Xiao, Hong-yue Niu, Lian Liu, Xiao-Yu Dai","doi":"10.53388/ghr2023-03-071","DOIUrl":"https://doi.org/10.53388/ghr2023-03-071","url":null,"abstract":"Objective: To explore the main acupoint prescription and mechanism of acupuncture in treating Functional dyspepsia (FD) by analyzing the clinical randomized controlled trial literature on acupoints and targets of acupuncture in the treatment of FD combined with association rule mining and bioinformatics/network pharmacology methods. Methods: Search relevant clinical randomized controlled trial literature on acupoints and therapeutic targets of acupuncture in the treatment of FD from eight databases from their inception to June 18th, 2022. We obtained acupuncture selection points, meridian, and therapeutic targets and established acupoints database for acupuncture treatment of FD. FD-related targets were collected from GeneCards, DisGeNET, OMIM, and DrugBank databases. We obtained the potential targets of acupuncture on FD by taking the intersection of acupuncture for FD therapeutic targets and FD-related targets. We performed the protein-protein interaction network, Gene Ontology (GO) analysis, and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway enrichment analysis. Results: In this study, 26 Randomized Controlled Trials related to acupuncture treatment of FD were retrieved. We obtained 29 acupoints, 17 acupuncture for FD therapeutic targets, and 10 intersection targets. Tumor necrosis factor (TNF), Interleukin (IL)-1, and Neuropeptide Y (NPY) are key targets of acupuncture in the treatment of FD. Conclusion: ST36-CV12-PC6-LR3 is the main acupoint prescription for FD. Peptic ulcer reaches intensely to the mucosal muscle layer in the stomach and duodenum. Various factors have been documented to be involved in the pathogenesis of peptic ulcers like Helicobacter pylori , nonsteroidal anti-inflammatory drugs (NSAIDs), acid and pepsin, genetics, and smoking. The conventional use of drugs like proton pump inhibitors (PPIs), histamine (H 2 ) receptor antagonists, antacids, potassium competitive acid blockers, and antibiotics has shown antiulcer effects. However, various researches have shown that herbal drugs can successfully treat peptic ulcers in preclinical and clinical models by different mechanisms. Many herbal drugs and their extracts from different parts like root, stem, leaf, flower, and seed showed potent ulcerprotective effects in the experimental setup. This review critically discusses the factors involved in the pathogenesis of peptic ulcers. Although some people believe that it may have some advantages in this regard,There is no reliable evidence-based study demonstrating its effectiveness. This study aims to systematically evaluate the healing effect and security of BXD in the treatment of GU. Methods: PubMed, Cochrane Library, EMBASE, ScienceNet, China National Knowledge Infrastructure (CNKI), Wanfang database, Weipu database, and China biomedical literature service (CBM) database were systematically searched to obtain all randomized controlled trials (RCTs) evaluating the treatment GU of BXD published as of April 2022. Two researchers independently screened and extracted all research data, finally evaluated the bias risk of inclusion in the study using revman 5.4. Results: This meta-analysis included 27 randomized controlled trials and 1411 patients. The clinical effective rate, recurrence rate, HP eradication rate, adverse reaction rate, and visual analog score (VAS) of BXD combined treatment and standard treatment alone were compared. The results of the meta-analysis showed that BXD combined treatment improve the symptoms related to the gastric ulcers and reduce drug-related adverse reactions. At present, commonly used drugs include enema and suppositories, laxatives, microecological preparations, gastrointestinal motility drugs and other treatments. However, the effect is limited. Traditional Chinese medicine (TCM) treatment of FC has advantages. Methods: In this randomized controlled study, 244 eligible patients were randomly assigned in a 1:1 ratio to a treatment group (Zangfu ointment and massage therapy + Lactulose Oral Liquid) and a control group (Lactulose Oral Liquid) for 14 days. Number of spontaneous defecation per week and First defecation time will be used as primary outcomes, and Traditional Chinese Medicine (TCM) syndrome scores and syndrome scores,PAC-QOL form will be used as secondary outcomes. Blood routine, liver function, and renal function will be used as safety outcomes. The primary and secondary outcomes will be performed at 0th, 7th, and 14th day, and the safety outcomes will be performed at 0th and 14th day. In the management plan of non-alcoholic fatty liver disease and type 2 diabetes, there is no drug with significant efficacy or both. The dialectical idea of traditional Chinese medicine has a good curative effect and significant advantages in treating metabolic disorders. The author now reviewed a case of non-alcoholic steatohepatitis combined with type 2 diabetes. In this case, the method of invigorating the spleen and eliminating turbidity was effective, which effectively prevented the progression of liver disease in the patient, and T2DM was alleviated by maintaining within the standard range after the use of insulin was stopped.
