NSF usually develops after exposure to Gadolinium (Gd) based contrast media, which are used in magnetic resonance (MR) studies. Nephrogenic systemic fibrosis has both cutaneous and deep, visceral manifestations, involving muscles, lungs, heart and more. The course of NFS is usually progressive, with no effective treatment reported, except for renal function improvement. Awareness to NFS is important not only for early diagnosis of affected patients, but also in preventing new cases. According to current FDA guidelines for imaging patients with renal failure, use of CT with iodinated contrast media is preferred in patients with renal failure, over MR studies with Gd in patients with renal failure.
Harefuah. They are associated with significant distress and impairment. Most of the children and adolescents with anxiety disorders are not detected and only a minority receives adequate treatment. Anxious children and adolescents often follow a chronic course of disease with elevated risk of depressive disorders, substance abuse and even suicide. Risk factors for anxiety disorders are complex and consist of intricate interplay of multiple factors, including genetic makeup and environmental risks. Effective, evidence-based treatments for anxiety disorders in children and adolescents consist of cognitive-behavioral therapy and pharmacotherapy with serotonin specific reuptake inhibitors. Further research is needed to design high-quality screening tools for anxiety disorders in the primary care setting.
Harefuah. Thus, the computer-internet addiction concept has been proposed as an explanation for uncontrollable and damaging use. Symptoms of addiction could be compared to other addictive behaviors such as pathological gambling, kleptomania, trichotillomania, sex addiction and pyromania. Although criteria to diagnose this addiction have been proposed, methods of assessing excessive computer-internet use are limited. Early diagnosis could help the patient that suffers from this addiction before developing additional psychiatric diagnoses. A review of the proposed etiologies in the literature is summarized, together with recommendations for physicians and mental health officials.
Harefuah. The initial clinical manifestations of the disease may be insidious, but rapid progression to sepsis may occur despite aggressive therapeutic intervention, so early diagnosis and treatment are important to prevent the catastrophic septic consequences. Conventional radiography and abdominal sonography are often the initial imaging modalities used for evaluation. Characteristic gas around the gallbladder fossa is imperative for a diagnosis of emphysematous cholecystitis. If the differential diagnosis remains or further evaluation of the severity or localization of the disease is needed, computed tomography should be considered. Knowledge of the common predisposing conditions such as diabetes and gallstones, the pathophysiologic characteristics, and typical imaging features will greatly favor a correct diagnosis and make successful treatment possible. We report on a 51-year-old male patient who complained of severe right upper quadrant pain. The clinical course, examinations, and treatment are discussed along with a review of the literature.
Harefuah. We summarized the worldwide data regarding endoscopic treatment of VUR using various tissue-augmenting substances presently available.\u0000\u0000\u0000RESULTS\u0000In terms of effectiveness and long-term successful results, polytetrafluoroethylene is still the most reliable injectable material for the endoscopic treatment of VUR. However, Dextranomer/hyaluronic acid copolymer (Deflux) is a new promising tissue augmenting substance which might be able to replace Teflon in the endoscopic treatment of reflux in terms of a similar to Teflon reflux cessation rate and exhibiting no evidence of migration.\u0000\u0000\u0000CONCLUSION\u0000Endoscopic subureteral polytetrafluoroethylene injection is a simple and effective outpatient procedure for in the treatment of vesicoureteral reflux. It occurs because of developmental anomalies of the chamber angle that prevents drainage of aqueous humor, thereby elevating intraocular pressure. Its incidence is 1 in 10,000 live newborns in Western societies and 1 in 1,200 live newborns in the Arab-Bedouin population of the Negev region in Israel. Most cases of PCG appear to be sporadic. The cytochrome P4501B1 gene located within the GLC3A locus on chromosome 2p21 is mutated in individuals with PCG. The triad of epiphora, photophobia, and blepharospasm is classical for PCG. General anesthesia is usually required for an adequate examination of intraocular pressure, corneal diameter, optic disc, and axial length in young children. Congenital glaucoma is almost always managed surgically, with medical therapy being used only as a temporizing measure before surgery or when surgical intervention has repeatedly failed. At least 50% of eyes with PCG presenting at birth will become legally blind (visual acuity < 6/60). Patients with PCG require follow-up examinations throughout their lives.
Harefuah. Today, the only center performing SRS in Israel is the Sheba Medical Center. A multidisciplinary committee grants authorization permitting surgery after a follow-up period of two years. We found that this multidisciplinary approach leads to the best therapeutic results. We briefly present the pre-operative evaluation, operative approach, post-operative course, final results and complications of sex reassignment surgery. This article aims to inform physicians of this therapeutic option, and the indications for referral.
Harefuah.
