High quality antenatal care is important to safeguard their health help women maintain normal pregnancies and reduce the rate of maternal and infant morbidity and mortality. Studies conducted to assess quality of antenatal care service which describes the components of antenatal care that the woman receive during her pregnancy and do not describe the factor that affects quality service and also they do not illustrate the quality service. Therefore this study tried to fill the gap which was not address by previous studies.","PeriodicalId":90594,"journal":{"name":"Journal of general practice (Los Angeles, Calif.)","volume":"06 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2018-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.4172/2329-9126.1000367","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"70285294","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

One of these roads is historical knowledge. If we study the form and content of medical textbooks at different times we could see how they reflect the dominant medical theory at each time according to the publication dates and so we could detect trends on the concepts of etiology, diagnosis, treatment, etc. of the different health problems. The contemporary debate on the development of science and technology, part of the assumption that science and technology have a linear development but the study of history affirms that the fundamental principles of medicine do not change in spite of scientific, technological or socio-political advances. Our profession has an immutable base of assistance, concern for others and curiosity of being the patient as a person, the axis of medical practice, respect for the autonomy of the patient, meticulous observation, etc. Some of the elements that define the medical being in an immutable and without time or epoch way are: 1-variability and induction; 2-research; 3-biopsicosocial, functional and somatic; 4-diagnosis; 5-healthy and sick: the disease develops as a time of life; 6-the anamnesis is almost everything and the complementary tests are not almost anything; 7-course, prognosis, evolution and context and 8-treatment depends on individual internal and external contexts.","PeriodicalId":90594,"journal":{"name":"Journal of general practice (Los Angeles, Calif.)","volume":"6 1","pages":"1-4"},"PeriodicalIF":0.0,"publicationDate":"2018-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.4172/2329-9146.1000364","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"70285606","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Our objective was to determine the causes and outcome of respiratory diseases in our facility for health planning, policy formulation and allocation of resources.Method: A retrospective study of all respiratory-related admissions between January 2011 and December 2015. Classification of diseases was based on the ICD-10 coding system.Results: Respiratory-related admissions accounted for 460 (12%) of all medical cases with a slight male preponderance of 1.4 to 1. Mean age of patients was 50.4 ± 20.8 years. The majority 342 (74.3%) of these admissions were due to communicable respiratory diseases. Pulmonary tuberculosis (44.1%) and pneumonia (28.9%) accounted for the highest admissions which were closely followed by COPD (11.1%) and asthma (7.2%). The commonest co-morbidity was HIV/AIDS (12.3%) which also coexisted with other diseases in most cases of mortality. The median hospital-stay was 7 days. Most diseases had peak presentations during the dry season except bronchial asthma with rainy and dry peak seasons. The overall mortality was 17.6% and the highest mortalities were recorded in patients who had pulmonary tuberculosis (55.5%), pneumonia (20.9%) and COPD (8.6%).Conclusion: Communicable respiratory diseases are the leading causes of morbidity and mortality. Scaling up on coverage of the control programmes as well as the methodical integration of non-communicable diseases into already existing control programmes will further strengthen the health-care systems in resource-limited settings.","PeriodicalId":90594,"journal":{"name":"Journal of general practice (Los Angeles, Calif.)","volume":"6 1","pages":"1-6"},"PeriodicalIF":0.0,"publicationDate":"2018-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.4172/2329-9126.1000352","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"70285392","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

There are those who claim that the vaccine is harmful and this proves the negative side effects that appear as a result of vaccination. Evidence-based medicines acknowledge that there are rare side effects, but that does not mean that the vaccine will harm every organism who receives it. Although the vaccines eradicated some illnesses, it is difficult to convince someone whose family member has been ill with a disease as a result of the vaccine. Unfortunately, such negative side effects are occurring in the practice.","PeriodicalId":90594,"journal":{"name":"Journal of general practice (Los Angeles, Calif.)","volume":"6 1","pages":"1-2"},"PeriodicalIF":0.0,"publicationDate":"2018-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.4172/2329-9126.1000e122","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"70285682","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Methods: The study population consisted of 48 patients (age 68 ± 8.51 years, 79.55% male). Patients underwent transthoracic echocardiography, electrocardiogram, the 6-minute walk test (6MWT) and NYHA functional class evaluation before biventricular pacemaker implantation and during follow-up. Results: Median follow-up was 21.89 ± 9.21 months. 4 (7.7%) patients died from heart failure. Reverse left ventricular remodeling was observed in 77.08% (N=37) patients. CRT resulted in statistically significant improvement of NYHA functional class (p=0.015) and the increased results of 6MWT (p=0.000). Logistic regression analysis demonstrated lower pulmonary artery systolic pressure (PASP) (OR 1.152, 95% CI 1.026-1.293; p=0.017) and more severe mitral regurgitation (OR 0.11, 95% CI 0.018-0.769; p=0.025) prior to device implantation as significant predictors of positive response to CRT. Poor response cut-off value of PASP ≥ 46 mmHg was established in ROC analyses, with 78% of sensitivity and 62% of specificity. Conclusions: Our study confirmed the reversible cardiac remodeling and improvement in systolic left and right ventricular function and functional condition in advanced heart failure patients treated with CRT. According to our study, lower pulmonary artery pressure and more severe mitral regurgitation prior to device implantation predicted favorable long-term response to CRT.","PeriodicalId":90594,"journal":{"name":"Journal of general practice (Los Angeles, Calif.)","volume":"06 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2018-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.4172/2329-9126.1000365","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"70285241","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Dr Rajesh Rajput at Hair Restore who is honoured to be Fellow of the International Society of Hair Restoration Surgeons and an International Certified Trichologist, talks about the current research in this field. The previous generations faced stress only after the age of 30-40 years [1]. Youngsters today face competitive life and stress on multiple fronts right since school days. Stress has become a part of routine life. Lack of inclination towards sports and good breathing exercise is also contributing to slow physical growth and poor hair quality. Young growing age needs very good nutrition in their formative years of life. Instead there is a trend towards eating less and neglecting nutrition.","PeriodicalId":90594,"journal":{"name":"Journal of general practice (Los Angeles, Calif.)","volume":"6 1","pages":"1-1"},"PeriodicalIF":0.0,"publicationDate":"2018-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.4172/2329-9126.1000355","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"70285461","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Most of the current anti-epileptics and anti-depressants however, especially when prescribed to the elderly patients, have troublesome drawbacks: drug-interactions, side effects and tolerability issues. Topical compounded creams might be a good alternative, especially since it is easy to test in the general practice to discover responders to these creams. At our Institute for Neuropathic Pain we see many patients suffering from peripheral neuropathic pain. We have developed a number of compounded creams based on amongst others amitriptyline 10%, ketamine 10%, clonidine 0.2%, baclofen 5%, and phenytoin 10%. Many patients profit from such topical formulations and we will describe how a primary care physician can fast decide whether as specific compounded cream is useful for a patient. Patients suffering from peripheral neuropathic pain, as in diabetic neuropathy, chronic idiopathic axonal polyneuropathy (CIAP) and chemotherapy induced polyneuropathy (CIPN) as well as from complex regional pain syndrome (CRPS), seem often to be responsive to such topical approach. Meanwhile, more than 800 Dutch medical doctors have been prescribing one or more of our compounded creams, and patients and medical doctors from abroad frequently contact us to explore the therapeutic options for their patients based on these creams. We will give some practical tips how to differentiate between responders and non-responders.","PeriodicalId":90594,"journal":{"name":"Journal of general practice (Los Angeles, Calif.)","volume":" ","pages":"1-3"},"PeriodicalIF":0.0,"publicationDate":"2017-12-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"47420330","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
