Karatieieva","doi":"10.21802/artm.2023.2.26.46","DOIUrl":"https://doi.org/10.21802/artm.2023.2.26.46","url":null,"abstract":"Since such a team game as football is a high-intensity sport that requires athletes to be active for 90 minutes, covering distances of up to 7 miles during a game on the field, according to Dr. Howard Libeskind, an orthopedist for the U.S. national soccer team. In our opinion, although many methods are currently available to assess the anatomical parameters of the body, there is no criterion methodology specifically defined for football players. Any coach wants to achieve the maximum possible result, especially in football. So, for the purpose to establish the anatomical parameters of the \"University\" football masters team of Ukraine, a study was conducted on 32 respondents aged from 16 to 18 years. The main group was made up of 16 players of the \"University\" football team of masters of sports of Ukraine. The control group consisted of 16 young boys that are studying at a higher education institution in Bukovyna (Bukovyna State Medical University). The representatives of the main group were practically healthy (no history of congenital or chronic pathology was noted), masters of sports of Ukraine, who systematically trained intensively and participated in championships of Ukraine among higher educational institutions, under the leadership of the team coach. Training took place 3-4 times a week, 1.5 hours on average. The subjects of the control group were also practically healthy young men who were loaded with hours of physical education, according to the programs of their specialty, and additionally did not play sports. \u0000Anthropometric examination included determination of total (body length and weight) parameters and partial (length of upper and lower limbs, thigh length, chest circumference during inhalation, exhalation and at rest, pelvic circumference, thigh circumference in the upper third, in the middle and lower third). Statistical analysis of the obtained data was carried out using the licensed program RStudio. A paired t-test (t-test of paired samples) was conducted to compare the indicators of the studied main and control groups. According to the results, the length of the right upper limbs is on average 78.50±2.02 cm, the left - 78.75±2.02 cm. The length of the right lower limbs is 92.63±2.06 cm, the left - 92.44±2 .06 cm. The average length of the right and left thigh is 52.25±2.04 cm. The circumference of the thigh in the upper third on the right is 55.31±2.03 cm, on the left – 54.18±2.03 cm, in the middle third on the right thigh, the indicator was 49.13±2.01 cm, on the left - 52.44±2.01 cm, in the lower third on the right, the average indicator is 44.31±2.06 cm, while on the left - 45.00± 2.06 cm. \u0000Football players have a lower weight index (± 2.54 kg), in contrast to the studied control group, taking into account the fact that the height in both groups is almost the same. More than 687 million people at least once had the positive COVID-test. Moreover, more than 27 thousand people are suffering from this disease in Ukraine now. SARS-CoV-2 virus can damage various organs and systems, primarily the respiratory system that will manifest in a dry cough, shortness of breath, general weakness, up to the development of acute respiratory distress syndrome, shock and acute heart failure. The damage of lover by SARS-CoV-2 virus can be both direct and indirect. The presence of liver damage often leads to a more severe course of acute coronavirus disease. In order to treat new cases more effectively and deal with complications of old ones it is necessary to study better the mechanisms of the viral effect on various organs, in particular the liver. The best way to find out this mechanism is to examine main laboratory and instrumental indicators, learn about the changes in histological sections. \u0000Aim. The aim of the research was to investigate the results of up-to-date researches of the main changes in laboratory and instrumental indicators because of acute coronavirus disease, as well as possible mechanisms of liver damage. \u0000Materials and methods. Analysis of open publications of Scopus, Web of Science, PubMed for the last 10 years. \u0000The results. In patients with acute coronavirus disease an increase in the levels of transaminases (in most cases due to alanine aminotransferase), lactate dehydrogenase, bilirubin, prolongation of prothrombin time, an increase in the level of D-dimer, and a decrease in the number of platelets and albumins were often observed. Histological examination of biopsies revealed diffuse lung damage, significant desquamation of pneumocytes and hyaline membranes, extensive interstitial fibrosis with areas of fibrinous exudation and inflammation in the alveolar space. Hepatocyte apoptosis, moderate microvascular and minor macrovascular steatosis were observed in the hepatic tissue. Thus, we can suppose that the possible mechanisms of liver damage are direct virus damage, immunomodulated inflammation, hypercoagulative processes, hepatic hypoxia, drug-induced damage. Virus SARS-CoV-2 can get into the cell using angiotensin-converting enzyme 2 receptors. Direct virus influence can be possible because of high quantity of ACE2 receptors in bile-ducts and hepatocytes. Great amount of them is also present in vascular endothelium. As a result of apoptosis of COVID- infected cells a great amount of cytokines is released. This can lead to immunomodulated damage of different organs including liver. Moreover, inflammation leads to hypercoagulable state that in future can cause hypoxia of hepatic tissue because of thrombosis of liver vessels. Ostafiychuk","doi":"10.21802/artm.2023.2.26.65","DOIUrl":"https://doi.org/10.21802/artm.2023.2.26.65","url":null,"abstract":"According to the WHO, every second woman has a problem with excess weight, while about 23% of women indicate a pathological body mass index, and excess weight (the so-called “pre-obesity”) is noted in 25% of women. \u0000The aim of the research was to evaluate the structural and functional parameters of the endometrium and ovarian tissue and to optimize the diagnostic algorithm and monitoring of the endometrium in women with obesity and impaired reproductive function. \u0000Materials and methods. 110 patients of reproductive age with impaired reproductive health underwent a comprehensive clinical and laboratory examination within the framework of this research. The main group included 64 patients with overweight and class 1 obesity, and the comparison group consisted of 46 women with a normal body mass index. The proposed diagnostic algorithm included a sonographic study of the structural parameters of the endometrium and ovaries, as well as an assessment of the features of the blood supply to the pelvic organs and statistical processing of the results. \u0000Research results and their discussion. The presented results allow us to note a significant percentage of comorbidity of somatic and gynecological pathology in overweight and obese patients. Pregnancy is accompanied by a higher percentage of early reproductive losses (14.0%), preterm birth (10.9%), preeclampsia (42.2%), placental dysfunction (46.4%) and low birth weight (17.2%). The main clinical forms of endometrial pathology in the main group were polyposis (32.8%), endometrial hyperplasia (23.4%) and dyschronosis of morphological changes of the endometrium (28.1%). The combination of endometrial hyperplasia with/or uterine leiomyoma, adenomyosis, benign ovarian tumors and retention formations was noted in 53.6% of observations in both groups; in 46.4% of women the isolated form was noted. The morphological characteristics of endometrial polyposis allowed us to note the dominance of glandular-fibrous polyps (45.2%) and in the case of a combination with myoma or ovarian retention cysts - glandular forms (32.3%). The most frequent sonographic marker of dysfunction of the endometrial potential (inconsistency of the ultrasound image of the endometrium with the phase of the menstrual cycle) was revealed in more than half of the cases (62.5%). Sonographic evaluation of the structural parameters of the endometrium and ovarian tissue showed an increase in the anterior-posterior size of the uterus and the thickness of the endometrium by 2.8 times, the volume of the ovary – by 2.0 times. It should be noted that the patients of the main group had a greater proportion of cysts and endometriomas. According to dopplerometry data, significantly lower parameters of the vascular resistance index were confirmed in the ovarian arteries and in the arteries of the stroma; in almost half of the observations, varicose veins of the parametrium were noted. \u0000Conclusions. {"title":"A DIFFERENTIATED APPROACH IN THE TREATMENT OF ACUTE FLOATING VENOUS THROMBOSIS OF THE INFERIOR VENA CAVA SYSTEM","authors":"Y. Unfortunately, there are no randomized studies in the world regarding the determination of clear tactical approaches to the treatment of floating venous thrombosis, in many cases doctors make decisions about the choice of surgical tactics based on their own experience. In our research, we tried to systematize our own experience and form the basic principles of determining surgical tactics in the treatment of floating venous thrombosis. To date, anticoagulants, surgical interventions in the form of thrombectomy, and thrombolytic therapy are used in the treatment of DVT. Unfortunately, at the moment, no consensuses clearly define the issues of surgical treatment of floating venous thrombosis, venous thrombosis in pregnant women, venous thrombosis complicated by pulmonary embolism, venous thromboembolism. \u0000The goal. The aim of the research was to develop a surgical tactics in the treatment of floating DVT of the inferior vena cava. \u0000Materials and methods. The treatment of 76 patients with floating DVT from 2008 to 2020 were analyzed. \u0000In a research among patients with floating DVT in 15(20%) patients experienced thrombosis of the inferior vena cava, 19(25%) patients had ilio-femoral thrombosis in 17(22%) patients with femoral - popliteal thrombosis in 9(12%) patients with popliteal - tibial. \u0000Deep vein thrombosis complicated by pulmonary embolism with medium and high risk of death was observed in 10 (13%) patients. Among them, floating thrombosis of the iliac-femoral segment was observed in 4 patients. \u0000Floating thrombosis of deep veins in pregnant women was observed in 6 (8%) patients. \u0000Among patients, 20 (296%) patients were admitted within 1 day of the onset of the disease, 30 (39%) patients on 2-3 days, 11 (14%) patients on 4-7 days, 8 on 8-14 days (11%) patients, from the 15th day - 7 (10%) patients). \u0000The results. Patients with long of floating part of the more than 4 cm, we used an active surgical tactics. In the presence of the floating thrombus a total iliac vein thrombectomy we performed in 12 patients with the removal of the floating thrombus with femoral access. For the prevention of pulmonary embolism in these cases we implanted a temporary cava-filter. When flotation thrombus in the external iliac vein, we used the technique of thrombectomy using Fogarty two catheters in 12 patients. When flotation clots in common femoral vein and distally using a vein thrombectomy with clamping head above floating thrombus. When treating patients with floating DVT, pulmonary embolism complicated with medium and high risk of death, we used systemic thrombolysis with pre- implantation temporary coffee filter in patients with floating DVT, pulmonary embolism complicated with medium and high risk of death, we used systemic thrombolysis with pre- implantation temporary coffee filter. The next page, «Nursing» is designed for students of specialty 223 «Nursing». Virtual learning blocks have been created for students, which contain videos and algorithms for various manipulations, test tasks, materials for preparation for the Krok M exam, tactical medicine guides, and more. \u0000From the «Nursing» page, one can navigate to the «Dentistry» page, designed for students of specialty 221 «Dentistry» and the «Pharmacy» page, designed for students of specialty 226 «Pharmacy». \u0000As part of the cooperation between the professional medical college of the Ivano-Frankivsk National Medical University and the Andrey Krupinsky Lviv Medical Academy before the war, various events took place. {"title":"THE EFFECT OF SMOKING ON THE INDICATORS OF GLUTATHIONE-DEPENDENT ENZYMES OF ORAL FLUID IN TEENAGERS AND YOUNG ADULTS","authors":"I. Lisetska, M. Центральну позицію у забезпеченні антиоксидантного захисту організму людини посідає потужна глютатіонова антипероксидантна система, що приймає участь в трьох лініях захисту з чотирьох. Вона представлена відновленим глутатіоном і ферментами його метаболізму: глутатіонпероксидазою, глутатіонтрансферазою і глутатіонредуктазою. \u0000Мета дослідження – вивчити вплив паління на показники глутатіон-залежних ферментів ротової рідини в осіб підліткового та юнацького віку. \u0000Матеріали та методи. Проведено вивчення показників глутатіон-залежних ферментів ротової рідини у 114 осіб підліткового та юнацького віку від 15 до 24 років, яких розділено на групи: у І групу включили 26 осіб, що палять традиційні сигарети; у ІІ групу – 22 особи, що палять електронні сигарети (Вейпи); у ІІІ групу – 23 особи, що палять пристрої для нагрівання тютюну (IQOSи); у ІV групу – 43 особи, що не палять. \u0000Результати дослідження та обговорення. Глутатіон-залежні ферменти ротової рідини характеризувалися активацією глутатіонпероксидази та значною недостатністю функції глутатіонтрансферази та глутатіонредуктази. Так, активність глутатіонпероксидази ротової рідини в осіб І групи підвищувалася в 2,8 рази, у осіб ІІ групи - в 2 рази та в ІІІ групі - 1,5 рази порівняно з особами IV групи, (р<0,05). Функціонування ферменту глутатіонтрансферази характеризується значним зниженням його активності: в обстежених І групи – активність знижувалася в 6,7 рази, в обстежених ІІ групи - в 2,6 рази, в обстежених ІІІ групи - 1,8 рази порівняно з особами IV групи, (р<0,05). Результати активності ферменту глутатіонредуктази свідчить про його зменшення: в осіб І групи відбувається зниження активності ферменту в 2,9 рази, в осіб ІІ та ІІІ груп - в 1,6 рази порівняно із з особами IV групи, (р<0,05). \u0000Висновки. {"title":"THE COMPLEX EFFECT OF VITAMIN D AND ALPHA TOCOPHEROL ACETATE ON THE LUNG MICROCIRCULATION IN THE EXPERIMENTAL INDUCTION OF SYSTEMIC SCLEROSIS","authors":"B. Doskaliuk, L. The aim of the study was to investigate the effects of vitamin D3 (vitamin D) and alpha-tocopherol acetate (vitamin E) on the microcirculatory vessels of the lungs in an experimental model of systemic sclerosis (SSc). \u0000Methods. We divided the experimental animals into four separate groups: an intact group (IG) consisting of 15 animals, a control group (CG) of 20 animals, experimental group 1 (EG1) and experimental group 2 (EG2) consisting of of 25 animals each. Randomization of groups was carefully performed using the weight variance minimization method. To induce SSc in laboratory animals of EG1, subcutaneous injections of 0.5 ml of 5% sodium hypochlorite (NaClO) were administered 3 times a week for 6 weeks in a row. The CG of rats received an isotonic solution following the same scheme, while the IG was not subjected to any injections. In addition to NaClO, the EG2 received intramuscular injections of vit E (10 mg/100 g) and vit D (1000 IU/100 g) for three weeks during the second half of the experiment. \u0000After 8 weeks, the animals were euthanized, and lung tissue samples were obtained for histological and electron microscopic analysis. The histological preparations were examined using light microscopy and photographed using a Leica DME light microscope, a DCM 900 digital microscope camera, and a Nicon Coolpix P5100 camera. Meanwhile, the electron microscopy was performed to visualize the ultrastructural characteristics of the lung tissue by use of \"PEM-125K\" (Selmi, Ukraine) with subsequent photography at magnifications from 2000 to 20000 times. \u0000All animal procedures were carried out in strict compliance with bioethical principles. The Ethics Commission of the IFNMU granted approval for the study under document number 117/20 on November 19, 2020. \u0000Results. The histological analysis of the lung specimens of CG animals showed a normal morphological structure. In the EG1, the lungs exhibited abnormalities in the structure of the hemocapillaries, including narrowing of the vessel lumen due to thickening of the vascular wall, edema, and infiltration of polymorphonuclear cells. However, in the EG2, there was an improvement in the histological features, with a reduction in edematous processes and a decrease in the intensity of polymorphonuclear cell infiltration. \u0000The electron microscopic examination of the lung samples from EG1 animals revealed the presence of platelet adhesion and aggregation, as well as aggregates of erythrocytes within the hemocapillary lumen. Swelling of endothelial cells and narrowing of the hemocapillary lumen were also observed, along with uneven thickening of the basement membrane. In contrast, the combination of vitamins D and E in EG2 animals resulted in improved rheological properties of blood and ultrastructural characteristics of the hemomicrocirculatory elements. The lumen of the hemocapillaries contained single erythrocytes and inactive platelets. The analysis is designed to increase the reliability of accounting information, to objectively, rationally and realistically imagine the course of processes and the results obtained. \u0000Organizations exist to achieve a certain goal, and it is the manager who determines the organization's goal and means of achieving it. The planning component of a medical institution of higher education involves defining its goals as an organization, outlining an overall strategy for achieving those goals, and forming a comprehensive hierarchy of plans for integrating and coordinating its activities. Setting goals keeps work on track and helps organizational members focus on the most important things. In addition, planning helps clarify the consequences of the actions that managers must take to respond to change. Therefore, planning is exactly what managers need in a chaotic environment. \u0000Managers are also responsible for the structure of the organization. Such activity of the manager is called organizational. It contains a definition of which tasks should be performed, who should perform them, how the tasks should be grouped, who reports to whom, and when decisions should be made. \u0000In order to make the results of joint work of a team of specialists useful, managers need to organize it in such a way that joint efforts are clearly coordinated, that is, correctly selected participants of joint work, each of whom would clearly imagine the scope of his work and its contribution to the final result. The means of achieving the goal must be agreed upon. \u0000Every organization consists of people and leading these people and coordinating their activities is part of the manager's responsibilities. When managers motivate employees, direct the activities of others, select the most effective communication channel, or resolve conflict between group members, they are managing people. \u0000The fourth type of activity performed by managers is control. Formulating goals, making plans, identifying structural changes, training and motivating people may not be enough. Actual results should be compared with predefined tasks. In the presence of serious deviations, the manager is obliged to correct the chosen direction of the organization. It is this method of tracking, comparing and adjusting that is understood as the process of control by the organization. \u0000Organizational goals indicate to managers a specific direction of activity. However, simple definition of goals and their recognition by employees is not enough; it does not guarantee that the necessary actions will be carried out in practice. An effective manager must ensure that the actions of others are carried out as they should be, and that the set goals are actually achieved. \u0000Control is the last connecting link, connecting element of the functional chain of management. Незважаючи на постійний прогрес та вдосконалення методів діагностики та лікування хворих на КРР, безрецидивна виживаність залишається низькою. Нами було проаналізовано результати обстеження та лікування 188 хворих на КРР ІІ-IІІ стадій (Т2-3N0-2М0), у яких виник рецидив або продовження хвороби в КНП «Прикарпатський клінічний онкологічний центр Івано-Франківської обласної ради» в період 2012-2020 роки. Аналізували такі прогностичні фактори: стадія, стать, вік пацієнта, локалізація пухлини, кількість уражених лімфовузлів, інвазію пухлини до серозної оболонки, некроз пухлини та наявність периопераційного хіміопроменевого лікування. Під час визначення прогностичних факторів було встановлено достовірний прямий непараметричний зв'язок між наявністю післяопераційних ускладнень та безрецидивною виживаністю хворих, а також отриманням цими хворими курсів ад'ювантної хіміотерапії (p <0,01). При аналізі локалізації пухлини не виявлено достовірного зв'язку з безрецидивною виживаністю хворих (p> 0,05). Встановлено, що кількість уражених лімфатичних вузлів не впливає на безрецидивну та загальну виживаність хворих (p = 0,35075; p = 0,08584), але факт ураження лімфатичних вузлів має прямий вплив на безрецидивну та загальну виживаність хворих (p = 0,01869; p = 0,00919). Встановлено прямий кореляційний зв'язок у хворих з раком прямої кишки між дозою телегамматерапії та протяжністю пухлини (r = 0,37; p <0,05). Early detection of asymptomatic target organ damage to determine additional risk is important, because pressure-dependent subclinical damage to several organs reflects progression along the cardiovascular disease continuum. The SCORE scale is based on data from a number of recent European multicenter studies and takes into account all variants of fatal cardiovascular events over a 10-year period. However, the SCORE scale does not take into account the presence of such subclinical risk factors for the development of cardiovascular complications as insulin resistance, endothelial dysfunction, microalbuminuria \u0000The aim was to assess the ten-years risk of development of cardiovascular complications in hypertensive's with metabolic syndrome, depending on the type of insulinemia, endothelial dysfunction, and the presence of microalbuminuria. \u0000Materials and methods. Were examined 96 patients (34 men, 62 women) with arterial hypertension and metabolic syndrome, average age (57±9) years. Office blood pressure measurement, the level of total cholesterol in the blood and microalbuminuria in daily urine were determined for all patients. An oral glucose tolerance test was performed with parallel determination of the level of endogenous insulin, endothelin-1, and vasculoendothelial growth factor in blood serum by the immunoenzymatic method. Patients were divided into three groups. Group 1 included 29 patients with normal basal and postprandial levels of endogenous insulin; in the 2nd group – 39 patients with reactive hyperinsulinemia, in the 3rd group – 28 patients with spontaneous hyperinsulinemia. To estimate the total cardiovascular risk, the version of the SCORE scale for countries with high cardiovascular mortality was used. Moderate risk criteria are defined – SCORE index level 1-4%, high risk – 5-9% and very high risk – ≥ 10%. \u0000Research results. A moderate cardiovascular risk prevailed in almost 52% of patients of group 1 with a normal level of endogenous insulin, high – in 35% of patients group 2 with reactive hyperinsulinemia, very high – in 48% of patients group 2 and 43% of patients of group 3 with spontaneous hyperinsulinemia. The highest levels of systolic and diastolic blood pressure were characteristic of high-risk patients with spontaneous hyperinsulinemia combined with impaired glucose tolerance, with increasing of total cholesterol levels in 1.7 times under obesity, increasing of endothelin levels by 7 times, vasculoendothelial growth factor in 2.7 times and the presence of microalbuminuria in 75% of cases. \u0000Conclusions.
