to 11.96). Thus, the average activation time among muscle ATP (12.81±5.70 s), ATP+CP (66.04±10.17 s, and muscle glycogen (295±9.5 s) are interrelated, and significantly correlated with respiratory parameters.</p><p><strong>Conclusions: </strong>Decreased total activity time was associated with accessing primary energy source in less time, during effort, improving the body energy power. Health professionals are in a position to model health related attitudes both in the general public and patients. Medical students are an interesting group to focus upon, since they are future health professionals and correcting stigmatizing attitudes is still possible during their educational curriculum.</p><p><strong>Methods: </strong>This study investigated the attitude toward mental illness in medical students at the Iuliu Hatieganu University of Medicine and Pharmacy. We surveyed first year students, since they have not yet received specific classes or internships in psychiatry; 322 students from the Romanian and English sections participated, representing a response rate of 94.7%. The questionnaire consisted of the Romanian and English versions of Link's Social Distance Scale towards people with mental illness scale.</p><p><strong>Results: </strong>Overall, medical students had a relatively negative attitude towards people with mental illness, with moderate social distance and stereotypical attitudes. The level of personal contact with people with mental illness was correlated with positive attitudes. International students had scored lower then Romanian students on social distance toward mentally ill patients.</p><p><strong>Conclusions: </strong>Medical education can play an important role in the attitudes of students toward mental illness. Medical students have stigmatizing attitudes about mentally ill patients. Ethical issues linked to obesity are numerous and still under debate even in countries with a long history in obesity treatment.</p><p><strong>Methods: </strong>From 2007 to 2015 we performed several types of bariatric surgical approaches on 250 patients with an average body mass index (BMI) of 42. The age range was 12-64 years. No death was recorded. Direct or phone contact was possible with 90% of them during follow-up. Starting from a specific question based approach in ethics we present aspects regarding obesity surgery in Romania. Patients' safety, informed consent, cost cover, the role of bariatric surgery in children and bariatric surgeons' training are discussed.</p><p><strong>Results: </strong>Co-morbidities improved or even disappeared in 90% of our patients. Informed consent is a major problem, due to the lack of public knowledge necessary. The private system in Romania offers bariatric surgery at lower prices than Western Europe but is still out of reach for a person with an average income. Lack of maturity and disharmonic family relations raise a series of challenges in assessing the best interest of children and adolescents. Ethics committees, which operate according to well-defined processes, are more and more active in universities and research centers in Romania, checking that methods and performance of scientific studies meet adequate standards.</p><p><strong>Conclusions: </strong>A detailed informed consent, thorough preoperative patient assessment and method selection are mandatory for good results in obesity surgery. SPT usually occurs in young females, without notable symptoms, with a low malignant potential and excellent prognosis.</p><p><strong>Method: </strong>We conducted a retrospective study during the period January 2005 - January 2015. SPT patients admitted in our institution were reviewed by describing demographic data, clinico-pathologic and radiological features, therapeutic management and prognosis records.</p><p><strong>Results: </strong>Thirteen patients with SPT were identified (10 females), with a median age of 30 years. The main clinical presentation was abdominal pain (92.3%). The tumor was mostly located in the body or tail of the pancreas (77%), and the mean size was 8.2 cm. Regarding the surgical approach there were 5 distal pancreatectomies with splenectomy, 3 body and tail pancreatectomies, 2 body and tail pancreatectomies with splenectomy, 2 pancreato-duodenectomy, 1 partial enucleation and of all only 2 partial resections. Postoperative hematoxylin- eosin staining and immunohistochemistry confirmed the diagnosis in all cases. None of the patients had lymph nodes metastases. Only one local invasion. There was one case of death due to postoperative complications. Four cases followed adjuvant systemic chemotherapy. The mean follow-up was 18 months, without evidence of recurrence during this period.</p><p><strong>Conclusion: </strong>SPT should always be considered in the differential diagnosis in young women with a pancreatic tumor. Complete surgical excision is the treatment of choice, and is usually curative. The decision to administer systemic therapy must be individualized. The article illustrates how the founder of these institutions led a valiant struggle for obtaining and arranging a location for the newly established Faculty of Dentistry, within the Institute of Medicine and Pharmacy. Professor Cornel Tiberiu Opriş established himself as the most prodigious researcher at the Faculty for over a quarter-century, until his retirement, introducing his original conception in the therapeutic and surgical field. A publication with many citations and high impact factor can propel researchers in their academic careers. The aim of this study is to explore the perceptions of medical and dental researchers in India about the important criteria to consider while selecting scientific journals for publishing their research.</p><p><strong>Methods: </strong>206 faculty staff members from three medical and five dental institutions were selected through convenience sampling. The study participants completed a questionnaire with 24 closed ended questions on various factors related to journal selection for publication. Factors such as publication frequency, journal citation, indexing, peer-review, impact factor, publication fees, acceptance or rejection rate, publishing house, previous submission and online submission process were considered. The responses were recorded using a Likert scale. Cronbach's alpha as a measure of internal consistency or homogeneity was 0.909. Descriptive statistics and Mann-Whitney U test were employed for comparison of responses among study participants.</p><p><strong>Results: </strong>The mean weight of 24 criteria on a scale of 0 to 4 varied between 2.13 and 3.45. The results showed that indexing of journal (3.45±0.74), online submission (3.24±0.83), impact factor (3.11±0.91), peer-review process (3.0±1.02) and publication fees (2.99±1.11) were among the most important criteria to consider in journal selection.</p><p><strong>Conclusions: </strong>Of the 24 factors considered by health researchers for journal selection, the most important were Journal indexing, online submission, impact factor, peer-review and publication fees. The causes of SIADH can be detected almost always. As a rare disorder, Morvan Syndrome can be defined by the sum of peripheral nerve hyperexcitability, autonomic instability and neuropsychiatric features. Antibodies to voltage-gated potassium channels (Anti - VGKC-Ab) including contactin associated protein-like 2 antibodies (CASPR2-Ab) and leucine-rich glioma inactivated protein 1 antibodies (LGI1-Ab) were previously known for the potential association with this condition.
