The Coping Long Term with Active Suicide Program (CLASP) has three major components: (1) three individual, in-person meetings, (2) one significant other/family meeting, and (3) 11 brief (15-30 minute) phone contacts with the patient and his/her significant other. The aim of the study was to elucidate the basic mechanism of action in laser-induced root canal disinfection, which could be a photothermal and/or photochemical effect.</p></div><div><h3>Materials and methods</h3><p>To find out whether high-power NIR laser bacterial killing is caused by the light itself (photochemical effect or direct bacterial heating by selective absorption) or by a general thermal process, bacterial suspensions of a non-pathogenic strain of <em>Escherichia coli</em> were heated in a water bath and by diode laser (940<!--> <!-->nm) each with the same temporal temperature course. Furthermore, bacterial suspensions were irradiated during which the temperature was fixed by iced water. Efficacy of bacterial inactivation was determined using double fluorescence labeling to distinguish vital from dead bacteria.</p></div><div><h3>Results</h3><p>Comparison of bacteria-killing efficacy between laser and water bath-induced heating showed no significant differences. The most important parameter was the maximum temperature achieved during heating. Laser irradiation of bacteria at low temperatures (elimination of temperature effect) did not show any bactericidal effect.</p></div><div><h3>Conclusion</h3><p>The experiments show that the inactivation of bacteria, at least for <em>E. coli</em>, using high-power 940<!--> <!-->nm laser irradiation is solely based on a thermal mechanism. This should be taken into consideration during laser-assisted endodontic disinfection, where efficient bacterial inactivation must be achieved whilst preserving temperature-sensitive neighboring tissue.</p></div><div><h3>Hintergrund und Zielsetzung</h3><p>Diodenlaser finden in der Zahnheilkunde im Rahmen der Oralchirurgie und der Desinfektion des Wurzelkanals bei der Wurzelkanalbehandlung Verwendung. Das Ziel unserer Studie war, den grundlegenden Mechanismus der Laser-induzierten Desinfektion des Wurzelkanals zu untersuchen, dem sowohl ein photothermischer als auch ein photochemischer Effekt zugrunde liegen könnte.</p></div><div><h3>Material und Methoden</h3><p>Um herauszufinden, ob die Inaktivierung von Bakterien durch die Laserstrahlung (photochemischer Effekt oder direkte Erhitzung der Bakterien durch selektive Absorption) hervorgerufen wird oder von allgemein thermischer Natur ist, wurden Bakteriensuspensionen eines nicht-pathogenen Stamms von <em>Escherichia coli</em> in vergleichbarer Weise durch Bestrahlung mit einem Diodenlaser (940<!--> <!-->nm) oder im Wasserbad erhitzt. Raman spectroscopy provides vibrational information about the sample. Combined with high-resolution confocal microscopy, it allows a complete Raman spectrum to be recorded at every confocal image point. This technique was applied here for the investigation of lipid bodies in a colon carcinoma cell line.</p></div><div><h3>Materials and methods</h3><p>The colorectal adenocarcinoma cell line Caco-2 and the rat intestine epithelial cell line IEC-6 were analysed with the confocal Raman microscope alpha300 R (WITec GmbH, Germany), using a frequency-doubled Nd:YAG laser at 532<!--> <!-->nm and 10<!--> <!-->mW for excitation. The use of a water immersion lens (63×, NA 1.0) allowed a lateral resolution in the sub-micrometer range. Raman images of cells were generated from the data sets by integrating over specific Raman bands. Mapping of the C–H stretching band (2800–3030<!--> <!-->cm<sup>−1</sup>) allowed for the visualisation of the whole cell, whereas the automated statistical evaluation of all spectra by <em>k</em>-means cluster analysis resulted in spectral unmixed images which provided an insight into the chemical composition of the sample.</p></div><div><h3>Results</h3><p>With the described method, it was possible to visualise the distribution of different cellular biomarkers. Lipid bodies, in particular, which are reported to be present in increased numbers in colorectal cancer cells as compared to normal tissue, could be characterised and localised. A quantitative approach was developed to assess the fraction of lipid bodies in the total cell area. This method was applied for comparison between malignant and non-malignant cell lines. The fraction of lipid bodies turned out to be significantly higher in malignant (13.8%, <em>n</em> <!-->=<!--> <!-->21) than in non-malignant cells (1.8%, <em>n</em> <!-->=<!--> <!-->16).</p></div><div><h3>Conclusion</h3><p>Confocal Raman microscopy is shown to be a powerful method for the investigation of lipid bodies.</p></div><div><h3>Hintergrund und Zielsetzung</h3><p>Die Untersuchung von lebenden Zellen unter physiologischen Bedingungen erfordert sensitive und nicht-invasive Methoden. {"title":"Concept of a fiber guidance instrument for laser-assisted laparoscopic partial nephrectomy","authors":"Wael Y. Khoder , Ronald Sroka","doi":"10.1016/j.mla.2011.08.006","DOIUrl":"10.1016/j.mla.2011.08.006","url":null,"abstract":"<div><h3>Objective</h3><p>New developments in laser-assisted laparoscopic partial nephrectomy (LLPN) have created a demand for surgical tools which are compatible with laparoscopic equipment to ensure that laser-assisted technique is safer and reproducible. A concept was developed for a novel laser fiber guidance instrument based upon observations made during minimally invasive LLPN with a modified, commercially available ENT laser guidance instrument.</p></div><div><h3>Patients and methods</h3><p>Ten patients underwent laser-assisted laparoscopic/retroperitoneoscopic partial nephrectomy by means of fiber-assisted light application of 1318<!--> <!-->nm using a surgical diode laser (Eraser; Rolle&Rolle, Austria). Observations made during the laparoscopic procedure were analyzed with a view to developing a novel laser fiber guidance instrument taking into account intraoperative laser-related requirements.</p></div><div><h3>Results</h3><p>All laparoscopic procedures were completed without any changeovers to open surgery. Tumor locations (kidney: 5 left/5 right) were central (1/10), upper pole (2/10), lower pole (2/10) and middle kidney parenchyma (5/10). The mean operation time and blood loss were comparable to standard techniques. All patients showed a favorable outcome with uneventful postoperative convalescence. A concept for a new instrument was developed taking into consideration the drawbacks associated with the clinical handling of the used modified ENT laser guidance instrument and ensuring that the new instrument is simple, cheap and easy to produce as well as easy to handle.</p></div><div><h3>Conclusion</h3><p>The <em>in vivo</em> observational study presented here shows that the 1318<!--> <!-->nm emitting diode laser is a promising device for LLPN. Laser-based treatment facilitates for both good coagulation as well as cutting effects. The observations made resulted in a concept for an improved instrument. It is therefore conceivable that in the future there will be a wider use of LLPN for selective and precise laser resection of tumors with almost bloodless operation field and adequate fiber manipulation.</p></div><div><h3>Zielsetzung</h3><p>Neue Entwicklungen in der Laser-gestützten laparoskopischen partiellen Nephrektomie (LLPN) haben zu neuen Anforderungen an das chirurgische Instrumentarium geführt, um das Verfahren noch sicherer und reproduzierbarer zu machen.
