Some are too comminuted for open reduction and internal fixation. Recently, total elbow replacement (TER) has gained popularity for the treatment of comminuted elbow trauma when other treatment options are not possible.\u0000\u0000\u0000METHODS\u0000Since 2007-2013 we treated 18 patients with TER due to comminuted distal humeral fractures. We used the Disabilities of the Arm, Shoulder and Hand scoring system (DASH) to evaluate the patient's satisfaction. In addition we evaluated the elbow range of motion and collateral stability.\u0000\u0000\u0000CONCLUSIONS\u0000Functional range of motion was achieved with high patient satisfaction. Based on the results we conclude that TER is a reasonable option for complex elbow fractures when open reduction and internal fixation is not suitable.
Harefuah. Main challenges include the adoption of universal definition of terrorism, the settlement of the legal status of terrorists and their rights and obligations mainly in relation to human rights. It is important to clarify the responsibility of the State for terrorism (State terrorism and attribution of his actions), in relation to the population, captured terrorists, and non-state actors. The latter pose several problems in reaction to terrorist crimes, such as the legality of the use of force against them including the right of self-defence or the use of drones for targeted killing. It is contentious whether it is possible to place terrorism under existing categories of international law or it constitutes a separate act which is necessary to regulate apart from valid international rules.
Harefuah. Most patients with colon cancer are older than 50 years of age. There is no consensus in the literature regarding the behavior of this disease in young patients.\u0000\u0000\u0000AIM\u0000The aim of the present study is to compare clinical and ic pathological features of colon cancer between young and sold patients.\u0000\u0000\u0000METHODS\u0000All clinical and pathological characteristics of 200 patients with colon cancer treated at our center during the period 2001-2006 were retrospectively reviewed.\u0000\u0000\u0000RESULTS\u0000Twenty five patients (12.5%) were <50 years age (young patients) at diagnosis (mean age 41 years) and 175 were >50 years (mean age 68 years). Males were 56% of the young group and 60.1% of the old one. Arab patients were 52% of the young group, although their total number was 35 of the 200 patients. No significant difference was found in the stage of tumor at diagnosis between the two groups. Histopathological grade 3 tumors were found in 33.3% of young versus 7.7% in old patients. Surgery and chemotherapy were performed in 96% and 88% versus 95.4% and 69.7% in the two groups respectively. In a median follow-up period of 96 months, 35% of young patients died of their disease compared to 33.1% of the old patients.\u0000\u0000\u0000CONCLUSIONS\u0000Other than histological grade, no difference was found in colon cancer features and survival of young compared to old patients. Further studies with higher numbers of patients are suggested to clarify our findings.
Harefuah. However, many affected patients are not referred for surgery because of old age and significant comorbidities. In recent years, an alternative treatment approach has been developed, transcatheter aortic-valve implantation. In this review we would like to describe this treatment modality and to present several cases performed in our center.
Harefuah.
