In February 2003, cases were reported in Hong Kong and from there the disease spread to many other countries, mainly, China, Hong Kong, Singapore, Vietnam and Toronto in Canada. The syndrome was defined as Severe Acute Respiratory Syndrome SARS), and a person is suspected of having SARS if he/she became ill after November 1 2002, has a fever exceeding 38 degrees C, has symptoms of a respiratory disease and was in a risk area or in close contact with a SARS patient within ten days prior the appearance of symptoms. The World Health Organization has received reports of 4,836 cases, of which 293 persons have died. Most were family members or medical staff treating the patient, persons who came into close and prolonged contact with the patient. The estimated incubation period is two days to one week. Symptoms of the disease include fever, shortness of breath and cough. Ten percent of patients afflicted with SARS require artificial breathing. The mortality rate is 6-7%. A novel coronavirus is associated with this outbreak, and the evidence indicates that this virus has an etiologic role in SARS. Infection is transmitted from person to person through direct or close contact with airborne droplets or personal objects of an infected person. Patients must be isolated and treated by contact and airborne isolation. Treatment consists of support care and artificial respiration when required. The use of anti-viral medications has not yet proven effective.","PeriodicalId":88502,"journal":{"name":"Harefuah. Most of these malignancies are squamous cell carcinoma. In this article were describe various definitions concerning the borders of the subglottic region. A summary of pathophysiology and behavior of cancer in this location is presented. The incidence of primary subglottic carcinoma is low. The origin of most cancers in this area is the glottic cancer that extends into the subglottic region. The tumor tends to spread through lymphatic channels to the paraglottic and preglottic (delphian) nodes and secondary to the jugular chain. The delayed diagnosis is due to the lack of symptoms in the early stage of the disease and the hidden location of the tumor. The presenting symptoms are usually dyspnea and stridor. Direct laryngoscopy is essential for early detection of the tumor and the use of CT and MRI is advocated. Therapy is usually radical. Total laryngectomy and radiation therapy have both been advocated. Between 1995 and 2000, 126 patients with laryngeal squamous cell carcinoma were presented to our department. Our experience in diagnosing and treating subglottic cancer in 4 patients is hereby described. The latest literature on the subject is reviewed.","PeriodicalId":88502,"journal":{"name":"Harefuah. It is due to autoantibodies directed against specific domains of the factor VIII molecule, leading to inhibition of factor VIII binding to von Willebrand factor, to activated factor IX or to negatively charged phospholipids. This results in bleeding into the skin, muscles, gastrointestinal and genitourinary tracts, and other sites. Mixing patient plasma with normal plasma prolongs the activated partial thromboplastin time of the normal plasma and the Bethesda assay provides a quantitative estimate of the strength of the inhibitor. The selection of therapeutic concentrates for the management of acute bleeding is related to the titer of the inhibitor; if less than 5 Bethesda Units, human factor VIII may be effective, but higher titer inhibitors usually respond only to porcine factor VIII, recombinant factor VIIa or activated prothrombin complex concentrates. Corticosteroid treatment leads to disappearance of the autoantibody in 50% of patients; cyclophosphamide and cyclosporine are effective in many who do not respond to steroids. Occasionally, high dose intravenous immunoglobulin or immunosorbent columns transiently decrease inhibitor titers and enable control of bleeding. Other autoantibodies have been described against factors V, VII, XI and, rarely, factor XIII and prothrombin. New approaches in the management of autoimmune disease and, especially, methods to establish tolerance are in development.","PeriodicalId":88502,"journal":{"name":"Harefuah. The incidence of VHL disease is assessed about one in 36,000 livebirths and the penetrance is higher than 90%. Similar to other tumor suppressor gene disorders, VHL disease is characterized by frequent development of specific types of tumors in selective organs. The disease is named after the German ophthalmologist Eugen von Hippel, who identified and described characteristic retinal manifestations, and the Swedish Pathologist Arvid Lindau, who discovered the frequent co-occurrence of retinal and cerebellar hemangioblastoma with tumors and cysts in visceral organs. He described the clinical spectrum of VHL disease in detail in a large series of cases from Sweden but also from other European countries. A clinical classification system divides individuals who are affected by VHL disease into two groups: Those predominantly without pheochromocytoma are classified as VHL type 1, and those predominantly with pheochromocytoma classified as VHL type 2. VHL type 2 is further subdivided into type 2A (with renal cancer) and type 2B (without renal cancer). In type 2C affected patients develop solely pheochromocytomas. of proteins of the and that lead to upregulation and refunctionalization of mutated VHL protein.","PeriodicalId":88502,"journal":{"name":"Harefuah. Transcutaneous oxygen partial pressures are performed on the skin adjacent to tissue loss to determine perfusion status. Early detection of vascular status allows the medical community to take a more proactive role in patient care to prevent medical complications arising from diabetes and PVD. Hyperspectral Imaging (HSI) technology has profound implications and practical uses in the field of medicine. It has been developed as a screening tool for patients who are at risk of developing diabetic-foot ulcerations and tissue breakdown. HSI is designed to predict the development of diabetic-foot ulceration, with a high level of sensitivity and specificity before the ulcer becomes clinically apparent. It constructs a spatial anatomic map of tissue oximetry based on the utilization of molecular data by analyzing oxyhemoglobin and deoxyhemoglobin levels. There is a growing number of such devices currently on the market and they are being evaluated for their Introduction Accord ing to the American Diabetes Association, more than 30 million Americans are diagnosed with diabetes. More than 190 million people are affected worldwide, and the number is expected to reach a staggering 642 million by the year 2040. If left untreated, many of these people can develop foot complications, such as a diabetic foot ulcer (DFU). Foot ulceration development among the diabetic population can be as high as 25%. All cases with a DFU are associated with high morbidity and mortality and have a significantly higher risk of lower-extremity amputation than patients without diabetes. Many of these patients also have other underlying medical illnesses, such as peripheral vascular disease (PVD). Management of diabetic-foot complications within the United States has posed a huge economic burden. As complications due to diabetes are ever increasing and have an enormous economic burden on patients and their families, early diagnosis and preventative measures are essential. In this article, we will present new technologies for early detection of diabetic foot disease, wearable technologies, and new treatments in the area of advances in wound care and limb salvage.","PeriodicalId":88502,"journal":{"name":"Harefuah. He developed the concept of early evacuation from the battlefield, and of immediate treatment of the wounded, even under fire. He implemented improved surgical techniques and improved wound care that were followed by surgeons all over Europe. His devotion to wounded soldiers crossed national boundaries and became a way of life. Here is an account of Larrey's involvement in the aftermath of the Battles of Lutzen and Bautzen (May-June 1813), when many soldiers were accused of self-mutilation and were about to be executed. He dared to oppose, singlehanded, the Emperor, the highest military authorities and their concurring physicians and surgeons, armed only by his undisputed honesty, professional authority and exceptional reputation won over years of devotion to wounded soldiers. Larrey saved the lives of many soldiers and set an example of unbent ethical integrity.","PeriodicalId":88502,"journal":{"name":"Harefuah.
