This is the first epidemiological facial soft tissue trauma study has been completed in South Africa. The main aim was to define the epidemiology and management of facial trauma injuries. METHODS AND MATERIALS: It was a retrospective population study to delineate the epidemiology of facial trauma incident in a large academic hospital in Charlotte Maxeke Johannesburg Hospital Johannesburg the between October 2016- January 2017.Data were included age, gender, mechanism of injury, length of stay in trauma units, Injury Severity Score (ISS). RESULTS: Out of 1505 patients included in this study. The mean age was 34.5±11.9 years. Interpersonal violence was most common cause of facial injury 56.7% (n=854) followed by road traffic accident 22.10%(n=332). Falls and others attributed were 21.7% (n=320). CONCLUSION: There were high incidents of facial poly-trauma in South Africa. Therefore it is important to develop protocol which includes multidisciplinary approach.

Using stereopsis phenomenon with virtual reality glasses is promising and useful practice in surgical procedures on the middle ear. Clinical Practice: This study included eight cadaver dissections performed on the temporal bone using two different scopy options. Four cadaver dissections were performed using traditional endoscopy which has proved its superiority to a microscope. The disadvantage of traditional endoscopy is the lack of immersion which is an important component in determining the relationship between the anatomical formations of the tympanic cavity. Four cadaver dissections were performed using our proposed method, based on the stereopsis phenomenon using a special camera adapter fixed on the endoscopic tube, which transmitted the video image to virtual reality glasses. It was found that the experiment participants experienced an immersion effect caused by the stereopsis phenomenon. The results of the study were evaluated by the NASA TLX scale for workload index assessing. Conclusion & Significance: Operators showed a lower level of the workload index using the proposed version of scopy based on the phenomenon of stereopsis in virtual reality glasses during dissection of the temporal bone. The participants felt the effect of immersion during the manipulations on the middle ear, as well as noted a more physiological ratio of the optical axis between the surgeon's eyes and working tools. The three-dimensional visualization of the surgical field based on the phenomenon of stereopsis using a virtual reality glasses allows perceiving the spatial depth of the wound thereby expanding the possibilities of surgical intervention and improving operator productivity.

Although 99% of all bacteria exist within biofilms, it is the planktonic state and swab samples from tonsillar surface that historically have received the attention. This is likely due to the fact that disease - causing bacteria is grown more rapidly in the planktonic form and it is much easier to sample fluids as opposed to surfaces (Snyder et al., 2009). Clinical Practice: At Pauls Stradiņs Clinical University Hospital was decided to microbiologically analyze the punch biopsy samples of tonsillar crypts of patients with chronic tonsillitis. Patients who met Paradise criteria were admitted to the Department of Otorhinolaryngology for tonsillectomies. Biopsies were taken before operations. Pathogenic bacteria of biopsy samples were tested for antibacterial susceptibility and biofilm producing ability. Results of the microbiological analysis revealed variety of pathogenic bacteria with resistance to common antibiotics un strong biofilm formation ability. Conclusion & Significance: Isolated pathogens from tonsillar crypts were resistant to antibiotics which are used to treat tonsillitis. Isolated pathogens from tonsillar crypts were weak, moderate or strong biofilm producers. Only 20 % of strains did not produce biofilm. During further studies the role of strong biofilm producers has to be evaluated as biofilm production plays a key role in the pathogenesis of chronic tonsillitis.

It is accompanied by Snoring, described by a malfunctioning vibration of the oropharynx generating noise emissions. Among the causes of this pathology, there are alterations in the soft structures of the Stomatognathic System. Objective: To describe the specific myofunctional therapy work for the Pharynx Walls in a female patient, a 22-year-old patient, diagnosed with Apnea and to show its efficiency. Method: The object of study was a patient of the FMU clinic, 22 years old, diagnosed with severe Apnea by Polysomnography exam and with oromyofunctional alteration. Patient performed 30 continuous days of pharynx wall exercises. Results: After 30 days the patient showed a significant improvement in facial appearance, in the coloration of periorbital hyperpigmentation of the eyes, Mallampati II classification, greater balance of the muscles during chewing and absence of pain when swallowing. Conclusion: In this case, pharynx wall exercises are effective for improving muscle condition and should be included in the therapeutic plan for working with Apnea; however, they were not sufficient to resolve the change, due to the severe degree of the pathology.

In the city of Umuarama, Parana, a project called “Speech Academy” is developed with great adherence of the telemarketers, managers, entrepreneurs, students, doctors, lawyers and many other professionals. This program has been running for two years, in my clinical practice with an average of 130 people. “Speech Academy” was developed to offer people the opportunity to conduct monthly training on their verbal and non-verbal communication in order to improve their evidence-based speech. Training is an approach that develops an impactful, credible and highly persuasive communication. It is held monthly in a group lasting two hours. The work always starts with a process of self-perception and individual assessment and then group training is conducted to improve communication and personal marketing. The program is divided in: breathing, voice, articulation and diction, clarity and objectivity, facial and body expression, intonation and emphases, speech velocity and pauses. And as a bonus we still work on accent and vocal warm-up and cool-down. There are an average number of meetings for each aspect based on the exercise physiologic and the development of groups and each one of these skills.

We report ectopic thyroid tissue base of hyoid region another at the base of tongue, at 6 years old patient presented with anterior cervical mass. This patient was clinically diagnosed with thyroglossal cyst and was being planned for surgery. Preoperative thyroid ultrasonography and CT helped to diagnose of ectopic thyroid tissue which was functioning as thyroid tissue. We started euthyrox medication with patient and the thyroid function become normal within 6 months. It’s important to differentiate this two diseases of the patient of hyoid mass with the help of US and enhanced CT, we do not recommend surgery with patient of ectopic thyroid in the hyoid region we only recommended surgery with the patient of mediastinal ectopic thyroid tissue and intracardiac ectopic thyroid tissue.

Pathologies in the para nasal sinuses can be managed endoscopically, however, the limited availability of skilled man power in endoscopic sinus surgeries, and the dearth of its instruments in this region, remains a challenge in most tertiary health care centres in Nigeria, thus promoting the use of external approach to manage sinus diseases. \u0000 \u0000Method: This was a retrospective study of 10 patients who had Frontoethmoidectomy over a 6 year period, from January 2012 to December 2017.Their medical records were retrieved from operating theatre, and Ear, Nose and Throat ward. Information retrieved included their age, gender, presenting complain, indication for surgery and length of hospital stay. Results were presented in tables and figures. \u0000 \u0000Results: Among the 10 patients, there were 7 males and 3 females, 6 (60%) were aged 10-19 years. The indications for surgery was Fronto-ethmoidal mucocele in 3 patients and frontal mucocele in 2 patients, frontal sinusitis with intracerebral abscess in 1 patient, pan sinusitis with sinu-cutaneous fistula in 1 patient, potts puffy tumor in 1 patient, fronto-ethmoidal abscess, and fronto-ethmoidal osteoma in 1 patient each. Nasal obstruction was the commonest complain 7(70%), followed by frontal headache 4(40%).The average hospital stay among the patients was 6 days. \u0000 \u0000Conclusion: External fronto-ethmoidectomy has remained the mainstay of treatment for frontal and ethmoidal sinus pathologies in this region, despite the advent of endoscopic sinus surgery.
