This probably accounts for the many reports that have emanated from resource-limited settings in the last decade. Reports highlighting bone infections and untoward complications in HIV-infected children are rare. This informed our decision to report the case of a 9-month-old HIV-positive Nigerian girl who presented with blistering dactylitis of the right middle finger. Plain radiograph of the right hand showed amputation of the terminal phalanx and changes consistent with osteomyelitis in the middle phalanx of the third finger. The case is discussed with a view of highlighting this rare complication and the challenges associated with management in a resource-limited setting.</p>","PeriodicalId":86092,"journal":{"name":"Journal of the International Association of Physicians in AIDS Care (Chicago, Ill. : 2002)","volume":"10 5","pages":"280-2"},"PeriodicalIF":0.0,"publicationDate":"2011-09-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.1177/1545109711399444","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"29822905","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Bidirectional travel between the United States and the Caribbean region by providing opportunities for sexual mixing may contribute to these high HIV rates. Caribbean immigrants face further risk because of limited health care access, social isolation, and stigma. Additionally, although substance abuse may not represent a major health issue in their countries of origin, Caribbean immigrants are composed disproportionately of adolescents who are at greatest risk of substance abuse. There is little information on the health care characteristics of these migrants, especially regarding HIV care. This article describes how the social and economic circumstances that surround the lives of people from the Caribbean and the challenges of the acculturation process have placed these individuals at risk of substance abuse and HIV infection. A total of 1138 women were surveyed between April and August 2006 using a self-administered survey instrument. A total of 82% of the women in the sample group reported a history of HIV testing. Logistic regression analysis revealed that those with a history of alcohol use, marijuana use, and other illicit drug use were more likely to report a history of HIV testing (P < .05). These findings suggest the possibility that women who engage in riskier behaviors may be aware of these risks and therefore make more of an effort to determine their status. Studies have shown that fenofibrate has the potential to increase serum creatinine, a phenomenon that seems to occur more in patients with baseline renal dysfunction or who are on medications that can affect renal hemodynamics. HIV-infected individuals are at an increased risk of developing renal problems and may be a target population for fenofibrate-induced increases in serum creatinine. The purpose of this report is to describe a case of an HIV-infected individual who experienced a considerable increase in serum creatinine after initiating fenofibrate, and to highlight this underreported yet important adverse effect. The study was conducted from 1996 to 2004 and is among the most comprehensive ever undertaken on HIV risk behaviors of drug users in Puerto Rico. This paper presents the main findings, many of which have been published in scientific journals. The study found that drug users in Puerto Rico became infected with HIV at a rate almost 4 times higher than Puerto Rican drug users in New York, and they died at a rate that was more than 3 times as high. The findings indicate that drug users in Puerto Rico are more likely than Puerto Rican drug users in New York to engage in injection drug use and sexual behaviors that put them at risk of becoming infected with HIV. In addition, they have fewer prevention resources available to them. HIV prevention programs are scarce in Puerto Rico and the availability of drug treatment programs in Puerto Rico declined by over one third during the period examined. Additionally, significantly fewer HIV-positive drug users in Puerto Rico were taking HIV-related medications than in New York. The paper concludes with recommendations and lessons learned from the study.</p>","PeriodicalId":86092,"journal":{"name":"Journal of the International Association of Physicians in AIDS Care (Chicago, Ill. : 2002)","volume":"10 4","pages":"248-59"},"PeriodicalIF":0.0,"publicationDate":"2011-07-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.1177/1545109710397768","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"29772753","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The prevalence of vitamin D deficiency (< 30 ng/ml) was 40.65% (87/214). After this analysis, hypercholesterolemia and use of current antiretroviral regimen ≥ 3 years remained independently associated with vitamin D deficiency.
