{"title":"[Effects of surgery on plasma amino acids levels].","authors":"G François, C Bouffier, J L Blache, M Blanc","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>This study was aimed at comparing pre- and immediately postoperative plasma amino acids levels in ten patients undergoing moderately important surgery and anaesthetised using the same protocol. Amino acid levels were measured using and automatic analyser equipped with an ion exchanger column. Changes in total blood amino acid levels showed wide variations. It is difficult to assess the role played by possible changes in blood volume, and for this reason results are expressed as a percentage of total amino acid levels. Under such conditions, findings were as follows: - taurine and alamine rose significantly; - there was a significant fall in serine, citrulline, alpha- amino-butyric acid and isoleucine. The results of this study differed markedly from findings reported in the literature: the authors noted a fall in plasma amino acid levels both in relation to preoperative levels as well as in relation to \"normal\" levels in the literature. This could be related to haemodilution, this hypothesis being supported by the fall in total aminogram in eight of the ten patients of the group. The most notable fact in the variations seen in this study was the rise in plasma alamine levels which could be indicative of a fall in gluconeogenesis. {"title":"[Postoperative gas gangrene. In the series studied mortality was 40.9 p. 100, independent of age and sex. Rapidly progressive forms were the most severe. The delay before effective treatment was prescribed influenced prognosis. In clinical terms, shock and associated renal insufficiency were grave, as well as a picture of respiratory distress which led, in certain cases, to contra-indication of one of the therapeutic possibilities, i.e. that of hyperbaric oxygen. Responsible organisms could be isolated in nineteen cases from local samples. There was a marked predominance (15 cases) of clostridium perfringens. Contamination with aerobic flora was common. Examination to assess favourizing circumstances led essentially to a conclusion of the role of microbial contamination, ischemia, broad spectrum antibiotics, absence of appropriate antibiotics and underlying immuno-depression. Treatment was based in the majority of cases on the triple combination of antibiotics, surgery and hyperbaric oxygen, as well as the correction of any general systemic disorders. Mortality was markedly reduced (31 p. 100) in patients receiving complete and early treatment. The gravity and recrudescence of disorders due to anaerobic organisms lead the authors to review current therapeutic possibilities. Appropriate treatment should be prescribed in all situations where an infection due to anaerobic organisms is feared, and should cover the risk of clostridial infection (penicillin 200,000 mu/kg/24 h) as well as the risk of bacteroides (metronidazole 25 mg/kg/24 h). {"title":"[Enteral nutrition by tube in burn patients].","authors":"J C Manelli, D Palayret, M Garabedian, P Kiegel, M C Blanc","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The authors report a year's experience off artificial feeding (A.F.) in the Regional Burns Centre of Marseille. 28 patients have benefited from an A.F. of longer than 10 days. It consisted of four cases of parenteral feeding, six cases of enteric feeding and eighteen cases of combined parenteral and enteric feeding. The efficiency of the techniques of A.F. is demonstrated by the results: a weight loss greater than 10% was observed in only three patients. The two groups, differed only in terms of the E.C.C. bath (Group A: Ringer Lactate; Group B: DDextran 60,000). The effects of E.C.C. on these parameters were as follows: - decrease in both groups in the gradient (OP-PAP) (respectively P < 0.001 and P < 0.01) but with a more marked decrease in group A than in group B (P < 0.05) with non-negativisation of the gradient in that group; - non-significant variations in Qs/Qt in both groups without any correlation with gradient (OP-PAP).
