It was approved for use in moderate-severe asthma in Spain in 2010.</p><p><strong>Objectives: </strong>The aims of the present study are to analyze the effectiveness and the safety of BT in clinical practice in our center.</p><p><strong>Methods: </strong>Participants had a confirmed diagnosis of severe asthma and poor control without therapeutic alternative. Effectiveness was measured by comparing exacerbations, admissions rates, asthma control, and medication 1 year prior and 1 year after BT was completed. All complications appearing during the procedure and in the first year were recorded.</p><p><strong>Results: </strong>Patients had a mean age of 51 (SD 8) years and were predominantly female (17/23). The average number of activations per patient was 147 (16). The number of severe exacerbations was reduced by 75% (<i>p</i> < 0.001). A 38% reduction in admissions per year was also observed (<i>p</i> = 0.03). The Asthma Control Test improved by 7.1 (3.7) points (<i>p</i> = 0.018). Before BT, the dose of inhaled corticosteroids was 1,621 (1,015) µg of budesonide-equivalent and the dose of oral corticosteroids was 15 (13) mg of prednisone-equivalent. There was a reduction in 430 (731) µg of budesonide-equivalent (<i>p</i> = 0.02) and 4 (11) mg of prednisone (<i>p</i> = 0.094). No changes in lung function were observed. The red color of beetroot derived from betalain pigments confers great advantage to this plant. In this study, the physiochemical and spectrophotometric characteristics of beetroot as well as the histological staining potential of various tissues were carried out to determine its tissue specificity.</p><p><strong>Methods: </strong>The aqueous and ethanol extracts of beetroot were prepared using distilled water and 95% ethanol, respectively. Spectrophotometry, pH, and concentration of both extracts were determined before histological staining with 10% neutral-buffered formalin-fixed, paraffin-wax-embedded tissue sections. Stained sections were viewed with a photomicroscope.</p><p><strong>Results: </strong>The aqueous and ethanol extracts of beetroot were slightly acidic and soluble at concentrations of 381.5 mg/100 g and 253.7 mg/100 g fresh beetroot sample, respectively. Both extracts consist of three betalain pigments with absorbances at different spectrophotometric wavelengths, namely betaxanthins (475 nm), betanin (525 nm), and betanidin (575 nm). The maximum absorbance was 0.925 and 0.615 for the aqueous and ethanol extracts, respectively, at a peak wavelength of 525 nm for each extract. The effect of different concentrations of lignocaine (0.5, 1.0, 1.5, and 2%) on the morphology and growth of the fibroblasts was studied using microscopy, cell viability, and proliferation assay, and apoptosis was detected using the FITC Annexin V Apoptosis Kit.</p><p><strong>Results: </strong>Morphological changes similar to those of apoptotic cells, including cytoplasmic vacuolation, shrinkage, and rounding were visualized in the cells treated with concentrations greater than 1.0% (i.e., 1.5, 2.0%). Animals were divided into 2 groups of 13 animals each: a treated group receiving a unique subconjunctival injection of 1,000 IU of EPO and a control group receiving a saline solution. In each group, 7 animals were used for retinal function evaluation (ERG) and 6 animals were used for retinal structural evaluation (histology). RT was measured, dorsally and ventrally, at 500 μm (RT1) and at 1,500 μm (RT2) from the optic nerve.</p><p><strong>Results: </strong>Retinal function evaluation: for both scotopic and photopic conditions, ERG wave amplitudes increased in the treated group. This increase was statistically significant (<i>p</i> < 0.05) in photopic conditions. Structural evaluation: for both locations RT1 and RT2, the retinas were significantly (<i>p</i> < 0.05) thicker in the treated group.</p><p><strong>Conclusion: </strong>Subconjunctival EPO administration showed beneficial effects both on retinal structure and on retinal function in induced glaucoma in albino rats. {"title":"Accuracy of Type III Portable Monitors for Diagnosing Obstructive Sleep Apnea.","authors":"Keiko Ito,&nbsp;Tokunori Ikeda","doi":"10.1159/000489158","DOIUrl":"https://doi.org/10.1159/000489158","url":null,"abstract":"<p><strong>Objectives: </strong>We evaluated whether the results from portable monitor (PM) devices for the diagnosis of obstructive sleep apnea (OSA), classified into type III and type IV devices by the American Academy of Sleep Medicine, correlated with the results from polysomnography (PSG) testing.</p><p><strong>Methods: </strong>Sixty-four patients with a sleep-breathing disorder used type III or type IV PM devices at home and were subsequently admitted for testing using PSG. The apnea-hypopnea index (AHI) from each machine was measured, and the AHI component, apnea index (AI), and hypopnea index (HI) were also analyzed.</p><p><strong>Results: </strong>There was a stronger correlation between the AHI values from PSG testing and those from the type III PM devices (<i>r</i> = 0.92, <i>p</i> < 0.001) than for the data from type IV devices (<i>r</i> = 0.69, <i>p</i> < 0.001). However, the correlation of HI values (type III: <i>r</i> = 0.43, <i>p</i> = 0.024; type IV: <i>r</i> = 0.14, <i>p</i> = 0.41) was poorer than that of the AI values (type III: <i>r</i> = 0.95, <i>p</i> < 0.001; type IV: <i>r</i> = 0.68, <i>p</i> < 0.001). However, both techniques may delay initiation of respiratory support. To allow lung aeration during cord blood transfusion, we implemented an extrauterine placental transfusion (EPT) approach. This study aimed to provide a detailed description of the EPT procedure and to evaluate its impact on the outcome of infants.</p><p><strong>Methods: </strong>A retrospective analysis was performed comprising 60 preterm infants (22<sup>0/7</sup> to 31<sup>6/7</sup> weeks of gestation). Of these, 40 were transferred to the resuscitation unit with the placenta still connected to the infant. In this EPT group, continuous positive airway pressure support was initiated while, simultaneously, placental blood was transfused by holding the placenta 40-50 cm above the infant's heart. The cords of another 20 infants were clamped before respiratory support was started (standard group). Data on the infants' outcome were compared retrospectively. In a subgroup of 22 infants (<i>n</i> = 14 EPT, <i>n</i> = 8 standard), respiratory function monitor recordings were performed and both heart rates and SpO<sub>2</sub> levels in the first 10 min of life were compared between groups.</p><p><strong>Results: </strong>Although infants in the EPT group were lighter (EPT: 875 ± 355 g, standard: 1,117 ± 389 g; <i>p</i> = 0.02) and younger (26<sup>6/7</sup> weeks ± 19 days vs. 28<sup>2/7</sup> weeks ± 18 days; <i>p</i> = 0.045), there was no difference in neonatal outcome, including the incidence of intraventricular hemorrhage, bronchopulmonary disease, and red blood cell transfusions (all <i>p</i> > 0.1). We observed a unique dacryolith formation in patients with long-term use of rebamipide eye drops and tried to identify the differences in protein compositions of dacryoliths from patients with or without use of rebamipide eye drops using novel proteomic analysis.</p><p><strong>Methods: </strong>Dacryolith samples were obtained from 7 patients (4 samples were without rebamipide usage, 3 were with rebamipide usage) who underwent endo-dacryocystorhinostomy or lacrimal endoscopic surgery and were subjected to protein identification and metaproteomic analysis.</p><p><strong>Results: </strong>The proteomic analysis revealed that most core proteins of dacryoliths are involved in immune and inflammatory responses and rebamipide-related proteins participated in several biological processes, including immune response, receptor-mediated endocytosis, and negative regulation of endopeptidase activity. Metaproteomic analysis of taxonomic diversity of dacryolith proteomes suggested less involvement of bacterial infections in dacryoliths from patients with long-term use of rebamipide.</p><p><strong>Conclusion: </strong>This is the first report to clinically detect the different features of dacryoliths related with long-term use of rebamipide eye drops with proteomic analysis. It is possible that lacrimal protein alteration may be accelerated by topical rebamipide treatment with less bacterial infection involvement, but this requires further study. False-positive cultures can result in clinical uncertainty and unnecessary antibiotic use.</p><p><strong>Objective: </strong>This study sought to assess whether a sterile blood culture collection bundle would reduce the incidence of false-positive blood cultures in a regional neonatal intensive care unit.</p><p><strong>Method: </strong>Clinical data was collected from all infants who had blood cultures taken before and after the introduction of the sterile blood culture collection bundle intervention. This intervention required 2% chlorhexidine and full sterile precautions for blood culture collection. False-positive blood culture rates (presence of skin commensals and ≥3 clinical infection signs) were compared before and after the intervention. The number of days of unnecessary antibiotics associated with false-positive blood cultures was also analysed.</p><p><strong>Results: </strong>In the pre-intervention group (PRE) 197 cultures were taken from 161 babies. In the post-intervention group (POST) 170 cultures from 133 babies were acquired. Baseline demographics were similar in both groups. The rate of false-positive cultures in the PRE group versus the POST group was 9/197 (4.6%) compared to 1/170 (0.6%) (<i>p</i> < 0.05). Unnecessary antibiotic exposure was reduced in the PRE group in comparison to the POST group (27 vs. 0 days, <i>p</i> < 0.01).</p><p><strong>Conclusions: </strong>Implementation of sterile blood culture collection intervention reduced the number of false-positive results. Its overarching objective, from the perspective of the pharmaceutical and diagnostic industry, is to develop innovative therapeutic \"concepts\" with increased value for patients in a global health economy context. This article analyzes the recent advances and remaining challenges from a research, medical, and regulatory perspective in the development and introduction of precision medicine in oncology, more precisely in immuno-oncology.</p><p><strong>Methods: </strong>Analysis of the most recent scientific publications and clinical evidence.</p><p><strong>Results and conclusion: </strong>Stakeholders need to combine efforts in order to turn scientific insights, such as those related to predictive biomarkers, into superior and affordable therapeutic concepts. Many millions of genomes linked to health records will soon be available for researchers and clinicians to make use of to advance precision medicine. To realise the full impact of genomic medicine, genomic and clinical data must be interoperable across traditional geographic, jurisdictional, sectoral, and domain boundaries. Extremely large and diverse data sets are needed to provide a context for interpretation of genetic sequences. No single country or institution can achieve the necessary scale and diversity alone.
