The aim of this paper is to report the latest information and development in the diagnostic conundrum that allergic fungal rhinosinusitis can present. A literature review was conducted of PubMed English language search of current diagnostic criteria for allergic fungal rhinosinusitis, from 1951 to 2017. The diagnosis of allergic fungal rhinosinusitis needs to be considered in all patients with severe chronic rhinosinusitis with nasal polyposis. Patients often have asthma and complain of thick ‘wallpaper glue’ like nasal secretions that can also often be seen on nasal endoscopy. CT scanning often shows heterogeneity and MRI scanning may also help to identify cases. The diagnostic role of total and fungus-specific IgE in AFRS requires further prospective evaluation. The role of genetic and immunological testing in AFRS requires developing as diagnostic tools. Chronic diseases such as diabetes (D), arterial hypertension (H) may act as an accelerating factor in agerelated degeneration of the auditory system. The most used diagnostic test is PTA, ABR is recorded when the pure tone audiometry cannot be access. \u0000 \u0000Objective: to compare the relation between ABR and PTA threshold in presbycusis patients and evaluate the influence of diabetes and/or hypertension on the relationship between ABR and PTA threshold. \u0000 \u0000Material and methods: This is a prospective hospital-based study on the comparison between ABR and PTA of presbycusis patients (≥ 50 years old) composed of 35 females and 21 males. The participants were divided into groups, set as follows group 1 composed of 21 patients, group 2 comprising 12 patients, group 3 made of 14 patients and group 4 composed of 9 patients. \u0000 \u0000Results: The comparison between the mean threshold difference of ABR and PTA in group I and II was significant at each frequency with the mean difference in decibel was 20 dB at 0.5 and 1 kHz in group 2,while in group 2 a mean difference 20 dB at 0.5, 1, and 2 kHz. Pembrolizumab is a humanized antibody which prevents tumor cells from deactivating host T-cells. Like most forms of immunotherapy, many of the adverse effects are autoimmune in origin due to the heightened immune response. Case: We present a rare case of Pembrolizumab induced autoimmune hearing loss in a patient with invasive melanoma which was responsive to steroids. A 67-year-old male presented with metastatic melanoma of the toe. He underwent amputation followed by treatment with IV pembrolizumab 2 mg/kg every 3 weeks. After a single dose, he experienced severe bilateral sensorineural hearing loss. His treatments were stopped and he was started on oral steroids. He later also underwent intratympanic steroid injection. He ultimately achieved improvement to normal-to-mild hearing loss levels in conversational frequencies with subjective return to baseline hearing. According to the FDA, this is one of 14 reported cases of autoimmune hearing loss from Pembrolizumab. Conclusion: Autoimmune hearing loss is a rare, but significant adverse effect of Pembrolizumab treatment for advanced melanoma which is responsive to steroids started up to 4 weeks after onset. Materiel and methods: This is a prospective and descriptive study conducted in the Otorhinolaryngology and Head and Neck Surgery Department of the Gabriel Toure University Hospital Center from January 2012 to December 2015. We included all patients over six years of age who had myringoplasty. Choleatomatous otitis, traumatic perforations, and children under six, patients with nasopharyngeal infections, patients with immunosuppression and incomplete files were excluded. Operative technique was the classic one with the microscope under general anesthesia. Results: During this study period, 84 patients were identified. The young age group of 06-25 was the most represented with 52% and the average age was 28 years old. The satisfactory anatomic result was obtained (tympanic membrane closure) in 95.2% and the reperforation and graft drop in 4.8%. An improvement of 11 to 30 dB was observed in 49.9% postoperatively. Conclusion:The anatomical result is an imperative for any otology surgeon. This result can be successful by looking for factors that can influence the outcome. Fortunately, complications due to foreign body ingestion are rare; however, if present, these can cause significant morbidity to the patient. It fairly known that the fish bone can migrate within the neck causing variable symptoms and signs in clinical presentation. Migration of a foreign body from the oropharynx to the submandibular gland is extremely rare, with only one case reported in English literatures. In this short communication we are reporting what we considered as second case of migrating fish bone in an adult. The foreign body was not seen in plain radiological study done initially. The patient presented a week later with swelling of left submandibular region. Computed tomography scan showed features of sialoadenitis with fish bone seen within the submandibular gland. Patient was treated conservatively with IV antibiotics, and was discharged with follow up but unfortunately he lost his follow up. Allergic rhinitis (AR) is a common debilitating disorder that can adversely affect the quality of life and the academic performance of school-age children and its prevalence has steadily increased during the last decades. Data on the prevalence of allergic rhinitis among secondary school students is scarce despite its reported impact on academic performance of students. The aim of this study was thus to determine the prevalence of allergic rhinitis and its clinical presentation among secondary school students in Kinondoni District which is one of the largest districts in Dar es Salaam Region. Methods: A community based, cross-sectional prospective study was conducted in selected secondary schools in Kinondoni district, Dar-es-salaam Region from August 2016 to January 2017 using random sampling technique. Data was collected using pre-tested questionnaires and analyzed using SPSS program. Results: A point prevalence rate of 34.7% was reported with female predominance and 15-16 age group predilections were found in this study. Sneezing (65.9%) was the commonest nasal symptom and hypertrophied inferior turbinate (61.8%) the commonest physical finding. The commonest aeroallergen was the house dust mite (77.3%) and the least was dog fur (5.9%). Conclusion: Allergic rhinitis affects a significant proportion of the adolescent population with symptoms which have a negative impact on the lifestyle of these participants. Patients with this condition are commonly investigated to exclude the risk of upper aero-digestive malignancies. One of the investigations carried out is barium swallow. However, there is great debate regarding the role of barium swallow in the diagnosis of head and neck cancers. Therefore, the aim of this audit is to review the validity of barium swallow in diagnosing malignancies amongst patients with globus. Data retrieved from the Head and Neck service in Ysbyty Gwynedd reviews all patients diagnosed with head and neck cancer in Betsi Cadwaladr Health Board, which covers all North West Wales in the period between January 2010 and December 2014. Results show that out of 708 patients, barium swallow suspected malignancy in 5 patients. Therefore, the audit confirms that that barium swallow has a very limited sensitivity and specificity in diagnosing head and neck cancer. In conclusion, barium swallow should not be requested routinely as part of the management of globus pharyngeus patients, as it is not. We present the case of a 40-year-old woman with pleomorphic adenoma. A CT scan was performed and a well-defined soft tissue lesion at the left anterior nostril was found. An endonasal endoscopic excision of the mass was achieved. To reach the free margin of the mass, the perichondrium of the nasal septum was dissected.
