This was a pre-specified secondary outcome for this study. For evaluation, Short Form-36 (SF-36) health-related quality of life (HRQoL) and the Geriatric Depression Scale-15 (GDS-15) were used. This was a single-center, single-blinded, randomized, controlled trial. Consecutive >55 years old stroke patients with risk of falls at 3-6 months after first or recurrent stroke were randomized to the intervention group (IG, n = 15) or to the control group (CG, n = 19) who received group discussion with focus on hidden dysfunctions but no physical fitness training. The 5-week high-intensive exercise program was related to an improvement in the CG in the SF-36 Mental Component Scale and the Mental Health subscale at 3 months follow-up compared with baseline values while no improvement was seen in the IG at this time. For the SF-36 Physical Component Scale, there was an improvement in the whole study group at 3 and 6 months follow-up compared with baseline values without any significant changes between the IG and CG. The GDS-15 was unchanged throughout the follow-up period for both groups. The intervention program contained high intensity functional exercises (HIFE) implemented to real-life situations together with education on falls and security aspects. This was a single-center, single-blinded, randomized controlled trial. Consecutive >55-year-old patients with risk of falls were enrolled and randomized 3–6 months after first or recurrent stroke to the intervention group (IG, n=15) or to the control group (CG, n=19) who received group discussions about hidden dysfunctions after stroke. Outcomes were Berg Balance Scale (BBS) primarily, Barthel Index (BI), Falls Efficacy Scale – International (FES-I) and number of falls secondarily and Frenchay Activities Index last 3 months (FAI-3) tertially. Assessments were done at baseline, post-intervention, 3- and 6-month follow-up by two physiotherapists and one nurse blinded to group allocation. Generalized Estimating Equations with Repeated-measure statistics were used to analyze the data. There were no significant differences between the IG and the CG regarding balance (BBS). BI at 6 months and FES-I post-intervention and 3 months follow-up showed significant improvement in the IG compared with the CG (p<0.05). Number of falls and FAI-3 were without significant change. Four physiotherapists and seven patients (three men, four women) participated in this qualitative study. Interpretative description was used to analyse data collected from multiple sources including in-depth interviews, clinical patient notes and treatment observations. Physiotherapy participants were cautiously enthusiastic of the role of GAS in HBSR, finding it useful to focus their treatment interventions on specific patient-centred goals, although it was not easy to set goals with all patients. Participants questioned the reliability of GAS as a method of measuring patient outcome. Physiotherapy in this setting was found to be complex and covered a range of interventions; interventions ensuring a process of care for patients requiring a great deal of care and support to interventions for patients functioning at a high level within their community. Two physiotherapists independently examined 132 persons (34 healthy, 32 with localized pain, 32 with widespread pain, 34 with psychoses), each using one method. Exploratory factor analysis was performed to reduce number of items. Internal consistency of the final scales was examined with Cronbach's alpha. Discriminative ability was examined with Mann–Whitney U-test and receiver operating characteristics curve area. The initial 25 items were reduced to nine and integrated into three new subscales and one main domain scale for Posture with acceptable psychometric properties. The new scale showed very good discriminating ability between healthy persons and the different patient groups (p<0.001; ROC 0.85–0.95), as well as showing that pain patients had better scores than patients with psychoses. Thirty-two consecutive patients with a femoral neck fracture were assessed by an independent physiotherapist on days 3–5 post-surgery. Follow-up telephone interviews with structured questions were conducted at 3 weeks and 4 months post-surgery. The Swedish version of the Elderly Mobility Scale (Swe M-EMS) had an inverted correlation with discharge destination (rs=−0.423). Age (rs=0.389), living alone or with someone (rs=−0.593) and the physiotherapy evaluation (rs=0.516) also correlated with discharge destination. The Timed Up & Go (TUG) (rs=0.085) and pre-fracture activity level (rs=0.186) did not correlate with discharge destination. The Swe M-EMS may be considered a predictor of discharge destination in patients with a femoral neck fracture living in the community prior to fracture and with normal mental state, but not an independent predictor. BBAT draws upon people's own health resources by promoting quality of movement. Seventeen women and one man with psychiatric disorders, ages ranging from 27 to 70 and with various diagnoses, participated in the study. All participants had attended a BBAT group for at least 6 months. Three focus group interviews were performed, and the interview material was condensed systematically with the aim to extract core meanings. The following three core themes emerged: increased awareness of one's own body and better knowledge of the self, threshold for taking part in time-consuming change, and relationships between oneself and others. The process of strengthening the experience of the lived body pointed towards feelings of wholeness, and feeling more at home in themselves and in the group. And the annual risk of stroke is 5-15% (1). In the United States more than 700 000 people suffer a stroke each year, and approximately two thirds of these survive and require rehabilitation (2). Stroke rehabilitation for milder cases is preferably conducted as home-based rehabilitation. Rehabilitation aims at helping survivors become as independent as possible and to attain the best quality of life and to facilitate return to work. In this issue of Advances in Physiotherapy, four articles deals with different aspects on stroke rehabilitation. A randomized controlled trial by Holmgren and coworkers, deals with a 5-week high intensity functional exercise (HIFE) program, implemented to real-life situations together with education on risk of falling and aspects of security. Stroke patients over 55 years of age, were randomized 3-6 months after fi rst or recurrent stroke to an intervention group or a control group. Assessments were made at baseline, postintervention, 3and 6-months follow-up. The authors suggest that the HIFE program, implemented in real life situations together with educational discussions, may improve falls effi cacy in stroke subjects with risk of falls. The authors also evaluated the benefi t of the HIFE program on health-related quality of life and on depression. The short form SF-36 and the Geriatric Depression Scale-15 were used. The authors conclude that exercise should also be complemented with discussion and education about hidden dysfunctions and not solely focused on falls and physical function, in order to obtain the best health related quality of life. Contraint-induced movement therapy (CIMT) developed by Taub and Wolf (3), is a treatment modality often used in rehabilitation of stroke patients. The treatment targets recovery of the upper limb, by limiting the use of the unaffected limb. Marklund and co-workers conducted a study where they did not focus on the upper limb, but on the lower limb CIMT. They investigated through qualitative interviews patients ’ experiences of participating in such a program. The study group, 262 occupational therapists and physiotherapists who graduated in 1999, is a sub-sample drawn from a national cross-sectional survey. Data were collected in their third year after graduation. The Swedish Demand–control Questionnaire and the Effort–reward Imbalance Questionnaire together with self-constructed questions were used to evaluate psychosocial factors at work. The results reveal that few were exposed to job strain or effort–reward imbalance (ERI). More were defined as having work-related overcommitment (WOC). Logistic regression analyses revealed a significant association between WOC and ERI, sex, degree of effort and degree of reward. One quarter was dissatisfied with their work and this dissatisfaction was significantly associated with ERI, reward (in the ERI questionnaire), control (in the Demand–control Questionnaire) and type of employer. The aim was also to investigate the reliability of the isometric strength test and the dynamic one-repetition maximum (1 RM) test in women and men. Healthy recreational weight trainers (15 women and 15 men) were tested firstly for maximal isometric strength and secondly for 1 RM-strength using an isoinertial-based (gym) knee-extension weight machine. The isometric test was carried out with the subject performing a muscle action that dislodged a particular test weight (within 5 s) and, as a result, a weighted rope fell out of the weight stack and the trial was regarded as successful. The test was repeated with increments of weight until failure. Intraclass correlation coefficients (ICC) were used to assess test–retest reliability. High ICCs (>0.90) were found for both the isometric and the 1 RM-strength tests, in both women and men. In conclusion, our new clinical test is a reliable tool for healthy, relatively strong subjects for the establishment of maximal isometric strength using an isoinertial-based weight machine. As a consequence, women more often seek healthcare than men do, and are more often on sick leave. Research shows that female patients and male patients are treated differently by physicians and that the physician's gender also influenced the choice of treatment. The aim was to study whether the patients’ and/or the physiotherapists’ gender influences physiotherapy treatments for patients with neck and/or low back pain. During 3 days in April 2006, 73 physiotherapists in primary care and private practices collected information on 586 patients with neck and/or low back pain. The information included data on the affected pain sites and the treatment procedures used by the physiotherapist. Baseline data on the physiotherapists were collected with a questionnaire. The results showed that female and male physiotherapists mainly used the same treatment procedures, but with some differences. The female physiotherapists used significantly more acupuncture and procedures directed toward treatment of mental function. They also gave their patients a unique combination of treatment procedures to a greater extent than their male colleagues. The malte physiotherapists used significantly more training of joint mobility.
