Criterion validity refers to the experimental demonstration that the method compares with a well-accepted standard (“gold standard”). Structural validity refers to internal consistency of the method. DATA SOURCES Electronic databases including PubMed (1948 to July 2011), OVID EMBASE (1980 to July 2011), and CINAHL (EBSCOhost) (1982 to July 2011) were searched. STUDY SELECTION Two authors independently screened all records for eligibility. For inclusion, the study population had to include patients with vitiligo, for which outcome measures were developed or evaluated on their measurement properties. The initial search retrieved 1249 records, of which 14 articles met the inclusion criteria. DATA EXTRACTION Characteristics of the included instruments, study population, and results of the measurement properties were extracted. The Consensus-Based Standards for the Selection of Health Status Measurement Instruments (COSMIN) 4-point checklist, combined with quality criteria for measurement properties, was used to calculate the overall level of evidence per measurement property of each instrument. Independent extraction and assessment was performed by 2 authors. DATA SYNTHESIS Eleven different measurement instruments were identified. Strong evidence was found for a positive internal consistency of the Dermatology Life Quality Index. For other instruments, the evidence of measurement properties was limited or unknown. CONCLUSIONS Recommendations on the use of specific outcome measures for vitiligo should be formulated with caution because current evidence is insufficient owing to a low number of studies with poor methodological quality and unclear clinical relevance. Her quality of life was significantly affected: she reported pain while urinating, was unable to wear certain clothing, and experienced dyspareunia. Treatment for thrush using over-the-counter medications had resulted in worsening symptoms. Examination showed tender erythematous erosions with a white border, located at the vaginal introitus. These lesions were tender. Inspection of the oral cavity revealed bilateral lacy white plaques on the buccal mucosa. The oral lesions were asymptomatic. Biopsy from the edge of a vulval erosion was in keeping with the clinical diagnosis of erosive lichen planus. The patient was counseled about the possible risk of squamous cell carcinoma (SCC) of the vulva, which is associated with the condition. She asked about the magnitude of that risk, but we were unable to give an exact figure. This prompted our review of the literature. Erosive lichen planus is an uncommon, chronic inflammatory condition mainly affecting the mucosal surfaces of the mouth and genitalia. It is thought to be an autoimmune disorder with T cell–mediated damage to basal keratinocytes. Vulval erosive lichen planus (ELPV) causes painful erosions and scarring of the vulva and vagina. Some researchers have postulated on the basis of case reports that ELPV lesions may develop into SCC. However, ELPV is underresearched and evidence from larger-scale studies is lacking. {"title":"Rapid healing of scar-associated chronic wounds after ablative fractional resurfacing.","authors":"Peter R Shumaker, Julia M Kwan, Evangelos V Badiavas, Jill Waibel, Stephen Davis, Nathan S Uebelhoer","doi":"10.1001/2013.jamadermatol.256","DOIUrl":"https://doi.org/10.1001/2013.jamadermatol.256","url":null,"abstract":"<p><strong>Background: </strong>Skin compromised by traumatic scars and contractures can manifest decreased resistance to shearing and other forces, while increased tension and skin fragility contribute to chronic erosions and ulcerations. Chronic wounds possess inflammatory mediator profiles and other characteristics, such as the presence of biofilms, that can inhibit healing.</p><p><strong>Observations: </strong>Three patients with multiple traumatic scars related to blast injuries initiated a course of ablative fractional laser therapy for potential mitigation of contractures, poor pliability, and textural irregularity. Patients also had chronic focal erosions or ulcerations despite professional wound care. All patients experienced incidental rapid healing of their chronic wounds within 2 weeks of their initial ablative fractional laser treatment. Healing was sustained throughout the treatment course and beyond and was associated with gradual enhancements in scar pliability, texture, durability, and range of motion.</p><p><strong>Conclusions: </strong>The unique pattern of injury associated with ablative fractional laser treatment may have various potential wound-healing advantages. These advantages include the novel concept of photomicrodebridement, including biofilm disruption and the stimulation of de novo growth factor secretion and collagen remodeling.
