McCullough, PhD, is Associate Professor of Psychology at Southern Methodist University. David Larson, MD, is Adjunct Professor of Psychiatry at Duke University Medical Center and at Northwestern University Medical School. In this exhaustive 712-page handbook, the authors set out to review and discuss the research on the relationship between religion and health conducted between 1900 and 2000. They touch on such areas as depression, anxiety, heart disease, and immune system dysfunction, as well as health-destructive behaviors such as smoking, substance abuse, and risky sexual behavior. They examine both the positive and negative effects on health throughout the lifespan. For. instance, they report on a study done with 10,059 men in Israel, and published only in Hebrew, that \"one of the factors which stood out was the inverse relationship between the degree of religiosity and the 3-year incidence of myocardial infarction-in other words, the more religious developed fewer infarcts\" (p. 346). This is just one of the 1,200 research articles reviewed. This is a dip-and-dig book. After hearing about it from Dr. Harold Lief, I spent a week just leafing through to savor the gems. One day I read about depression, another suicide, another delinquency, then heart disease, disability, pain, and marital instability. And so it went. I spent a weekend on research methods and measurement tools and another weekend on priorities for future research. Yum. Without proper care and rehabilitation, it can be one of the most overwhelming and disruptive surgeries performed. It is estimated that about 450,000 Americans of all ages, genders, and sexual orientations have an ileostomy, colostomy, or urinary diversion and that approximately 120,000 new surgeries are performed each year. After ostomy surgery, both the loss of control over elimination of stool and/or urine and the need to wear an external collection device, called an ostomy pouching system, create feelings of depression, low self-esteem, and fear of rejection by current or potential sexual partners. In some cases, these concerns become so extreme that individuals withdraw entirely from their family or other types of socialization. As a consequence of removing the bladder or the rectum, many males experience erectile dysfunction (ED) or retrograde ejaculation. Many women are concerned about body image, pregnancy, or being sexually desirable. Because it is estimated that the average age of individuals with stomas is about 68, education about sexuality and sexual functioning after ostomy surgery is often overlooked. Due to reduced postoperative hospitalization stays, new patients are often discharged with little or no instruction or counseling. However, effective pain management is often complicated by the multiple determinants of the chronic pain experience. For a minority of survivors, nociceptive or neuropathic pain may develop or persist following a pain-free interval of variable duration despite apparent recovery of damaged tissue and continued disease-free status. Additionally, survivors may also suffer from other chronic pain conditions that may have been acquired before or after the cancer diagnosis and treatment. The experience of chronic pain often has deleterious effects on sexual functioning, related either to the noxious stimulus of pain or as a negative consequence of pain management. Common pharmacological approaches and interventional techniques utilized in the treatment of pain have documented effects on sexual functioning. Current treatment strategies offer varying degrees of symptom resolution. Future research is needed to examine the prevalence, characteristics, and impact of chronic pain and medications for pain management on sexual function. Women are usually diagnosed at early stages and treated locally, allowing for the maintenance of optimal quality in most areas of life. Because the disease targets organs associated with reproduction, however, sexuality and fertility are not spared. Even though the cancer is eliminated, its psychological impact intrudes into a woman's idea of what it means to be feminine, sexual, and generatively powerful in the world. Sadly, the women who are most likely to survive cancer at a youthful age are often physically healthy but plagued by sexual side effects for the remainder of their lives. Dyspareunia (painful intercourse) is one of the most common sexual symptoms in cervical cancer survivors. Dyspareunia results from damage to vaginal tissue and reshaping of the vagina secondary to surgery and radiation. When ovarian function is curtailed by treatment, hormonal dysregulation associated with menopause also leads to dyspareunia, if untreated. Like the cancer, dyspareunia carries an impact beyond that which is physically palpable. The heightened awareness of the vagina, which may never have emerged notwithstanding dyspareunia, often precipitates an unexpected exploration of the deeply personal meaning a woman attaches to her sexual self. These sexual self-images vary depending on the attributions placed on having withstood unanticipated, albeit lifesaving, alteration to genitalia. Intimate relationships may also be profoundly affected. As in any non-randomized survey such as this one, the reader must not be lured into the comfort zone of being able to generalize women's sexuality based on the Ellison-Zilbergeld findings. The women in Women's Sexualities are far from \"average\": a high percentage had attended some college, including 40% who had done some graduate work or had a graduate degree (unlike most American women), and 83% were Caucasian. As Ellison herself disclaims: \"Please keep in mind that our statistics and other research findings describe the particular group of women we survey, not American women in general\" (p. 7). The narrative of the book encompasses women's stories, information from the research studies, and Dr Ellison's comments and advice. Ellison puts it this way: \"Women's Sexualities is a guide to self-understanding and self-acceptance ... a source of information that will deepen your understanding of pleasurable lovemaking and enhance your love making skills\" (p. 2). Ellison also states that this book is \"a facilitator for sexual decision making\" and \"a stimulus for discussion\" (p. 2). A unique feature is the section at the end of each chapter titled, \"What Does This Mean to You?\" Here the reader is encouraged to respond to a list of questions relevant to that chapter. Ellison suggests that the readers record their thoughts and feelings in a separate notebook or binder as they proceed through the book. Overall, two themes emerge. One is self-acceptance as a key to accepting our sexual selves. The other is the benefit of learning from the intergenerational dialogue that takes place throughout the text. Ellison observes, \"Patterns emerge and by paying attention to those patterns you can learn from these women's collective experience how to experience your own sexuality and accept your sexual self more completely\" (p. 5). One of the most interesting chapters is \"Sexual Concerns and Problems of American Women,\" particularly the section, \"Problems Related to Physical Responsiveness\" (pp. 266-270). Here, Ellison offers an original twist to the often-cited \"lack of sexual desire\" scenario. Although she did find that desire I frequency was an important sexual concern, physical responsiveness and lovemaking also ranked high. She breaks these problems into two categories: three concerns that had to do with lack of desire in a woman's partner, and three with her own lack of desire. She then provides practical remedies for each of these concerns. In another section, she discusses an aspect of \"sexual concerns and problems\" that usually gets little ink, but one that is high on the list of sexual problems of \"women who weren't born yesterday\" : lack of a partner with whom to be sexual (p. 276). This topic relates nicely to another especially interesting chapter: \"Solo Eroticism: Masturbation and Fantasy,\" with an indepth discussion of why women masturbate. Healthy sexual development is based on the delicate interplay of physical, psychological, and interpersonal factors. Young patients with any chronic illness, including cancer, must tackle normative developmental issues as well as disease- and treatment-related challenges. Because development occurs as a succession of inextricably related stages, one building on another, disruption at an earlier stage may have a profound impact on subsequent development. Pediatric cancer and its treatment have the potential to subtly but dramatically disrupt the development of healthy adult sexuality by compromising one or more of these developmental building blocks. A small but convincing literature is developing that, while supporting the overall well-being of cancer survivors (e.g., no evidence of psychiatric symptomatology), suggests that sexual well-being is compromised by a childhood cancer diagnosis and treatment. Most notably, a number of studies have highlighted the devastating effects that treatment for cancer during childhood can have on sexual self-concept and body image in generally well-adjusted survivors. Because human sexuality is so complex, a full range of clinical interventions can be used to address both the current and potential psychosexual disruptions that young adult survivors experience. The developmental context of sexuality must be considered when planning interventions. In addition, the private nature of the topic, as well as the nature of the disease and treatment course, can impede the effective implementation of the interventions.
