Sodium hydroxide dissolves the keratohyalin granules and the inter fibrillar material in the keratin. It has no effect on the epidermal and keratin fibers and on the marginal dense band of the horny cells. It \"produces\" a keratin pattern in the psoriatic horny layer. After sodium thioglycolate treatment the keratohyalin disappears, the epidermal and keratin fibers show a periodicity and the cementing material in the horny cells is more opaque. After lipid extraction the structure of keratohyalin becomes inhomogenous. The effect is somewhat similar to that of sodium thioglycolate but there is no periodicity in the horny layer. These genetically determined basic alterations may represent an important factor for the manifestation of psoriasis. They consist of a decreased penetration of DHEA through the cell membrane and an increased reduction of DHEA to ADIOL, leading to a reduced DHEA/ADIOL ratio. In the present investigations the question was whether the assay of these parameters may aid in the detection of latent psoriasis. For this purpose 31 members form 6 unselected families of psoriatics (8 manifested psoriatics, 18 potential psoriatics, 5 clinically and anamnestically healthy family members by marriage) as well as 6 further controls were examined. It could be shown that those changes in the DHEA metabolism, which are evident in manifested psoriatics also occur at least in part of potential psoriatics. However, both parameters investigated do not always behave the same way. Hence, from the present data a satisfactory diagnosis of latent psoriasis does not seem possible. Still, the estimation of the above parameters may aid in the attempts to define latent psoriasis. The biopsies were taken from: 1. clinically normal skin from nonpredilection areas, 2. intact skin from predilection areas, 3. involved skin and 4. experimentally frictioned skin. The main ultrastructural alterations detected are as follows: hypoplasia of anchoring fibrils, split or blister formation between basal lamina and dermis, hernia-like protrusion of basal cells, sub- and intraepidermal deposition of fibrillar bodies, and duplications of basal lamina. Among them, the constantly observed finding in all of the four biopsy groups in the structural defect of anchoring fibrils, namely, that the anchoring fibrils are rudimentary and reduced in number. The intensity of reaction was high with dehydroepiandrosterone, estradiol, androsterone and pregnenolone, and none with 11-beta-hydroxyprogesterone, testosterone, dihydrotestosterone, 17-alpha-hydroxypregnenolone and 17-alpha-hydroxyprogesterone. Reactivity also varied with the hair cycle. Concentrated sulfuric acid was applied to the skin for a period of 30 sec and 5 min. After this the skin was washed with tap water for 1 hr. The treated area was then subjected to macroscopic and microscopic examination. When the acid was applied for 30 sec, followed by rinsing, an almost complete restitution of the skin was found on the test area macroscopically and microscopically. At the end of treatment the mechanical properties of dorsal skin and aorta of the experimental animals are determined. 4 weeks of treatment with D-penicillamine or paramethasone, respectively, leads to a decrease in the skin thickness of the experimental animals. With simultaneous administration of both substances the decrease in skin thickness is about the same. Load resulting in rupture is considerably reduced by D-penicillamine, while only a slight decrease is seen with paramethasone. Simultaneous administration of both substances results in considerable decrease in load resulting in rupture, similar to the effect produced by D-penicillamine. Tensile strength which is not influenced by paramethasone treatment, decreases after treatment with D-penicillamine as well as after simultaneous administration of D-penicillamin and paramethasone. Load resulting in rupture of the aorta remains uninfluenced under the experimental conditions. Comparison of the groups shows that simultaneous administration of D-penicillamine and paramethasone leads to the same results as those which may be obtained with sole administration of D-penicillamine. Separation of the epithelium from the connective tissue by treatment with trypsin allows the demonstration of the three-dimensional structure of the primary and secondary connective tissue papillae and of the papillary epithelium. The distal surface of each primary connective tissue papilla is studded with secondary connective tissue papillae. Their lamellated surface structure corresponds with a complex system of cytoplasmic processes of the basal epithelial cells. The complicated surface architecture of the epithelial-connective tissue junction may suggest a close structural and functional relationship between the two types of tissues. There are two distinct cell populations forming the epithelial papilla, which differ in ultrastructural morphology from the basal cell layer to the surface of the epithelium. A dual pattern of keratinization, phylogenetically analogous to the hard and soft keratinization in rodent tongues, is found also in filiform papillai of the human tongue. The final product of epithelial differentiation is a type of keratin different from that in epidermal keratinocytes and in other regions of the oral cavity. The variability in size, shape and association with other organelles suggests the heterogenous nature of lingual keratohyaline granules. At high concentrations of FAME (500 mug/ml culture medium) the inhibition of nucleic acid synthesis is 6 times higher on the average than the inhibition of protein synthesis. However, the application of the cis-isomer, maleic acid monoethylester (MAME), results in an increase of the incorporation rate of the labelled precursors into the RNA and DNA. This is 3.5--9.3 times higher than after application of FAME. The results demonstrate the specific inhibition by FAME. The rate of labelling of nucleic acids is decreased above 10 mug FAME/ml culture medium and in the case of MAME above 50 mug/ml medium. Tests were performed during resting and proliferation phases. Changes of cell metabolism were measured by O2-consumption by the Warburg technique. With increasing corticosteroid concentrations (10-10000 gammaml) a dose dependent increase of cell respiration was found with and without addition of nutritive substrates. Even high concentrations of corticosteroid did not cause an antimetabolic effect.
