However, leech saliva contains proteins with anticoagulant properties that can lead to prolonged bleeding from the bite area. We report a case of a 35-year-old male who presented to the Emergency Department due to prolonged bleeding from a leech-bite wound. He did not have any significant past medical history or medication history. He had undergone medicinal leech therapy for chronic back pain. After the therapy, he noted that the bleeding did not stop after 7 hours (which normally stops within 30 minutes) and decided to seek treatment. There were wounds over his lower back and both feet, which were all clean. The wound over the lower back had persistent oozing. We applied topical adrenaline and manual compression on the wound, and eventually the bleeding reduced and stopped. There are many suggested methods to stop bleeding from leech-bite wounds but there is scarcity of evidence regarding the superiority of any method. This is the first case report regarding bleeding complications in medicinal leech therapy in Malaysia. This report aims to raise awareness regarding the complications arising from a leech bite among the practitioners of medicinal leech therapy, the public and healthcare providers in emergency facilities. Prevention and management of DM are largely dependent on self-care in our daily lives. Nonetheless, we have little information concerning factors influencing public engagement in DM self-practice, which might have contributed to the high prevalence of DM in Perlis. This study was conducted to evaluate knowledge, attitudes, and factors associated with self-practice towards DM among the public in Kangar, Perlis. A cross-sectional study was conducted in June 2018 for three months. Eligible participants were invited to complete a selfadministered questionnaire in public areas in Kangar. Descriptive analyses on the sociodemographic, medical backgrounds, knowledge, attitudes, and self-practice towards DM were performed. Independent t-test and Spearman’s correlation were performed to assess the association between sociodemographic characteristics, medical history, knowledge, and attitudes with self-practice towards DM. Multiple linear regression was used to adjust for potential confounding variables. A total of 319 respondents consented and participated in this study. Most of the respondents were female (66.5%), Malay (94%), and had tertiary education (51.7%). Overall, most participants had good scores on knowledge (80.3%), attitudes (98.4%), and self-practice (97.2%) towards DM. A statistically significant positive association was found between age, education level, and attitudes score with DM self-practice (p<0.05). Currently this is not practiced in Malaysia. This study evaluates feasibility and impact to stroke team door to review time when prehospital notification is employed. Duration of case control study was between June 2018 to January 2019. Control phase consists of conventionally activating stroke team after in-hospital assessment by emergency medical officer. This was then followed by an intervention phase where on scene activation of stroke team was done by the Prehospital Emergency Care (PHC) staff. Training of PHC staff in recognising an acute stroke was based on identification of BE-FAST (Balance, Eyes, Face, Arm and Speech Test) abnormalities. The objectives were to compare the mean between two groups for acute stroke team review time, door to computerised tomography (CT) scan and door to thrombolysis time. Thirty-nine patients were analysed (control n=29, intervention n=10). Results were insignificant (p>0.05). Mean time in minutes for control phase vs. intervention phase was as follows: Door to stroke team review time, 25.96 + 39.16 vs. 15.9 + 13.14, door to CT scan was 43.04 + 40.00 vs. 25.8 + 11.35. Only 3 patients underwent thrombolytic therapy during study period. Limitation was non-parametric data with lack of number of acute stroke cases responded during the intervention period. Both have different presentations and treatments. We report a case of 35-year-old female who came to our Emergency Department (ED) with symptoms of lethargy, drowsiness, and difficulty breathing with foreign body sensation and a metallic taste in the throat which started 15 minutes after a right shoulder injection with 250 mg of lignocaine mixed with Triamcinolone in the Orthopedic Clinic. She was pushed to the resuscitation bay and was treated as anaphylaxis. Upon further evaluation, the blood pressure was never hypotensive, no sign of compromised airway, no gastrointestinal symptom, and no mucosal involvement which directs to the diagnosis of LAST rather than anaphylaxis. Surprisingly, she was gradually improving and was discharged home after six hours of observation in the ED. Factors that contribute to complicated PPE remain uncertain. This study was aimed to describe the characteristics, clinical predictors and outcome of children with parapneumonic effusion (PPE) in Universiti Kebangsaan Malaysia Medical Centre (UKMMC). A retrospective study on children with PPE who were admitted to UKMMC between January 2010 to December 2017 was conducted. Patients were categorised into 2 groups: simple and complicated PPE. Of 45 patients recruited, 20 (44.4%) patients had simple PPE and 25 (55.6%) had complicated PPE. Their median age was 32 months (IQR 16-63). The most common isolated organism was Streptococcus pneumoniae (61.9%), followed by Mycoplasma pneumoniae (19.0%) and Staphylococcus aureus (4.8%). Only 11.1% of patients received pneumococcal vaccination. There was no statistical significant difference in clinical features between simple and complicated PPE. Only 62.2% patients had ultrasound thorax done. Majority (95.0%) of patients with simple PPE were successfully treated with intravenous antibiotics alone. Sixty percent of patients with complicated PPE needed surgical intervention. Mean length of hospital stay for simple PPE was 10 (4.0) days and complicated PPE was 28 (16.5) days. In conclusion, clinical features could not predict complicated PPE. Use of ultrasound thorax as the main investigation tool for diagnosis and staging should be emphasised. It increases oxygen consumption and carbon dioxide production resulting in delayed recovery from anaesthesia and other adverse events. Granisetron is one of the drugs used to prevent post-anaesthetic shivering. This prospective, double-blind and randomised control study compared the efficacy of prophylaxis IV granisetron at 2 mg and 3 mg doses in preventing post-anaesthetic shivering. There were 104 patients, ASA I and II scheduled for elective open gynaecological surgery recruited and randomised into 2 groups: Group A and B, receiving 2 and 3 mg of IV granisetron, resepectively. Intravenous anaesthetic drugs were administered and tracheal intubation was facilitated by muscle paralysis. Patients’ baseline and periodic tympanic core temperatures were measured perioperatively. The incidence and severity of shivering were assessed postoperatively using Wrench Scoring Classification whereby Group A and B had 8.3% and 16.7% of incidence, respectively, in which they were not statistically significant (p = 0.199). All patients from both groups who shivered experienced Grade 1 shivering except for one patient who registered a Grade 2 in Group B. None of our patients had Grade 3 or 4 shivering postoperatively. No one experienced nausea or vomiting in the recovery area. Therefore, this study aims to determine the sleep quality, daytime sleepiness, depression and attention span in patients with OSA. This was a cross-sectional study among patients who were recently diagnosed with OSA in a Malaysian hospital. The socio-demographic and clinical data, Epworth Sleepiness Scale, Pittsburgh Sleep Quality Index, Patients Health Questionnaire and Comprehensive Trail Making Test were used as instruments. Findings were analysed and presented using Chi-Square, One-Way ANOVA and Independent- Sample T-test statistical analyses. Most of the respondents presented with poor sleep quality and impaired attention span. Approximately, half of the respondents had excessive daytime sleepiness. There were significant associations between sleep quality and daytime sleepiness (p-value=0.051), daytime sleepiness with depression (p-value=0.049) and severity of OSA with depression (p-value=0.026). Daytime sleepiness was significantly associated with working status (p-value=0.009) and driving status (p-value=0.033). In conclusion, most patients with OSA had poor sleep quality, excessive daytime sleepiness, and impaired attention span. To date, there has yet detailed research on this collaboration in Malaysia. This study aimed to explore interdisciplinary collaboration in the Medical Department (wards and clinics) of a state hospital. Four focus group discussions (FGDs) were held. A medical specialist, medical officer, nurse manager/charge nurse, staff nurse, senior and junior clinical pharmacists were all present at each FGD. Purposive sampling was used to recruit participants (nomination by heads of department). FGDs were performed in English, but responses in Malay were accepted and translated into English. All FGDs were audio-recorded, transcribed, and analysed thematically. In the theme of role clarity, most participants opined that the doctors led in patient management, while the nurses were in charge of monitoring, ambulating and drug administration. However, some participants were unfamiliar with the role of pharmacists. The majority believed that effective collaboration did exist, but insufficient. Weak communication skills, lack of communication, personnel and time were obstacles to effective collaboration. Regular discussions between different disciplines can encourage interprofessional collaboration. Despite doctors acknowledging pharmacists' Medication Therapy Adherence Clinic (MTAC) services, and nurses' human immunodeficiency virus (HIV) counselling and diabetic education services, some nurses and pharmacists were unaware of each other's services. Our study aimed to determine the effects of preoperative sugarless gum chewing on gastric fluid volume and pH. This prospective randomised study was performed on patients who underwent elective operation under general anaesthesia. Sixty patients who fasted overnight were randomly assigned to either “non-chewing gum” as control or “chewing gum group” as intervention group who chewed one piece of gum upon the call to the operating theatre. They were further enquired whether gum chewing helped cope with hunger and reduced anxiety. Gastric volume and pH were determined immediately after induction of anaesthesia. Gum chewing had no significant effect on gastric fluid volume measured [30.4+13.6 (control) vs 29.1+11.6 ml (intervention), p = 0.693] and gastric fluid pH [1.4+0.4 (control) vs 1.6+0.4 (intervention), p = 0.05] while managing to reduce anxiety in 90.3% patients who chewed gum. Patient may present with typical symptoms of acute heart failure or ischemic chest pain, depending on the underlying aetiologies. It is the responsibility of the treating physician to find out the cause as prompt treatment could possibly alter the prognosis of the patient, but at the same time bearing in mind other possible diagnosis apart from the one which is obvious. In this case, we present a lady who came with breathlessness and no chest pain. She was tachycardic and tachypnoeic upon arrival and her electrocardiogram (ECG) showed ischemic changes in the inferior and lateral leads. Bedside echocardiogram showed dilated left ventricle. Her cardiac biomarker was raised, making an ischemic event unlikely to be excluded. She eventually succumb to her illness.
