He was noted to have choroidal metastasis in this eye and was subsequently admitted for a thorough work-up per oncology to find a primary tumor. After whole body imaging was completed, he was found to have hepatocellular carcinoma proven histopathologically from liver biopsy. He was treated with external beam radiation therapy and his visual acuity and imaging at one-month follow-up showed no improvement. This case highlights the interesting relation between two highly vascular organs – the liver and the choroid - and the diagnostic challenge that exists with choroidal metastases from an unknown primary tumor. It also underscores the need for additional studies to determine whether primary tumor type affects therapeutic management and prognosis.

The indication for enucleation varies among the centers depending on medical equipment and ophthalmologists’ experience. Few studies have been performed regarding etiologies of enucleation surgeries. In this study we aimed to assess clinicopathologic correlation of surgical eye specimens and changing trends of enucleation operations. \u0000Methods: Medical records of patients underwent eye surgeries referred to pathology department of a tertiary hospital, Tehran, Iran during 1999 to 2015 was collected. For better comparison, first time period (1999-2006) and second time period (2007-2015) were defined. Type of surgery, causes of enucleation and histopathological diagnosis were reviewed. \u0000Results: A total of 342 surgical eye specimens were sent to pathology department over the time period. Ocular tumors were the most common indication for enucleation with retinoblastoma and melanoma constituting the majority of them. An interesting change in trend was observed between two time periods. The number of enucleated eyes due to trauma were declined during second time period whereas the percentage of ocular tumor was not significantly changed. In evaluating clinicopathologic findings, 19 discordant cases were found which most of them were mimickers of malignancy. \u0000Conclusion: The indication of enucleation surgery varies among the centers. Although majority of enucleated cases were due to tumors, some non-tumoral conditions may mimic tumoral conditions which should be always in differential diagnosis.

It maintains the intra ocular pressure and is important in maintaining transparency of the media for maximum photon transmission to the retina. With advancing age the vitreous undergoes liquefaction resulting in its transformation from a formed gel form to fluid form. Vitreous substitutes or the so called tamponading agents are used in vitreo-retinal surgery to restore intraocular pressure and provide intraocular tamponed. The type of substitute used is dependent on the individual clinical situation. The proper vitreous substitute for long-term drug delivery system should either reduce or eliminate the need for multiple intravitreal injections.

A 66 year-old woman with a history of breast carcinoma was seen with the complaints of headache, blurred vision, and diplopia. On neuro-ophthalmologic examination restriction of left, right and up gaze in her left eye was found. Magnetic resonance imaging of orbit revealed enlargement of all extraocular muscles, heterogeneous enhancement of lacrimal gland and retroorbital fat in her left eye. The patient underwent biopsy of lacrimal gland and extraocular muscles. Lacrimal gland metastasis was shown by histopathologic examination, but there was no tumor cells in the extraocular muscles. After radiotherapy for lacrimal gland metastasis, her symptoms relieved remarkably. In patients with a history of malignancy, orbital symptoms and findings should be thought as a cause of malignancy primarily.
