Pregnancy results in various types of complex adaptive and protective changes in the endocrine, cardiovascular, nervous and other systems of the body, that leads to deterioration in dental and periodontal health. These changes are further aggravated and become more aggressive in case of pregnancy affected by any pre-existing systemic disease. Disorders occurring in dia­ betes mellitus result in abnormal pregnancy, substantial deterioration of enamel condition, fast developing and complicated caries. At the moment, the high prevalence and multiple complications related to dental caries in diabetic pregnancy remain an open problem in dentistry and necessitate development and intro­ duction of new and effective methods of diagnosis, treatment and prevention of this pathology into clini­ cal practice of dental practitioners. A study has been carried by the author to improve diagnostic methods and treatment and preventive measures upon faults in structural and functional acid endurance of enamel and dental caries in diabetic pregnant women. The study revealed deterioration of tooth health, the high prevalence of caries and decay of calcium and phosphorus content in saliva of diabetic pregnant women in comparison with non-diabetics pregnant women. Klyomin","doi":"10.18413/2075-4728-2019-42-3-327-335","DOIUrl":"https://doi.org/10.18413/2075-4728-2019-42-3-327-335","url":null,"abstract":"In 72 % of cases, acrylic plastics are used for the manufacture of dentures, despite the fact that it can cause the denture stomatitis, among which the inflammatory reactions of allergic genesis are most relevant. The results of various studies indicate that allergic intolerance to denture materials is growing every year, and now the prevalence of denture stomatitis reaches 40 %. In order to create a mathematical model for calculating the risk of developing stomatitis on removable acrylic dentures, 54 patients aged 42 to 88 years were examined. The symptoms of stomatitis were recorded 1 month after the installation of the denture. Immediately prior the denture treatment, an anamnesis was studied and laboratory testing was performed. Oral fluid (IL-4, TNF-a and sIgA) and blood serum (IL-4, TNF-a, sIgA, total IgE, total IgG4) were studied. Allergological diagnostics included the determination of IgE and IgG classes specific for acrylic antibodies, as well as the performance of RTML with methyl methacrylate monomer. For statistical data processing, the methods of rank correlation of Kendall and binary logistic regression were used. 1 month after prosthetics, the symptoms of stomatitis were identified in 29.6% of cases. The development of denture stomatitis depended (p <0.05) on the presence of an allergic disease in patients (t = 0.32), thyroid pathology ( t = 0.20), chemical adverse factors in the workplace (t = 0.20), and on the initial concentrations of IL-4 (t = 0.31) and total IgE ( t = 0.24) in blood serum. The mathematical model for calculating the risk of denture stomatitis, created using the method of binary logistic regression, includes the use of allergy history and IL-4 concentrations in blood serum as predictors. The total share of correctly predicted results using the developed model is 78.4 %. The obtained data should be used in practical public health to predict the development of denture stomatitis. The aim of the work is to increase the effectiveness of treatment of patients with obesity and coronary heart disease (CHD) by including perindopril in therapy. Material. An open randomized study to improve the effectiveness of treatment of 84 patients with coronary heart disease in combination with obesity I-II art., of which 45 women and 39 men, the average age was (56,2 ± 4,8) years. Results. The patients were randomized into groups: the first 42 patients received perindopril in the complex therapy at a dose of 10 mg per day for 6 months, the second standard drug therapy. We used the Russian version of the SF-36 questionnaire. As a result of complex therapy in group 1, there was a significant (p < 0,05) increase in НАУЧНЫЕ ВЕДОМОСТИ | | Серия: Медицина. Том 42, No 3 quality of life on five studied scales of the SF-36 questionnaire (General Health -by 16 points, 40 %; Vitality by 15,9 points, 48 %; Physical Functioning by 29,5 points, 85 %; Sosial Functioning 10,2 points, 20 %; Role-Physical 17,8 points, 45 %), the indicator of the pain intensity scale significantly decreased (Body Pain by 24 score, 48 %). In group 2, there was also an improvement in all indicators of quality of life, which did not reach the degree of reliability, a significant (p < 0,05) decrease by 35 % of the pain intensity index (17,4 points). Conclusion. An effective method of treatment of patients with CHD and obesity is the inclusion of perindopril 10 mg/day in this cohort of patients. In the development of this disease, an important role is played by immunological reactions that are insufficiently studied. Immunological disorders were studied in 127 children in saliva aged 4-15 years, suffering from chronic periodontitis of moderate and severe degree. The control group consisted of 108 healthy children of the same age. Determination of interleukins in saliva was carried out by immuno-enzyme analyzer, and further calculation of their informativeness was performed. It was found that informative interleukins are IA-1, IL-2, IL-10. Based on informative interleukins, a discriminant model was developed that allows diagnosing chronic periodontitis in children with the required level for medical purposes. Arobaze created a discriminant model on the control sample showed good results. Erroneous diagnosis of chronic periodontitis in children in 9.6-9.8% of cases. This fact motivated on search of new ways of obtaining the best quality of an artificial limb, and need of improvement of manufacturing techniques. The purpose of this work was to improve manufacturing techniques of removable designs of dentures from besprekursorny basic polymer \"Belakril-E GO\" and to carry out comparison with the manufacturing techniques recommended by the producer. Samples were for this purpose made of new domestic besprekursorny basic polymer \"Belakril-E GO\" by different technologies: compression pressing, according to the instruction of the producer; molding formation; free casting; and the developed method. The received samples were studied regarding the content of residual monomer of methylmethacrylate in comparative aspect. The received results showed, the advanced manufacturing techniques of removable designs of dentures allow to improve their quality significantly. Molecular mechanisms responsible for the anti-inflammatory effects of statins are described, as well as clinical data on the non-lipid-lowering effect of statins on cardiovascular outcomes. Analyzed the results of a number of randomized clinical trials, including GREACE, ALLIANCE, TNT, PLANET I, PLANET II and meta-analyzes. Since, cardiovascular events and renal dysfunction are associated with increased morbidity, reduced quality of life and higher mortality rates. The prospects for further research are discussed to compare the clinical efficacy of statins (atorvastatin and rosuvastatin) in patients with acute coronary syndrome with ST elevation. The possible role of statins on endothelial cell function is discussed. The pleiotropic effects of the new generation of statins are actively explained. Using the electrometric method to study the state of the teeth, im­ portant, new information was obtained on the maturation of tooth enamel, on the diagnosis of tooth decay of enamel and dentin, damage to tooth pulp and periodontitis. Based on the materials of 297 clinical ob­ servations the change of electrical conductivity of hard tissues of teeth correlating with the depth of lesion was established. The use of electrometry of hard tissues of teeth can improve the efficiency of diagnosis of hidden forms of caries and its complications. This feature of dental care is due to the fact that dif­ ferent approaches and methods of providing such assistance contributes, first of all, to improve the health of patients, improve their quality of life. The article describes the features of dental care for patients with chronic non-communicable diseases. According to the author, the Feedback of the dentist, endocrinologist and therapist also plays an important role in improving the quality of care for patients with diabetes, as the dentist taking into account the recommendations of the endocrinologist (the use of certain drugs) and timely referral to the dentist of such patients by the therapist and endocrinologist will allow regular moni­ toring of the state of the teeth and oral cavity of patients. The author comes to the conclusion that patients suffering from diabetes mellitus are included in a special group of dental patients who should be under special control of the dentist and receive his help in a timely manner to prevent the development of com­ plications of the underlying disease and dental pathologies.
