Telling the difference between phyllodes tumors and giant juvenile fibroadenomas, that rarely affect adolescent and young women, is difficult but of importance because resection with margins is the mainstay of treatment for phyllodes tumors. We report a clinical case of an eighteen-year-old female patient, with past history of left breast lump excision four years before, classified as “juvenile fibroadenoma”. In early 2015, she was referred to the consultation by the appearance of a bulky ipsilateral lump (10.8 x 6.9 cm), core-biopsy histology revealed “fibroadenoma with more lax stroma vs benign phyllodes tumor”. Surgical excision of the lesion was performed. Histology of the specimen confirmed “benign phyllodes breast tumor (14 x 12 x 8 cm and 788 g)”. Given its benign nature and negative margins, the patient did not require further treatment and currently attends in follow-up consultation. Several previous studies have shown that decreased number of circulating endothelial progenitor cells (EPCs) correlated well with vascular remodeling and impaired pulmonary artery function. Moreover, a deep deficiency of EPC characterized a poor clinical outcomes in PAH individuals. There is large body of evidence regarding that the EPCs could be not just biomarker of endothelial dysfunction, but a target of PAH therapy. However, improvement of PAH severity associated with increasing number of circulating EPC with angiopoetic phenotype accompanying sildenafil care. The internship book serves for their pre-post evaluation. Objective: We aimed at describing the intern’s improvement of skills during training. Methods: We conducted a quasi-experimental study, including interns having carried out their stage in general medicine, from March 2008 to February 2015. Our main outcome was comparing pre-post stage acquisitions. Results: Within the period study, 566 interns realized the internship. Acquisitions improvement rate increased significantly after stage in all items for the Lickert scale very good and excellent. Self-evaluation rates pre-post stage passed from 1.7%-34.5% for primary care current activities and from 2.3%-35.1% for care globality. For technical abilities, the rate passed from 7.5%-22.8% for gastroenterology skills. The highest non-response rate was notified for care coordination passing from 2.6%-10.4% and for ophthalmology (from 18.8%-27.8%). However, the best student competence was care coordination (26.1%). The highest increasing technical skill item was unregistered in otoscopic examination (24.9%), the lowest was practice and reading an intradermal reaction (5.3%). Irdem, Habip Gedik","doi":"10.4172/2329-9126.1000354","DOIUrl":"https://doi.org/10.4172/2329-9126.1000354","url":null,"abstract":"Objective: The aim of this study was to evaluate mood changes in mothers of infants diagnosed with infantile colic.Materials and Methods: In this prospective case-control study, mothers with infants who were referred to the Pediatric Clinic of Ministry of Health Okmeydani Research and Training Hospital and diagnosed with infantile colic between November 2014 and February 2015 were compared with those of infants without infantile colic using the Beck Depression Inventory and a questionnaire.Results: A total of 31 mothers with male infants (51%) and 30 mothers with female infants (49%) participated in the study. The mean Beck depression inventory scores were 18.3 (min 5-max 35 points; median score: 21) in the infantile colic group and 14.6 (min 5-max 33; median score: 12) in the control group. Beck depression inventory scores in the infantile colic group were significantly higher than those of control group. The mean Beck depression inventory scores were 17.0 (min 5-max 32 points; median score: 17) in mothers who had no problems during the pregnancy and 24.9 (min 5-max 35 points; median score: 25.5) in mothers who had problems during the pregnancy, respectively. Beck depression inventory scores in mothers who had problems during the pregnancy were significantly higher than those who had no problems during pregnancy.Conclusion: Beck depression inventory scores in mothers with infants diagnosed with infantile colic were significant compared to the control group. This study aims evaluate the safety and efficacy of Nebivolol (Nebilet) 5mg in the treatment of hypertension among Filipino adult patients. Methods: This is a prospective, open-label, non-interventional study which evaluated the safety of Nebivolol (Nebilet) 5mg in Filipino adult subjects with hypertension, aged 21 to 80 years, in an out-patient setting. Results: A total of 1,154 patients were recruited into the study, 36 (3%) of which had reported adverse events (AEs). Majority of the AEs were due to headaches (18%) and other non-specified events (32%). Most of the adverse events reported were mild to moderate and some were known effects of β- blockers. There were no serious or unsuspected adverse events described during the study. In terms of efficacy, nebivolol reduced blood pressure to less than 140/90 mmHg or a decrease of >10 mm Hg diastolic blood pressure (DBP) in 82% of subjects in the first follow-up visit. At the end of the treatment period, 97% had achieved target blood pressure levels. It is eminently treatable but when it presents atypically diagnosis can be missed. Aim: To look for the common and uncommon clinical features and antibody profile in patients with features of myasthenia gravis. Settings and design: Myasthenia gravis though well known to be associated with antibody to AchR receptor in the post synaptic membrane, a large group of patients are seronegative for AchR with atypical presentations. Anti- Titin antibody titre was done in 7 cases and anti-MUSK in 1 case when strongly indicated by clinical features. Materials and methods: 29 patients who were confirmed as Myasthenia gravis, seen by authors in the last 3 years were evaluated with anti AchR antibodies in all cases and; Anti-titin and Anti-musk antibodies when clinical indication was strong and patient could afford. Statistical analysis: Descriptive analysis was done as the number is small. Results and consideration: Maximum numbers of patients were seen in the younger age group. But patients with more serious disease, patients with Thymic and extrathymic Tumors, Anti-titin and musk associated patients were in the later age group. Thymic tumour has been not detected for nearly 10 years after disease onset in one case. Macroglossia, Motor neuron disease overlap is seen in one case which is relatively very rare. Though Myasthenia gravis is seen commonly in the young persons, Thymic and extrathymic tumors, Anti-titin antibody associated Myasthenia gravis is seen in later age group.
