The etiology of NHL is unknown, though for some of them there has been a clear association with certain infectious agents and immunodeficiency states. For accurate diagnosis, it is critical that adequate tissue biopsy be obtained. Classic histology is routinely supplemented with immunophenotyping, cytogenetics, and molecular diagnostics. After establishing the tissue diagnosis of NHL and a subtype, additional tests are done to evaluate the extent of the disease (staging procedures). For treatment purposes, NHL are usually divided into three groups: indolent lymphomas, aggressive lymphomas, and very aggressive lymphomas. The most common indolent lymphoma is follicular cell lymphoma. Selected patients with limited stage disease may be cured with radiation only, while most who present at advanced stage are considered incurable with standard treatments. The most common aggressive lymphoma is diffuse large cell. Patients with limited stage disease may be cured with a limited number of cycles of chemotherapy followed by involved-field radiation. Advanced stage disease has been treated with CHOP chemotherapy but the recent addition of the monoclonal antibody rituximab to CHOP has resulted in improved response rate and survival.
