These systems must take a number of needs into account, ranging from ease of delivery to effectiveness of the drugs. The paper reviews an overview of a conventional and novel approach in the topical drug delivery system. Drug delivery via the skin is becoming progressively popular due to its convenience and affordability. The skin is the most important mechanical barrier to the penetration of many drug substances and acts as an ideal site to deliver the drug both locally and systemically. The topical route has been a favored route of drug administration over the last decades. Despite conventional topical drug delivery systems limits in poor retention and low bioavailability. To address some of the limitations posed by conventional dermatotherapy, nano-based technologies have been developed and have demonstrated a significant improvement in dermatotherapy. Their distinct physicochemical properties demonstrate their overall superior therapeutic efficacy in providing sustained and effective targeted drug release, as well as improved solubility of hydrophobic actives with optimized drug formulations. As a result of certain limitations such as drug absorption, poor bioavailability, first-pass hepatic metabolism and medicine target to particular organs, may cause problems for administration via oral route. Therefore parenteral route, transmucosal route and transdermal route are preferred over oral route. Intranasal route is deemed to be a desirable route because of the time profile of concentration a drug is close to that of the intravenous route. The nasal route has also been successfully used for bypassing the blood-brain barrier and afterword delivering drug molecules to the central nervous system .The in-situ gelling system is a process in which   is in a solution form before the administration in the body but it converts in to a gel form after administration . The formation of gel depends on factors like temperature modulation, pH change, presence of ions from which drug get released in sustained and controlled manner. PrEP is one of the preventative measures that is effective in preventing HIV. Different clinical trials and population studies have shown PrEPto have an efficacy rate of 40% to 90% when it comes to preventing HIV. Due to its high efficacy, PrEPcan reduce new HIV infections populations considered to be high risk. However, for efficacy to be high, there is a need to increase medication adherence and address the existing barriers that limit uptake. Some of these barriers are lack of knowledge, inadequate access, stigma,lack of trust in service providers, low perception of HIV risk, cost, and fear of side effects. Addressing these barriers can increase PrEP uptake and help in reducing HIV’s risk among high-risk populations. Besides the other regular constituents it has oligosaccharides in it which are indigestible by the infant yet are consumed by the microbial populations in the developing intestine. These oligosaccharides are believed to facilitate enrichment of a healthy infant gastrointestinal micro biota, often associated with bifidobacteria. Goat milk is a rich source of oligosaccharides that have enormous biological activities such as anti-bacterial, immunological, anti-inflammatory, hypoallergenic and therapeutic properties. It is used against tuberculosis in folk medicine and also helps in the enhancement of platelet count during dengue fever. Goat milk oligosaccharides have anti-inflammatory properties and are involved in the repairing process after a dextran sodium sulphate-induced colitis. Keeping in mind the biological importance of goat milk and the role of oligosaccharides, in the present studies, goat milk was analyzed for its oligosaccharide content which led to the isolation of a novel tetrasaccharide, ducose, C28H48O21N2. However, sEH, an enzyme that breaks down EETs into diols, limits these benefits. The development of sEH inhibitors (sEHIs), particularly those based on 1,3-disubstituted urea, has shown promise in enhancing the therapeutic properties of EETs. These inhibitors are antihypertensive and anti-inflammatory and can protect the heart, brain, and kidneys from damage. In other words, it represents an abnormal scar formation. The tissue extends beyond the borders of the original wound and does not usually regress spontaneously. It causes cosmetic deformity, itching, pain, and stress ultimately resulting in compromised quality of life and diminished functional performance. The treatment of Keloid is often difficult because there is an associated high risk of recurrence. Vrana represents a vast part of Shalyatantra and the pathogenesis of Keloid is similar to Vrana granthi which has been explained as a growth of mass at the site of Vrana following dietetic complications or any physical irritation in a Vrana patient. Jalaukavacharana (leech therapy) is a parasurgical type of Raktamokshana and is considered as the most unique and effective method of bloodletting. Leeches are widely used in most disorders ranging from skin disease with evidence of successful results. This condition can be caused by a variety of factors, including sun exposure, hormonal changes, inflammation, and skin injuries. Hyperpigmentation can have a significant impact on a person's self-esteem and quality of life. Hormonal changes, such as those experienced during pregnancy or menopause, can also lead to hyperpigmentation. Inflammation from conditions like acne or eczema can cause the skin to produce excess melanin, resulting in dark spots or patches. Additionally, skin injuries, such as cuts or burns, can trigger the body to produce more melanin as part of the healing process, leading to hyperpigmentation. Creams containing ingredients like hydroquinone, retinol, or vitamin C can help lighten dark spots and even out skin tone. Medications containing corticosteroids, may be recommended for more severe cases of hyperpigmentation. In addition, novel drug delivery systems such as liposomes, Transferosomes, Niosomes etc. have shown better penetration topically hence they can be used to treat pigmentation. Understanding the causes of hyperpigmentation and seeking appropriate treatment can help improve the overall health and appearance of the skin. The World Health Organization (WHO) defines child maltreatment as “all forms of physical and emotional ill-treatment, sexual abuse, neglect, and exploitation that results in actual or potential harm to the child’s health, development or dignity.” Abuse of children can have serious short and long term effects on physical, sexual, and mental health. These include physical harm, such as concussions and severe disabilities, especially in young children, as well as anxiety, sadness, and post-traumatic stress disorder. Child abuse has a strong correlation with alcohol and drug usage and can impact cognitive and academic performance.Priority must be for protecting and preserving the safety of abused children. The goals of ongoing treatment are to lessen the long-term psychological and physical effects of abuse as well as preventing reoccurrence.\u0000Child maltreatment is a global concern with profound implications for the physical, emotional, and psychological well-being of children. While conventional approaches in the field of child protection have made significant strides, there is growing interest in exploring complementary practices, such as Ayurveda, to enhance the overall well-being of maltreated children. The potential role of Ayurveda in addressing child maltreatment, considering its ancient roots in promoting holistic health and balance is to be highlighted.\u0000Ayurveda also proposes the principles for taking care of childhood and preventing maltreatment for them. Therapies, including herbal remedies, massage, and meditation is having the potential in managing and rehabilitating the children affected by maltreatment. Mutrakruccha translates to difficulty in micturition. It can be associated with urethral stricture due to the similarity of symptoms. Urethral stricture disease is often seen in patients with recurrent urinary tract infections, urethral injuries, or post-surgical procedures conducted through the urethra. The most common causes of urethral stricture today are traumatic or iatrogenic. Inflammatory or infectious, malignant, and congenital aetiologies are less common. There is a low success rate of Western medical science treatment for urethral stricture disease. Surgical treatment of urethral stricture disease is indicated when the patient has severe voiding symptoms, bladder calculi, increased postvoid residual, urinary tract infection, or when conservative management fails. Urinary tract infections (UTIs) should be adequately treated before surgical intervention. In this case study, a 39-year-old male patient complained of suffering from symptoms like the strangury of urine, pain during micturition, the flow of urine is very poor, frequent micturition and occasionally burning micturition. After clinical examination and radiological testing, the patient was identified as having urethral stricture and treated with Jatyadi Taila Uttarbasti for 21 days at Shalyatantra OPD, Government Akhandanand Ayurved College and Hospital, Ahmedabad. After completion of treatment, the patient felt 75% relief in symptoms.
