These conditions can provide obstacles to traditional forms of sex therapy, especially when the therapist asks the couple to perform homework exercises. Knowledge of applicable Jewish law is essential for therapists working with this population. A couple was referred for the wife's hypoactive sexual desire and dyspareunia. Her husband developed a premature ejaculation problem in response to his wife's reduced libido. Successful treatment required the therapist to establish credibility regarding sexual matters and create comfort in the couple with his acceptance of Jewish law regulating their sexual behavior. Strategic prescriptions for nonsexual intimacy behaviors were given along with sensate focus exercises, resulting in rapid improvement of the presenting problems.","PeriodicalId":83768,"journal":{"name":"Journal of sex education and therapy","volume":"26 1","pages":"22 - 27"},"PeriodicalIF":0.0,"publicationDate":"2001-03-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.1080/01614576.2001.11074378","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"59228831","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Some of these behaviors may be illegal, some addictive, some obsessive and/or compulsive, or a combination of all of these. Certainly many of these behavioral constellations interfere with an emotionally or sexually intimate relationship and are often the basis for a referral for relationship and/or sex therapy. One couple, married 23 years, presented for treatment regarding the wife's longstanding hyposexuality. Assessment of this couple revealed that the husband's hypersexuality and nonparaphilic attachment played a significant role in his wife's disinterest in sex and served as a defense against developing an intimate relationship with his wife. The couple did not continue in treatment. Another couple, two gay men involved in a monogamous relationship of 8 years, also entered treatment with the presenting complaint of hyposexuality. With this couple, however, the hyposexuality seemed to reflect a maturational or developmental issue rather than a paraphilia- related disorder. Treatment success was enhanced by the couple's commitment to the relationship and their ability to communicate intimately with one another.","PeriodicalId":83768,"journal":{"name":"Journal of sex education and therapy","volume":"26 1","pages":"50 - 58"},"PeriodicalIF":0.0,"publicationDate":"2001-03-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.1080/01614576.2001.11074382","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"59228468","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

As our discussion continued, and the list of possible topics grew, we eventually decided that having readers submit cases that either had been challenging to them, different from the more typical cases coming through the door, or represented a new way of looking at and doing therapy than what is typically the norm for a particular disorder, would be most interesting. So often we read about how to conceptualize a problem, or how to apply a new technique or approach-more seldom do we read about how this actually works in the therapy process. Additionally, there seems to be less and less time available these days to discuss cases with colleagues. The changes in the mental health field due to managed care have led many clinicians to increase their caseloads and see more patients. Consequently, there is less time for the busy therapist to discuss cases with colleagues. Yet we think that most clinicians are still interested in hearing how others work with patients and how they handle certain challenges and issues. We thought that focusing this special issue on case studies was one way this could be accomplished. The case studies selected for this special issue are comprehensive and, as most clinical cases are, complex. The authors have done an admirable job making the material come to life in a way that enables the reader to understand the patient and the therapist. These cases are typical of most clinical caseloads; previous treatment failures, family-of-origin issues, concurrent psychological distress or psychopathology, or physical symptomatology. In many of these cases success was achieved by asking certain questions not previously asked, through willingness on the therapist's part to be creative in approach (e.g., acquiring knowledge of the culture and religious beliefs of the patient), and through close collaboration with other health care professionals. It is my hope you will gain, as I did, some helpful ideas or insights that might benefit you in your work with patients. Let me give you an overview of this special issue. There are three papers that focus on vaginismus (we received several abstracts on this topic). It is clear that therapists feel quite challenged by this particular dis-","PeriodicalId":83768,"journal":{"name":"Journal of sex education and therapy","volume":"26 1","pages":"5 - 6"},"PeriodicalIF":0.0,"publicationDate":"2001-03-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.1080/01614576.2001.11074375","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"59228685","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

A couple 21 years apart in age presented with severe PE (acquired, situational). The husband was a healthy, 60-year-old man who was afraid he could not “keep up,” sexually and otherwise, with his 39-year-old, highly energetic, sexually voracious wife. He feared she would leave him. The integration of PE exercises and psychodynamic couples therapy alleviated the PE and improved the overall relationship in 24 sessions. This success, however, was contingent upon both partners understanding and resolving their individual conflicts regarding power and control. These family-of-origin-based conflicts were found to be responsible for the couple's decision to mate, their general relationship strife, as well as the husband's anxiety and hence, his PE. Age difference played a vital role in that it served as the primary vehicle for the underlying conflicts.","PeriodicalId":83768,"journal":{"name":"Journal of sex education and therapy","volume":"26 1","pages":"34 - 44"},"PeriodicalIF":0.0,"publicationDate":"2001-03-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.1080/01614576.2001.11074380","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"59228410","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The psychiatrist was treating Steve for depression, anxiety, and intrusive violent obsessive thoughts with psychoactive medications, including paroxetine (Paxil) and buspirone (BuSpar). The patient described erectile dysfunction (ED) and retarded ejaculation (RE). The couple's child (age 2) was conceived through artificial insemination with semen provided by the patient via masturbation. At the time of referral, Steve's spouse Susan (23 years old) was taking fertility drugs and artificial insemination was planned again. The couple was treated previously by a sex therapist, improving Susan's orgasmic capacity, but the marriage remained unconsummated. Unbeknownst to the previous therapist, the patient was secretly masturbating in an idiosyncratic and compulsive manner—a frequent cause of RE. Alteration of Steve's masturbation style and frequency was a necessary, but insufficient, intervention of this current treatment. Steve began obtaining erections and achieved penetration with his spouse, but remained coitally anorgasmic. Alternative formats (individual/conjoint) were required to strategically manage the treatment process. Successful therapy required maintaining rapport and congruence with the referring psychiatrist who continued pharmacotherapy using different psychoactive agents. Management of fertility issues, artificial insemination, pregnancy, the birth of a second child, and Jewish ritual law further complicated the therapy. Additionally, sildenafil (Viagra) was used to augment the cognitive-behavioral sex therapy, resulting in a more robust and powerful treatment. Eventually the couple consummated their marriage, and Steve was able to ejaculate intravaginally on a regular basis.","PeriodicalId":83768,"journal":{"name":"Journal of sex education and therapy","volume":"26 1","pages":"13 - 21"},"PeriodicalIF":0.0,"publicationDate":"2001-03-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.1080/01614576.2001.11074377","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"59228792","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

It is customary to treat the patient and her spouse or partner together, using cognitive-behavioral therapy and employing exercises with dilators. In this case study, a 33-year-old single female with only moderate English language ability and limited financial resources sought treatment for lifelong vaginismus. Several potential marriage partners had left her because every attempt at intercourse resulted in extreme pain for her. Desperate about the possibility of never being able to marry or have children, she had attempted suicide. Treatment entailed constant use of hand gestures and drawings in conjunction with conversation to ensure that there would be no misunderstandings resulting from language barriers. Over a period of 18 months, the patient was gradually assigned exercises designed to help her overcome inhibitions and exercises using a variety of vegetables which substituted for more expensive dilators. The treatment was highly successful, culminating in her ability to have painless, pleasurable sexual intercourse with a partner. The case demonstrates the significance that vaginismus may have in the life of a woman who does not have a partner, and the role of innovation in treating cases that may not fit a typical profile.","PeriodicalId":83768,"journal":{"name":"Journal of sex education and therapy","volume":"26 1","pages":"28 - 33"},"PeriodicalIF":0.0,"publicationDate":"2001-03-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.1080/01614576.2001.11074379","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"59228869","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The seductive anonymity encourages rapid and easy intimacy. The experience of communicating with a mysterious “other” allows for the development of transference similar to traditional psychotherapy.
