{"title":"Are anti-Chlamydia pneumoniae antibodies prognosis indicators for peripartum cardiomyopathy?","authors":"Arnaud Cénac,&nbsp;Ali Djibo,&nbsp;Christine Chaigneau,&nbsp;Nicolas Velmans,&nbsp;Jeanne Orfila","doi":"10.1097/01.hjr.0000065925.57001.3b","DOIUrl":"https://doi.org/10.1097/01.hjr.0000065925.57001.3b","url":null,"abstract":"<p><strong>Background: </strong>The authors recently pointed out an epidemiological relation between specific anti-Chlamydia pneumoniae antibodies and peripartum cardiomyopathy in Niamey (Republic of Niger).</p><p><strong>Design: </strong>In this work, they studied the prognosis value of such specific antibodies.</p><p><strong>Methods: </strong>The serological status for specific IgG, IgA and IgM anti-C. pneumoniae antibodies of 50 African women (age, mean+/-SD = 30.2 +/- 7 years) hospitalized in Niamey, with peripartum cardiomyopathy, was determined at the time of diagnosis. The diagnosis was categorized as 'complete remission' (13 patients, age = 29.3 +/- 6.5 years, observation delay = 27 months), 'incomplete remission' (27 patients, age = 30.7 +/- 7.6 years, observation delay = 14 months) and 'deceased' (10 patients, age = 30.3 +/- 6.2 years, observation delay = 13 months). The control group comprised 27 African women (age = 25.2 +/- 4.6 years), living in the same area. The Mann-Whitney and Fisher's exact tests were used for the statistical comparison.</p><p><strong>Results: </strong>The dilution of IgG specific anti-C. pneumoniae antibodies was higher (P = 0.047) in the 'incomplete remission' compared with 'complete remission'. The dilution of IgA specific anti-C. pneumoniae antibodies was higher (P = 0.033) in the patients with a severe evolution ('deceased' + 'incomplete remission') compared with 'complete remission'. There was no significant difference between patients in 'complete remission' compared with 'controls'.</p><p><strong>Conclusions: </strong>At the time of peripartum cardiomyopathy diagnosis the specific IgG and IgA anti-C. pneumoniae antibodies are of prognosis value: a high dilution is more often associated with a poor prognosis. The objective of this study was to examine how social relations are affected by marital stress and work stress in a population-based sample of Swedish women with CHD.</p><p><strong>Method: </strong>Data was obtained from the Stockholm Female Coronary Risk Study, comprising 292 women aged 65 years or younger, with a mean age of 56 (SD=7) years admitted for an acute event of CHD and examined 3-6 months after hospitalization. Marital and work stress was assessed using the Stockholm Marital Stress Scale and the Swedish version of the Karasek demand-control questionnaire, respectively. Condensed versions of the Interview Schedule for Social Interaction (ISSI) and of Interpersonal Support Evaluation List (ISEL) were used to assess social relations and social support.</p><p><strong>Results: </strong>Marital stress was associated with less social integration (P<0.001), less appraisal support (P<0.001), a lower sense of belonging (P<0.01) and less tangible support (P<0.01) even after controlling for work stress. Adjustment for age, socioeconomic status (education and occupational status) did not alter these results significantly. Work stress did not show statistically significant effects on any of the measured social relations.</p><p><strong>Conclusion: </strong>The present study showed that marital stress influenced women's social relations. Biological effects may mediate this association, but differences in utilization of preventive services may be important.</p><p><strong>Design and methods: </strong>Cross-sectional analysis of adults participating in the Third National Health and Nutrition Examination Survey with data on alcohol use and prior use of services for the detection and treatment of hypertension and hypercholesterolaemia. Regression models were analysed to provide age-, sex- and race-adjusted risk estimates for outcomes related to these services for several patterns of alcohol use relative to regular light drinkers.</p><p><strong>Results: </strong>Hypertension was more common in most alcohol use strata compared with regular light drinkers (adjusted prevalence ratios 1.22 for lifelong abstainers, 1.33 for nonlifelong abstainers, 1.35 for infrequent moderate drinkers, 2.01 for frequent moderate drinkers, 1.73 for infrequent heavy drinkers and 1.98 for regular heavy drinkers, P-values < 0.05). Having had blood pressure measured was similar in all drinking strata. The prevalence of hypercholesterolaemia was similar in drinking strata, but most drinking patterns were associated with lower use of services to detect high cholesterol (prevalence ratios 0.7 for lifelong abstainers, 0.8 for nonlifelong abstainers, infrequent moderate and regular moderate drinkers, 0.6 for infrequent heavy drinkers, and 0.7 for regular heavy drinkers, P-values < 0.05).</p><p><strong>Conclusions: </strong>The utilization of cardiovascular disease preventive services may contribute to the beneficial association of moderate drinking with cardiovascular mortality. Female AGA has usually been linked with hyper-androgenism and hirsutism and, most recently, also with polycystic ovarian syndrome (PCOS), even though epidemiological documentation of the latter association is scanty. Polycystic ovarian syndrome is quite common among Caucasian women, and its association with insulin resistance is well documented.</p><p><strong>Objectives and design: </strong>The aim of this study was to obtain a more precise estimation of the prevalence on female AGA and to describe its possible connections with insulin resistance linked parameters and with paternal and maternal family history of alopecia. A cross-sectional population based cohort survey was carried out in the City of Oulu, Finland in 1998.</p><p><strong>Setting and participants: </strong>As a part of a population based cohort study the hair status of 324 women aged 63 years was assessed by a modification of Ludwig's scale. The background data consisting of anthropometric measures (weight, height, body mass index, waist, hip and neck circumferences), smoking status, chronic diseases and their medication as well as the family history of AGA were collected by questionnaires and interviews made by study nurses and in clinical examination. Blood samples for laboratory tests were taken on the same occasion.</p><p><strong>Results: </strong>The prevalence of extensive loss of hair (at least grade II or III on Ludwig's scale) was quite high (31.2%). The insulin resistance associated parameters, such as waist and neck circumferences, abdominal obesity measured by waist-to-hip ratio, mean insulin concentration (11.3 mU/l versus 9.95 mU/l, p=0.02) or urinary albumin-to-creatinine ratio (1.80 versus 1.58, p=0.01), were significantly higher in women with extensive hair loss compared to those with normal hair or only minimal hair loss (grade I on Ludwig's scale). The women belonging to the highest quintiles of neck or waist circumferences had significantly increased risk for extensive hair loss compared to those with normal hair or minimal hair loss, the unadjusted ORs being 2.25 (95% CI, 1.26-4.03) and 1.75 (95% CI, 1.00-3.07), respectively. Similarly in women with hyperinsulinemia (fs-insulin >10 mU/l), microalbuminuria (urinary albumin-to-creatinine ratio exceeding the highest microalbuminuria decile (>2.5 mg/mmol) and paternal history of AGA the ORs for alopecia were increased being 1.65 (95% CI, 1.02-2.67), 2.39 (95% CI, 1.21-4.73) and 2.08 (95% CI, 1.26-3.44). However, while a number of experimental data support this concept, the protective role of antioxidants that may prevent LDL oxidation in atherosclerosis is only partially confirmed by studies in man. Observational and epidemiological data as well as randomised trials failed to provide clear-cut indications, because of mixed results on the protective role of antioxidants against cardiovascular diseases. In spite of the lack of a general consensus, recent data reinforce the concept that a regular intake of antioxidants present in food blocks the progression of atherosclerosis and that the reduced ability of LDL to oxidise may represent a good marker to follow the action of antioxidants. Diabetes mellitus is emerging as a major health problem in subSaharan Africa [1]. The economic burden of 1–195 diabetes mellitus on the patient, family, community and nation is enormous. However, medical innovations that can delay the onset and slow the progression of diabetes have tremendous potential to mitigate the associated clinical and cost repercussions. In sub-Saharan Africa, the HIV/AIDS pandemic is an unparalleled catastrophe and, coupled with the resurgence of tuberculosis and the unfinished agenda of communicable diseases, including malaria and childhood infectious diseases, most countries in the region have run a high health deficit. Results from the 2000 Behavioral Risk Factor Surveillance Survey.","authors":"Modele O Ashaye,&nbsp;Wayne H Giles","doi":"10.1097/01.hjr.0000065927.57001.a9","DOIUrl":"https://doi.org/10.1097/01.hjr.0000065927.57001.a9","url":null,"abstract":"<p><strong>Background: </strong>Our objective was to determine whether persons with coronary heart disease (CHD) were more likely to engage in healthy lifestyle behaviors (HLBs) than persons without CHD.</p><p><strong>Design: </strong>Cross-sectional study.</p><p><strong>Methods: </strong>Data from the 2000 Behavioral Risk Factor Surveillance System (BRFSS) (n=35,677) were analyzed. HLBs included maintaining an ideal body weight (body mass index < 25.0), eating five or more fruits and vegetables daily, performing at least 30 minutes of leisure time physical activity (LTPA) at least five times per week and non-smokers (former and never smokers). Logistic regression was used to determine whether persons with CHD were more likely to engage in all four HLBs than persons without CHD after adjusting for age, sex, race and education.</p><p><strong>Results: </strong>Only 6.3% of persons with CHD and 6.8% among persons without CHD engaged in all four HLBs. In the crude analysis, persons with CHD were 10% less likely than persons without CHD to engage in all four HLBs [odds ratio (OR)=0.9; 95% confidence interval (CI)=0.7-1.3]. After adjusting for covariates, persons with CHD were equally as likely to engage in all four HLBs as persons without CHD (OR=1.0; 95% CI=0.7-1.3).</p><p><strong>Conclusions: </strong>Overall, only a small proportion of persons engaged in all four HLBs. After adjusting for covariates, persons with CHD were just as likely as persons without CHD to engage in all four HLBs. This class of inhibitors, known as 'statins', has been shown to reduce cardiovascular morbidity and mortality. Accumulating data demonstrates a variety of mechanisms in which HMG-CoA reductase inhibition benefits the cardiovascular system.
