His first degree was in Natural Sciences at Cambridge, and he was an ordained Anglican priest who taught chemistry and worked as a school chaplain in Uganda. Having lost his faith following the massacres he witnessed there in the early 1970s, Tom returned to the United Kingdom and settled in Bradford, where he completed his PhD in Interdisciplinary Human Studies, becoming a lecturer and eventually a professor in 1998. This potted account of his career does not easily explain how he became one of the most influential figures in dementia care. Nearly 30 years on, his name is still synonymous with person-centered dementia care. I was privileged to know Tom Kitwood as a mentor and a friend. I first heard him speak at a conference in 1988 when I was working as a clinical psychologist in UK psychiatric services for older people. My work in dementia studies has been so intertwined with Tom Kitwood’s influence that I find it impossible to provide an objective review of his legacy. Nursing staff had to work under psychological pressure to maintain patient safety. Resilience has proven to be a protective psychological health factor that can safeguard healthcare professionals’ mental health and well-being during healthcare crises. The state of resilience among Dutch healthcare professionals is not yet known. \u0000Objective: This study investigated perceived resilience and perception of patient safety during the COVID-19 pandemic among Dutch nursing staff. Additionally, individual differences in resilience were examined in relation to patient safety. \u0000Method: An online survey about resilience and patient safety was sent to 2,611 members of the Dutch Nursing Staff Panel in June 2021, after the third wave of the COVID-19 pandemic in the Netherlands. \u0000Results: 884 certified nursing staff professionals completed the survey (33% response rate). Nursing staff considered themselves resilient with a positive perception of patient safety within their organization. Factors that influenced resilience were education, age, part-time employment, experiences with treating patients with a COVID-19 infection, and having suffered from a COVID-19 infection. Nursing staff who perceived higher resilience reported to act more flexible during their work, to encounter more unexpected situations and they found that working according protocols and guidelines had improved during the pandemic. \u0000Discussion: The results show a more positive pattern compared to other European countries. In the Netherlands, nursing staff considered themselves as highly resilient. \u0000Conclusions: Resilient healthcare professionals report to be better able to handle crises such as the COVID-19 pandemic. Ethical imperatives, including respect for all persons’ dignity and “first do not harm” as well as scientific considerations, including recognition of the eco-bio-psycho-social-spiritual multidimensionality of person and health were attended to.Results: The approach outlined here attends to Universal Sexual Rights, and the multidimensionality of persons’ sexual diversity, in the context of the current pandemic. This is illustrated through the Gender Identity Law enacted by the Argentine Ministry of Health in 2014 and its 2020 amendment, formulated in the face of the pandemic. The healthcare approach outlined here involves recommendations to promote access to health care for transgender, transvestite, and sexual non-binary people in the context of the pandemic.Conclusions: All persons can present and experience a unique and multidimensional sexual profile that should be assessed thoroughly within the multidimensional framework of total health and total person. In Uruguay, unlike most countries, a mandatory confinement was not declared. On the contrary, an extensive education and prevention campaign was carried out associated with measures to reduce social mobility, such as prohibiting meetings and closing bars.Objectives: The aim of the present study was to evaluate the COVID-19 pandemic in Uruguay and its repercussion on the clinical evolution of the two most common surgical pathologies: acute appendicitis (AA) and acute cholecystitis (AC).Methods: A retrospective comparative cohort study was performed at the Emergency Department of the University Hospital “Hospital de Clinicas Manuel Quintela,” the most important tertiary referral hospital in the city of Montevideo, capital of Uruguay. Two cohorts were identified: 13th of March 2019 to 13th of June 2019 [Pre-Covid period (PCP)] and the same period in 2020 [Pandemic Covid period (PCVP)]. Demographic and clinical data were analyzed.Results: A total of 118 cases were registered in 2019 and 109 in 2020. There were 43 cases (36.4%) of acute appendicitis in the pre-Covid period and 42 cases (36.5%) in Pandemic Covid period (p = 0.745). Acute cholecystitis cases differed significantly between cohorts, with a raise of cases in the Pandemic Covid period (14 vs 25) (p = 0.027). The surgical approach (Laparoscopy vs Open) did not change significantly (p = 0.207). A significant increase in complicated cases (AA + AC) was found during the pandemic (PCP 57 cases vs PCVP 67 cases) (p < 0.001). The sub-analysis of AA and AC showed a significant increase in AA’s complicated cases during PCVP (14 vs 25; p < 0.001) and no significant changes in the AC group (p = 0.99).Conclusion: An increase of complicated cases of AA was observed with maintenance of the number of consultations that might be explained by the excellent pre-hospital care system and absence of lock-down measures. The complex problem of COVID-19 and the possibilities of responding through innovative general health strategies have been raised. The development of innovative health strategies was stimulated by the 1978 Alma Ata Declaration on Primary Health Care and by related perspectives around the 2018 Astana Conference. To this may be added the contributions of the International College and the Latin American Network of Person Centered Medicine toward the exploration and formulation of innovative concepts and strategies of health focused on persons.Objectives: (1) Ontological clarification of the concept of health and of the strategies and tactics for health actions; (2) Delineation of and assessment of multifactorial support for promising general health strategies focused on persons; and (3) Elucidation of the relevance of such strategies to respond to the demands of the COVID-19 pandemic.Methods: (a) Analysis and reflection on health concepts and strategies by a multidisciplinary authors group; (b) Selective review of the international and Latin American literature on health systems and the pandemic, displayed, and analyzed tabular and narratively.Results1. Ontological concepts corresponding to the meaning and functions of health and of specific general strategies and tactics to restore and promote health were formulated.2. A general health strategy was outlined based on an analysis of the process of building promising health constructs emerging from conferences and international declarations in recent years. Then, early and contemporary historical considerations, as well as contextual perspectives (Social Determinants of Health, the Sustainable Development Goals, and the Essential Functions of Public Health) were identified as supporting the specific elements of the persons-centered mutual and integral healthcare strategy.3. The relevance of the essential concept of health and the elements of the examined general health strategy was ascertained positively to respond to the demands of the current pandemic.Discussion: The value, implications, and limitations of the results obtained were discussed in the light of COVID-19 and future pandemics. Millar","doi":"10.5750/ijpcm.v11i2.1081","DOIUrl":"https://doi.org/10.5750/ijpcm.v11i2.1081","url":null,"abstract":"Background: Healthcare professional specialties, including psychiatry, have had to adapt to the unprecedented challenges created by COVID-19 by transforming the service to benefit patients and improve sustainability of the workforce.Methods: This short paper provides an experiential perspective and highlights a selection of the recent relevant guidance and key publications in the field while outlining examples of practical adaptations made by mental healthcare professionals to rapidly organize a “virtual” person-centered model of care to deliver a hybrid psychiatric service while maintaining staff mental and physical well-being.Results: Mental healthcare professionals have risen to the challenge of COVID-19 by reconfiguring the current model of care and advancing in the use of technology to provide a hybrid model of patient care. With the unprecedented situation created by COVID-19, these rapid developments promoted innovative ways of working that could be beneficial, flexible and improve patient care as well as potentially create a more healthy work–life balance for staff in the medium to longer term.Conclusions: COVID-19 is the greatest public health challenge in the 21st century to date. Mental healthcare professionals have demonstrated an ability to successfully and rapidly adapt to create a virtual way of working and provide a more flexible person-centered model of care for patients. By shifting conventional work patterns, the situation allowed the opportunity to rethink current healthcare delivery.
