Antibiotics can effectively treat LRTIs due to bacterial infection. Unfortunately, they are commonly prescribed even when the cause of the infection is non-bacterial, leading to an increase in antimicrobial resistance, mortality and health costs. This review aims to highlight the importance of procalcitonin (PCT) for the diagnosis of bacterial LRTIs. We conclude that PCT serum levels can help clinicians establish the need for antibiotics and individualise the duration of treatment in LRTIs. In addition, serum PCT provide prognostic information about the severity of the disease. However, PCT serum levels cannot inform decisions on a stand-alone fashion and should not be expected to substitute a thorough history and physical examination. It was designed to help fill gaps in our understanding of the biological complexity and clinical heterogeneity of COPD. The original goals for this study were to identify subgroups of smokers who may benefit from targeted therapies and to discover and validate intermediate biomarker endpoints. Here we review important findings from SPIROMICS over the past five years, including the impact of environmental and occupational exposures on the respiratory health of smokers, the characterisation of symptomatic smokers with preserved pulmonary function and the investigation of several diagnostic and prognostic biomarkers such as airway mucin concentration, functional small airways disease on chest computed tomography and a gene expression signature of interleukin 17A response in airway epithelium. It is considered severe in 5-10% of cases, with poor disease control despite a high burden of treatment. Severe asthma encompasses a heterogeneous pathobiology reflecting host-environment interactions at all scales of disease from the genetic to organ level. Characterisation of the T2-endotype has led to significant reductions in exacerbations for patients with eosinophilic disease, however control is incomplete in some, likely due to contributing factors that may include poor adherence to current therapy and co-existence of distinct pathological processes within the individual. In this article we consider where attention should be directed in the future in severe asthma; strategies to improve the use of current licensed therapies, therapeutic targets beyond T2 inflammation, and how expectations should adapt to reflect progress in managing disease overall. The first refers to SubPopulations and InteRmediate Outcome Measures in COPD Study (SPIROMICS), a multicentre, cohort study of nearly 3000 never-smokers and ever-smokers with and without COPD. This outstanding work is reviewed by Wassim W Labaki, from University of Michigan (Ann Ar bor, MI) together with two distinguished members of the Global Initiative for Obstructive Lung Disease (GOLD) Science Committee, FJ Mar tinez, from Weill Cornell Medical College (New York, NY), and MeiLan K Han, from the same university as the first author, who leads the publication, a talented worldwide COPD researcher. This huge observational study focuses on the natural history of COPD, funded by the United States (US) National Institutes of Health (NIH), and represents a milestone in the effort to further phenotype the challenging complexity and heterogeneity of COPD. Indeed, SPIROMICS has gener ated significant knowledge on the clinical phe notypes of COPD through the identification of distinct subgroups and the investigation of various chest and imaging biomarkers. Through the description of the major findings achieved by SPIROMICS, the reader is invited to pay special attention to table 2 that includes a very thorough summary of the strengths and shortcomings of the study. I will give just one example, that one of on exposures. It is observed that second-hand smoke exposure and ever-use of e-cigarettes are associated with a higher burden of respiratory symptoms in subjects with COPD; however, a lot of work on long-term effects of e-cigarette use on respiratory health, lung function and clinical effects in individuals with COPD remains to be investigated.","PeriodicalId":87223,"journal":{"name":"Barcelona respiratory network reviews","volume":" ","pages":""},"PeriodicalIF":0.0,"publicationDate":"2019-09-20","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"44702963","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

It is based on the elimination of six foreign inert gases simultaneously infused intravenously, with computer analysis of their elimination pattern to deduce, qualitatively and quantitatively, the shape of the responsible V . a/Q . distribution. After summarising MIGET’s physiological basis, this article presents the key observations made using MIGET over many years in health and in three chronic lung diseases – chronic obstructive pulmonary disease (COPD), bronchial asthma and interstitial lung disease. While significantly advancing our understanding of the pathophysiology of these diseases, the findings have led to many still unanswered questions of clinical significance in each case. These questions are also presented and discussed. (BRN Rev. 2019;5(4):294-309) Corresponding author: Peter D. Over the past five years, GINA has initiated substantial changes in key concepts about asthma and its management, and in the structure and content of the GINA strategy report. These changes have sometimes generated controversy, but they have been welcomed in primary care, where most asthma is managed. The changes discussed in this review include a new definition of asthma that emphasises its heterogeneity; expansion of the scope of evidence considered by GINA, to give priority to both internal and external validity of research findings; and the integration throughout the report of practical advice about tailoring of treatment across the whole spectrum of asthma severity. The GINA report provides not only evidence-based recommendations, but also multiple practical resources for implementation in clinical practice. It is now clear that COPD is heterogeneous, different components of the disease being present in different patients. Yet, the diversity of COPD pathophysiology, severity and how this relates to disease prognosis and treatment outcomes is far from understood. In order to address this, mathematical techniques such as cluster analysis have been employed to identify subgroups or clusters of COPD patients with differing disease attribute profiles. However, significant methodological shortcomings call into question the validity of the COPD clusters identified in such studies. Furthermore, few published studies relate COPD clusters to underlying disease mechanisms and treatment outcomes. Where this has been addressed, progress has particularly been made for patients with an eosinophilic-predominant profile. In order to maximise the usefulness of COPD cluster analysis studies, we propose that future studies must implement more stringent methodologies and focus on COPD inflammatory biology. Under the senior authorship of Ron Crystal, a legend in respiratory research since the nineteen seventies1, this paper focuses on the complex genetic architecture in COPD resulting from multifaceted interactions between environmental exposures, genetic susceptibility and gene-by-environment interactions. Although the most important risk factor for the development of COPD continues to be long-term cigarette smoking, environmental factors cannot be ignored. Only 15-20% of ever smokers develop COPD and not all of the patients with COPD have a history of tobacco smoking, which points to an individual susceptibility to environmental stress and/or factors independent of environment exposures, including genetic susceptibility and gene-by-environment interactions. All in all this backdrop contributes to disease development along with the familial aggregation observed in COPD and in monogenetic conditions, such as alpha-1 antitrypsin deficiency, hence supporting a role for genetic susceptibility in disease pathogenesis. Within this context, key issues such as exome sequencing, expression quantitative trait loci, genome-wide association study (GWAS) and single nucleotide polymorphism (SNP), along with epigenetic and integrative approaches, are attractively exposed to provide a more thorough picture of COPD hereditability.","PeriodicalId":87223,"journal":{"name":"Barcelona respiratory network reviews","volume":" ","pages":""},"PeriodicalIF":0.0,"publicationDate":"2019-07-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"44353416","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

It was based on existing birth cohorts to redefine classical phenotypes and identify novel phenotypes of immunoglobulin (Ig)E-associated asthma, allergic rhinitis and dermatitis. The project included a wide range of population-based, clinical and mechanistic studies applying systems medicine. MeDALL demonstrated the existence of the multimorbidity of eczema, rhinitis, and asthma both in IgE-sensitized and non-sensitized children and integrated the evidence on multimorbidities and polysensitization in a new framework of allergic diseases. Innovative approaches included unsupervised statistical modelling, computational analysis of the topology of the protein interaction network and the IgE MeDALL allergen-chip. Mechanistic studies included testing candidate biomarkers, epigenetics, and transcriptomics. Assessment of individual and environmental factors included puberty, maternal smoking and pregnancy.
