All relevant information is generated, disseminated, shared and recorded in an automated manner. The information includes donor and patient health records, transfusion requirements and transfusion history, data and process traceability, among others. This compact network of information is having a significant impact on almost all phases of the transfusion process, being the most relevant: Safety in donor selection; clinical and biological shared information for all hospital; safety in the identification of patients, samples and blood components; complete real-time traceability; increased quality of the decision to transfuse and decrease unnecessary transfusions; decreased number of adverse effects, component administration errors and near miss related to blood component transfusion; or optimization of training programmes, as it is a standardized activity for all staff involved on blood transfusion. After decades of investigation, cryopreservation and cold storage are gaining prominence as methods that can dramatically improve PLT availability in remote, rural and isolated areas, including war zones. Patients with active bleeding who need to clot rapidly benefit from the procoagulant phenotype of cryopreserved or cold‐stored PLTs, which can quickly bind to the damaged endothelium. An innovative research area is combining pathogen reduction technologies (PRTs) with different storage methods. The advantages of refrigeration and cryopreservation can be maximized when used together with PRT treatment, allowing longer storage with a considerably reduced risk of infectious disease transmission.

To understand the factors associated with the return of first-time blood donors, we conducted a retrospective analysis of first-time blood donors who donated blood to the Japanese Red Cross Blood Services in the year 2017, by following them up for 2 years and then statistically analysing the returning donors in comparison with the non-returning donors. The blood donors’ features were obtained from the Donor Management System and statistically analysed according to whether they returned or not to make another blood donation during the 2-year period. Data from 421 415 first-time donors were analysed, and the return rate was 39·6%. In a univariate analysis including all donors, the factors significantly associated with return donation were male gender, younger age, higher BW, 200 ml whole blood collection (200 WB) and higher Hb. In the subgroup analysis, we analysed student donors aged less than 23 years (157 847 donors) and found younger age, higher BW, 200 WB and female gender as factors significantly associated with returning. Among non-students in the same age group (n = 42 623), younger age, higher BW, 400 WB and male gender were the factors associated with higher returning. The median age and BW of the non-student group were higher compared with the students. Although 200 WB donation is not cost-effective, from our results it seems to play a role in the acquisition of donors who are potential repeat blood donors, essential for maintaining the blood supply in Japan.

An international scientific committee (SC) defined 17 Population-Intervention-Comparison-Outcome (PICO) questions for the three topics preoperative anaemia, red blood cell (RBC) transfusion thresholds for adults and PBM implementation. Based on these questions, an extensive literature search was conducted in four biomedical databases. The GRADE framework (= grading of recommendations, assessment, development and evaluation) was used to develop a systematic approach to the presentation of evidence summaries and for making clinical recommendations. Three expert panels (EP) consisting of clinicians, scientists, nurses, patient representatives and methodologists were established and used these methods to develop recommendations driven by published evidence. Out of more than 17 500 literature citations, data from 145 studies were incorporated. The expert panel for preoperative anaemia developed 4 clinical and 3 research recommendations. A strong recommendation advices early detection and management of preoperative anaemia before major elective surgery. For RBC transfusion thresholds, 4 clinical and 6 research recommendations have been formulated. Two strong clinical recommendations for RBC transfusion thresholds comprise a haemoglobin concentration of <7 g/dl for critically ill, but clinical stable adult intensive care patients independent of septic shock and <7·5 g/dl for adults undergoing cardiac surgery. For the implementation of PBM programmes, 2 clinical and 3 research recommendations were developed. Research recommendations were formulated as encouragement of new studies to answer open questions. Due to the relative paucity of strong evidence-based answers in current publications to the 17 PICO questions, additional research, an international consensus for accepted definitions and Hb thresholds as well as clinically meaningful end-points for clinical studies are necessary. The 2018 PBM ICC defined the current status of PBM evidence and established 10 clinical and 12 research recommendations for preoperative anaemia, RBC transfusion thresholds for adults and implementation of PBM programmes.

Of this group, n = 253 were offered a discount voucher for a local cafe (intervention group) and n = 989 received the business as usual approach in the donor centre (control group). The subsequent donation behaviour of participants was monitored over three months.
\u0000
\u0000 \u0000
\u0000 \u0000
Results
\u0000 \u0000
Overall, 69·6% accepted the discount voucher, with 18·2% of those redeeming it. Those who accepted the voucher were younger and less experienced plasma donors. The intervention did not significantly increase the likelihood of return to donate compared to the control group; however, it did reduce the time to return to donate plasma. The overall rate of donation within 3 months was also significantly higher for donors who were offered the incentive than for the control group. Declining the offer did not negatively impact donors’ return behaviour.
\u0000
\u0000 \u0000
\u0000 \u0000
Conclusion
\u0000 \u0000
This paper provides new insights around strategies to increase plasma donation frequency, as well as donor response to introducing a discount voucher reward within a voluntary non-remunerated donation system.
\u0000
\u0000
