The purpose of the study is to evaluate the immunocorrective efficacy of antihypertensive pharmacotherapy in patients with arterial hypertension alone and in combination with Mexican. The open, randomized controlled trial included 68 patients with a diagnosis of stage II hypertension, grade 2 arterial hypertension, left ventricular myocardial hypertrophy, risk 3. The concentration of cytokines (tumor necrosis factor, IL-1a, IL-6, IL-2, IL-8, IL-10, receptor antagonist IL-1), components of the complement system (C3, C4, C3a, C5, C5a, factor H and C1 inhibitor) and functional metabolic activity of peripheral blood neutrophils in patients with arterial hypertension against the background of the use of antihypertensive therapy (perindopril with amlodipine and perindopril with hypochlorothiazide) separately and in combination with mexicor. As part of an open, one-center, comparative study, 320 patients who had had MI more than 6 months ago were examined. They were counted on an outpatient HR and selected the optimal maximum tolerated dose of BAB in achieving HR, then were divided into 2 groups: 1) with a resting HR <60 per min. and 2) with resting HR> 60 per min. Co-and polymorbidity in increasing the HR pathology and its severity in the studied groups were identified and compared. Том 42, No 2 181 the HR pathology was significantly more common in patients with heart rate> 60 per 1 min .: smoking (p = 0.002); latent alcohol abuse (p = 0.024); carbohydrate metabolism disorders (p = 0.01); congestive heart failure (p = 0.01) with a functional class of 3 or more (p = 0.02); anemia (p = 0.04) and latent iron deficiency (p = 0.02); respiratory failure of 2 or more degrees (p = 0.03); high situational (p = 0.01) and high personal anxiety (p = 0.007). The number of co-and polymorbidity in the increasing HR pathologies in the group with HR> 60 in 1 min. were more: more patients with three (p = 0.008), four or more (p <0.001) of such diseases and condi­ tions at the same time. The Charlson comorbidity index was also higher in this group (p <0.05). Conclusion: in 55.9% of the studied patients it was not possible to reach the target HR, it was associated with an older age (p = 0.048) and the presence of co-and polymorbidity in the increasing HR pathology. Endoscopic papillosphincterotomy with lithoextraction(stone extraction) is the “gold standard” for the treatment of choledocholithiasis. In elderly and senile patients with severe comorbidities and high operational risk, endobiliary stenting is an alternative treatment method. While performing EPST and stenting of the common bile duct, the risk of developing an ascending infection and cholangitis increases. Endobiliary stenting, like any other implantation, is not a harmless procedure. Bacterial contamination of the stent occurs in a matter of hours after installation. The presence of biofilms on the surface of the implant leads to rapid incrustation of bile salts and obturation of the internal lumen of the stent, as well as concrements on its surface. Permanent replacement of stent in a short period of time, on one hand it is not economically viable and on other hand it causes discomfort to patients. All this dictates need to improve the endobiliary stents themselves. The functional state of the skeletal muscles study moose according dynamometry hands and peak expiratory flow (associated with the strength of the intercostal muscles). The state of the myocardium was assessed by echocardiography. The most significant factors that have a significant impact on the state of skeletal muscles and myocardium in patients in different age groups are the speed of glomerular filtration of the kidneys and indirectly dependent on it indicators: total hemoglobin and potassium of blood plasma. Age dynamics of changes in such parameters as muscle strength, total protein, hemoglobin, potassium of blood plasma and glomerular filtration rate is adequately approximated by a linear function. The dynamics of changes in myocardial status indicators (mass and mass index of the myocardium, ejection fraction, end systolic and diastolic dimensions of the left ventricle) is better described by a nonlinear polynomial function with a curve of the upward or downward convexity. There was no unidirectional correlation between the decrease in the strength of skeletal muscles and the deterioration of myocardial parameters according to echocardiography. This may indicate differences in the mechanisms of involutional changes occurring in skeletal muscles, and changes in myocardium with age in patients with hypertension. Concomitant rectocele, rectal mucosal prolapse causes obstructive defecation syndrome which is manifested in obstructive defecation and fecal incontinence. Until now, the problem of surgical treatment of this pathology has not been completely solved. Unsatisfactory results of treatment are observed in 30% of patients. The article presents the results of treatment of 18 patients. For descending perineum concomitant with rectocele and rectal mucosa prolapsed they were performed abdominal sacrocolpopexy using a synthetic implant. After surgery these patients were still complaining of persistent symptoms of obstructive defecation. With defecography, ultrasound, MRI, it was shown that, despite the correction of the of the perineum level, which was -3.2 ± 0.4 cm in the rest, -5.6 ± 0.5 cm during straining relative to the pubo-coccygeal line, 2.2 ± 0.3 cm rectocele size, which were correspond to norms, absence of vaginal apical prolapsed rectal mucosal prolapsed was observed in these patients. It was the cause of difficulty emptying the rectum. Thus, it was found that this pathology is not corrected by abdominal sacrocolpopexy. All patients were additionally performed stapled transanal rectum resection (STARR). There were no serious complications in the postoperative period. Defecography revealed the absence of rectal mucosal prolapse. All patients noted improvement of emptying function. It were confirmed by anorectal functional tests and by test with balloon ejection. We estimated calcium deficiency by peroral calcium-tolerative test with taking of 0,25 mmol/kg body weight calcium gluconate by decrease of loading calciemia elimination speed during 240 min, endothelial dysfunction by brachial artery dilation in reactive hyperemia, daily blood pressure (BP) monitoring, functional pulmonary capacity, intensity of calcium transport between the extracellular fluid and the bone compare to results of examination of 30 healthy persons and 30 patients with essential hypertension. After course of treatment with taking of 500 mg calcium gluconate 2 times per day during 3 weeks the loading calciemia elimination speed increased ((1,98 ± 0,05) мкмоль/л/мин; p < 0,05) almost in two times, especially significantly ((2,23 ± 0,05 mkmol/l/min) p < 0,05) in patients with daily BP profile “night-peaker”. Том 42, No 1 lowering decreased by (4,0 ± 1,3) mm Hg (p < 0,05), brachial artery dilation in reactive hyperemia increased by 25,0 % (p < 0.05). Intensity of calcium transport between the extracellular fluid and the bone increased to (0,071 ± 0,006) mmol/min (p < 0,05), confirming slow calcium resorption from the bone tissue. The degree of bronchial obstruction decreased mainly due to large bronchi dilation by 10,0 % (small bronchi dilation was only (5,8 ± 1,9) % (p < 0,05).
