Therefore, “Speedy Biochemistry” was introduced to assist students to apply knowledge they have learnt in the classroom. This setting promotes teamwork and application of knowledge in problem solving which can be achieved through active learning. Active learning has piqued the interest of the academic community in giving more benefits to the students. It can be due to lacrimal drainage malfunction of any kind. The aim of this study was to evaluate the effects of epiphora with good passage on lacrimal irrigation on a modified Ocular Surface Disease Index (OSDI) questionnaire. A total of 101 patients (53 males/ 48 females) were included in the study. The modified OSDI questionnaire had 11 items concerning ocular symptoms that affects daily activities. The relationships between the questionnaire score for each question and the clinical features included age, gender and previous cataract surgery. The most affected activities according to modified OSDI were watching television, windy conditions and cold weather (average means of 3.36 + 0.82, 3.42 +±0.86 and 3.40 +±0.88, respectively). Epiphora was found to have a statistically significant effect on daily life, all except with regard to interpersonal relations (p=0.493) with increasing age. Women were found to be experiencing more eye grittiness (p=0.0001), getting more affected during reading (p=0.027), at hot/dry climates (p=0.0001) and believed epiphora has an important impact on interpersonal relations and general happiness (p<0.0001). Pseudophakic patients also showed statistically significant increase of discomfort in daily life questions in 7 of the 11 questions (p<0.0001). This study showed the subjective epiphora related symptoms impact on a modified OSDI. This questionnaire was easy to manage and quick to complete while showing similar results to those that were specifically designed for epiphora with nasolacrimal obstruction. {"title":"A Casemix: Congestive Heart Failure Readmission Rate and its Associated Factors in a Tertiary Teaching Hospital in Kuala Lumpur","authors":"A. Around 20% of heart failure patients are readmitted within 30 days worldwide. Readmission within 30 days for congestive heart failure often related to non-compliant to fluid restriction, natural progression of the disease or pre-mature discharge. The study aims to evaluate the 30 days readmission rate of congestive heart failure in Universiti Kebangsaan Malaysia Medical Centre (UKMMC) from the year 2016-2017 using casemix data and to determine the associated factors related to it. Based on the data coding, cost of readmission incurred was calculated. The 30-days readmission rate of heart failure to UKMMC in the year 2016-2017 was 53.5% & 43.5%, higher than the developed countries standard but similar to that of local teaching hospitals. After adjustment, age and level of severity are the only significant associated factors for the 30-days readmission rate. Shorter average length of stay (ALOS) & lower extra-tariff were achieved for readmission of heart failure. Total of RM80,329.73 was saved in the year 2017 from prompt treatment and comprehensive care plan of treating 19.3% level 3 severity patients at level 1 ALOS and tariff-cost. The 30-days readmission rate for heart failure in UKMMC was comparable to local teaching hospitals. The reduction of 30-days readmission rate in 2017 saved much cost, shows good clinical governance and advanced treatment strategy. The management of S. epidermidis infections become more challenging with the increase of antimicrobial resistance cases over the past years. Limited reports on S. epidermidis antibiotic resistance among healthy people leave uncertainty about the magnitude of antimicrobial resistance spreads among the community. Therefore, this study aimed to identify the antibiotic susceptibility pattern of S. epidermidis isolated from healthy undergraduate students in one of Malaysia public universitieshealth campuses. Ninety-six hand palm swab samples were collected and undergo several tests, including microscopic, biochemical identification tests and antibiotic susceptibility test for erythromycin, oxacillin, gentamicin, penicillin and tetracycline following Kirby-Bauer test. A total of 43 samples showed the presence of S. epidermidis (44.8%), where 72.1% of the isolates showed resistance towards at least one type of antibiotic. The highest and lowest resistance was observed for penicillin and gentamycin, respectively. The aim of the study was to evaluate the level of satisfaction and factors affecting patient satisfaction in the preanaesthetic clinic (PAC). A total of 304 patients who attended PAC for preoperative evaluation were recruited into this study. A bilanguage validated Patient Satisfaction Survey in English and Bahasa Malaysia with total questions of 18 examining non-provider factors (NPF) (facilities and appropriateness of waiting time) and provider factors (PF) (doctor, counter services, nurses and supporting staff) were utilized. The survey form was graded in a strongly agree/ disagree five-point Likert scale format for patients’ level of satisfaction. Overall satisfaction of preoperative evaluation was reported as 98.7% with mean scores from doctor (4.49 + 0.60), counter service (4.48 + 0.61), support staff (4.47 + 0.62) and the NPF (4.40 + 0.62), respectively. All factors were found to be significantly correlated with the overall patients’ satisfaction (p<0.001) and waiting time shown to be the independent predictor affecting the level of satisfaction. {"title":"Comparing Teachers and Medical Students as Trainers of Cardiopulmonary Resuscitation (CPR) among Secondary School Students","authors":"M. Training the public on CPR is one of the methods to increase the bystander CPR rate. This study aimed to compare the efficacy in acquiring and retaining CPR skills and knowledge among secondary school students in Klang Valley trained by school teachers and medical students. We recruited five school teachers and five medical students as trainers. They were trained in several sessions by American Heart Association (AHA)-certified instructors using the video-assisted CPR training module. The recipients were 44 secondary school students divided between the teacher’s group and the medical student’s group. We compared knowledge and psychomotor skills between these two groups prior, immediately after and at three months after CPR training. Students in the teacher’s group showed a higher increase in knowledge comparable to the medical student’s group (median score difference 3 vs 2, p>0.05) and in psychomotor skill (median score difference 5 vs. 7, p<0.05). The level of knowledge and skills decreased after 3 months but remained significantly higher than at baseline for both groups. In conclusion, teachers could provide CPR training to their students as effective and retainable as medical students. {"title":"Adult Personality and its Relationship with Stress Level and Coping Mechanism among Final Year Medical Students","authors":"Z. The relationship between the big five personality traits, coping mechanisms and stress level among final year medical students of Universiti Kebangsaan Malaysia (UKM) was investigated in this study. In this cross-sectional study, 152 final year medical students batch 2017/2018, were randomly selected, excluding those who had chronic diseases, married and repeated final year. The questionnaires used were Big Five Personality Inventory and 12-Item General Health Questionnaire. An open-ended question was used to determine how the respondents cope with their stress. Data was analyzed using SPSS 20.0. Agreeableness was the most common personality trait portrayed among the students, whereas Openness to Experience appeared to be the least. The relationship between the Big Five Personality Traits and coping mechanisms (p=0.016; p<0.05) were significantly related. Students with Neuroticism personality had the highest rate of practicing avoidant coping mechanism, while students with Extraversion personality had the highest rate of practicing active coping mechanism. Total score of stress were significantly difference between the different personality traits (p<0.001; p<0.05). However, the p value of 0.359 (p>0.05) and 0.94 (p>0.05) for the Big Five Personality Traits and level of stress showed no significant results on academic performance.
