It could be demonstrated that band 9 was rich of glycine, alanine, aspartic acid, leucine, and serine and that band 10 was rich of glycine, threonine, and serine. {"title":"[Ultrastructure of the epidermis in seborrhoic dermatitis (author's transl)].","authors":"J Metz,&nbsp;G Metz","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Electronmicroscopical investigation on 12 cutaneous biopsies of different localisation showed that the seborrhoic dermatitis is not comparable to a psoriatic tissue-reaction. Both, the cyto-morphological feature and the demonstrated localisation of the acid phosphatase are distinctly different from that of the psoriatic epidermis. The described changes are clearly more similar to the well known ultrastructural pictures of eczemateous reactions. In spite of the \"eczema-like\" ultrastructural picture seborrhoic dermatitis apparently can be separated from the allergic and irritant contact-dermatitis. Nevertheless, the epidermal alterations are also similar to chronic nummular eczema. Using vaseline as vehicle 2-10(-4) molar concentrations are found in the epidermis and 3-10(-5) molar concentrations in the dermis. Further, three vehicles revealed minor concentrations. Comparison of the results with similar in vitro-investigations, reported earlier, allow to judge the resorption rate by the intact capillary system. Uptake of hydrocortisone by the vessels is observed. With high rates of penetration vasoconstriction seems to limit the resorption, while this is not the case with low rates, for instance with polyethyleneglycol as vehicle. Thus a differing drug liberation from the vehicle causes qualitatively different permeation kinetics. The elimination by urine, however, was practically the same. No increase of maligonomies has been observed except for the tumors induced by arsenic. The question may be asked whether the psoriatic cell has additional or more effective mechanisms to eliminate onocogenic somatic mutations. In this study the capacity of the excision repair of circulating blood lymphocytes from 12 psoriatic patients has been investigated and compared to a control group of equal size. After irradiation with ultraviolet light, the lymphocytes are incubated with 3 H-thymidine and at various intervals the radioactivity in the cell material precipitated with perchloric acid is measured. It appears, that the excision reapir in lymphocytes from patients with psoriasis and normal persons is equal. In the same way experimental suction blisters of the peri-lesional skin were investigated. Therefore lipid measurements are possible in areas with only few sebaceous glands and even on scalp hairs. The hair lipids are continuously measureable without disturbance by sebum from the skin of the head and without extracting the hair of the test person; the time needed for measurement is only short. The curve for the refatting of the hair can be quantitatively recorded. Comparison with the 3 times daily application of the cream containing fluprednylidene and the 3 times daily application of a cream containing betamethasone 17-valerate did not show any differences in the result of treatment which was good to very good in each case within a 15-day trial period. No side effects were determined. In all 53 patients the peripheral blood lymphocyte count showed no substantial difference from the control values. The blasts count after phytohaemagglutinin stimulation in 30 patients ranged from 40-50% with no noticeable difference from the control values. Intradermal tests with the staphylococcal antigen, PPD-tuberculin and streptokinase-streptodornase carried out in 59 patients proved negative in a significant number of cases, mainly those of Kaposi's sarcoma. The non immune inflammatory response to chemical irritant (benzalkonium chloride) was more often negative (17 of 59 cases) than in controls (5 of 30 subjects). The DNCB sensitization test carried out in 28 patients was negative in 5 cases (2 cases of late basal-cell epithelioma, 2 cases of advanced squamous-cell carcinoma and 1 case of Kaposi's sarcoma). The findings do not thus indicate the presence of a gross CMI defect in the material studied, which would be borne out by all the tests employed. The sometimes discordant results of the various tests suggest the existence of partial CMI defects in certain tumours, but mainly in Kaposi's sarcoma. In nearly one third of the basal cell epitheliomas newly formed elastic fibers could be observed in the stroma. In places where a new formation of elastic fibers was demonstrated fibroblasts and mast cells (toluidine blue stain) were unusually numerous. Apparently the influence of the growing tumor masses of basal cell epitheliomas leads to a dermo-epidermal interaction in the form of 1. increase of mast cells 2. stimulation of fibroblasts 3. new formation of elastic fibers. The observation suggest that there exists a relationship between the above noted findings. Due to the dermo-epithelial interaction (even in senile skin) in the stroma of basal cell epithelioma fibroblasts can be stimulated to such a degree that they regain the ability to produce elastic fibers-a function which usually is greatly diminished with increasing age.
