Case of Methylmercury Poisoning 甲基汞中毒个案

Wunder, Piali Bhati","doi":"10.5206/uwomj.v90i1.13516","DOIUrl":"https://doi.org/10.5206/uwomj.v90i1.13516","url":null,"abstract":"Since the early 1960’s the Asubpeeschoseewagong Netum Anishinabek in Northern Ontario have been living with the consequences of chronic methylmercury poisoning.  From 1962 to 1970, mercury waste was dumped into the English-Wabigoon river upstream from Grassy Narrows First Nation, contaminating the water and the fish within.  In 1970, inappropriate mercury disposal was ceased, commercial fishing was prohibited, and individuals were advised not to consume fish from the river.  These restrictions and their consequences profoundly affected the economic, cultural and medical wellbeing of the Asubpeechoseewagong people.  Methylmercury poisoning, coined Minamata disease in Japan, has been one medical consequence of this pollution.  Minamata disease describes irreversible damage to the central nervous system as a result of methylmercury poisoning, including lesions in the cerebellum, anterior calcarine cortex, and surrounding the central sulcus.  Individuals commonly experience somatosensory disturbances and distal paresthesia, cerebellar ataxia and dysarthria, and visual field constriction.  This case report will describe a young Asubpeechoseewagong person suffering the early consequences of methylmercury poisoning.  Subsequently, the history of Minamata disease in Ontario, the pathophysiology of methylmercury poisoning, and investigations and management will be discussed.  It is important to note that the story of this case is not a historical perspective.  The Grassy Narrows First Nation continue to live with the results of this profound environmental injustice.  In the words of Chief Simon Fobister, “we must not deny another generation of our children a bright future by refusing to clean our river.”","PeriodicalId":87852,"journal":{"name":"University of Western Ontario medical journal","volume":"31 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2022-03-24","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"76922978","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Although almost entirely preventable by vaccine, there have been recent case surges, with almost 9.8 million cases reported globally in 2019, the highest seen since 1996. This spike in measles cases, can be attributed to several factors including vaccine hesitancy, low vaccine confidence, international travel, and poor access to vaccines. While measles cases appear to be declining across the globe since the beginning of the COVID-19 pandemic, a catastrophic measles resurgence following the pandemic is likely. With the increased public health demands related to the COVID-19 pandemic, healthcare resources have been prioritized differently, resulting in disruptions in measles case surveillance, investigation, and vaccination efforts. This shift has resulted in many measles vaccination campaigns being postponed, resulting in a large measles immunity gap in some of the world’s most vulnerable populations. Martin","doi":"10.5206/uwomj.v90i1.13932","DOIUrl":"https://doi.org/10.5206/uwomj.v90i1.13932","url":null,"abstract":"During cadaveric dissection of an 84-year-old male, a previously undocumented mixed-type (direct/indirect) inguinal hernia was found within the spermatic cord. Unlike typical indirect hernias, it did not originate from the deep inguinal ring. Instead, the hernia penetrated the abdominal wall medial to the inferior epigastric vessels, through the wall of the spermatic cord, and continued distally into the scrotum within the spermatic cord. The hernia consisted of a peritoneal sac containing compressed omental tissue. This sac protruded through a weakening in the abdominal wall and into the spermatic cord, but not beyond the superficial ring. The hernia observed presents with characteristics of both direct and indirect inguinal hernias. Consequently, it cannot be classified in the traditional way. This article examines the determinants of vaccine hesitancy and its impact during the COVID-19 pandemic. We explore current Canadian law surrounding vaccines and various ethical and empirical arguments for and against mandatory vaccination policies. Community support, as experienced in past outbreaks, may be helpful in mitigating these psychosocial effects. Examining the current unfolding of the coronavirus disease (COVID-19) pandemic for new insights, this study sought to understand how community support is represented and perceived as valuable to HCWs in written press media.\u0000Methods: Utilizing a media discourse methodology, written news media from the first month of the COVID-19 pandemic was sought for analysis. Included news media consisted of articles discussing community support for HCWs in London, Ontario.         \u0000Results: A total of forty news articles pertaining to community support for HCWs was collected and analyzed. Forms of community support depicted by press media fell into the categories of acts of solidarity, discounted goods, donations of personal protective equipment, and personal services. The underpinning perceptions of news articles was community support for HCWs had a positive influence on HCWs well-being. Direct discussions with HCWs or representatives of HCW’s in press media only further reinforced this discourse of benefice.\u0000Conclusion: This analysis shows that community support for HCWs has been present since the beginning of the COVID-19 pandemic. Ease of access and reduced cost of novel technology has revolutionized delivery of medical education. Immersive extended reality (iXR) technology may play an invaluable role in improving delivery and quality of medical education in rural and extra-planetary settings. \u0000Methods: An extensive literature review was conducted for using MeSH terms specific to iXR technology related to medical education. Relevant manuscripts were accessed from database searches and filtered based on pre-specified inclusion and exclusion criteria. Extraction of data and thematic qualitative analysis was conducted in relation to relevance to remote medical training with an emphasis on potential for deep-space exploration class missions. \u0000Results: From a total of 4005 search results, 35 final papers met inclusion criteria for this study. Current applications of virtual, augmented and mixed reality technology in medical education were explored, and themes from each modality of iXR technology were defined. \u0000Conclusions: Themes determined from the results were applied to a discussion regarding the application of iXR technology in two distinct areas: remote and rural medical training and space medicine. They were generally well-respected by their patients and trusted to testify as medical experts in cases of paternity, rape, and witchcraft at community forums. Yet this trust was capricious; female medical practitioners could easily be accused of being witches, usually for failing to heal a patient or causing illness through supernatural means. This paper examines two women who practiced medicine and were accused of witchcraft in 17th-century New England. The details and circumstances surrounding the witchcraft accusations against Jane Hawkins of Boston, Massachusetts in 1637 and Ann Burt of Lynn, Massachusetts in 1669 are gathered from court documents, journal articles, and secondary source materials. In analysing the cases of Jane Hawkins and Ann Burt, several themes are explored to explain why early female medical practitioners tread a thin line between trusted healers and witches. In Hawkins’ case, inexplicable medical maladies were often attributed to witchcraft when a woman was involved and she did not fit Puritan ideals of how a woman should behave. In Burt’s case, an accusation of witchcraft was used by male physicians to eliminate competing female healers in a crowded medical marketplace.
