{"title":"Autologous fibrin adhesive in experimental tubal anastomosis.","authors":"S Rajaram, U Rusia, S Agarwal, N Agarwal","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>To evaluate autologous fibrin in rabbit oviduct anastomosis versus 7-0 vikryl, a conventional suture material used in tubal anastomosis.</p><p><strong>Methods: </strong>Thrombin was added to the autologous fibrinogen at the site of anastomosis to obtain a tissue adhesive. The anastomotic time, pregnancy rate, and litter size were evaluated. Three months later, a relaparotomy was done to evaluate patency and degree of adhesions, and a tubal biopsy was taken from the site of anastomosis.</p><p><strong>Results: </strong>Analysis of results showed a statistically significant (P < .001) shortened anastomotic time and superior histopathological union in the tissue adhesive group. {"title":"Reproductive outcome following hysteroscopic adhesiolysis in Asherman's syndrome.","authors":"Z Katz, A Ben-Arie, S Lurie, M Manor, V Insler","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>To determine whether hysteroscopic adhesiolysis improves reproductive outcome in women with Asherman's syndrome and pregnancy failure.</p><p><strong>Subjects: </strong>Ninety consecutive women who had undergone hysteroscopic adhesiolysis of intrauterine adhesions during a 5-year period. Only women in whom at least two previous pregnancies had ended with either a spontaneous abortion or a premature delivery accompanied by fetal or neonatal mortality and a hysteroscopic diagnosis of intrauterine adhesions were enrolled.</p><p><strong>Results: </strong>Whereas pregnancy outcome prior to the operation was 18.3% term deliveries, 3.3% premature deliveries, 62.4% first-trimester abortions, and 16.0% late abortions, after hysteroscopic adhesiolysis pregnancy outcome was 68.6% term deliveries, 9.3% premature deliveries, 17.4% first-trimester abortions, and 4.7% late abortions. In women with two previous unsuccessful pregnancies, the operative success rate measured by delivering a healthy newborn improved from 18.3% preoperatively to 64% postoperatively. In women with three or more unsuccessful pregnancies the success rate improved from 18.3% to 75%. {"title":"A double-blind comparison of nafarelin and leuprolide acetate for down-regulation in IVF cycles.","authors":"S L Corson, J N Gutmann, F R Batzer, B Gocial","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>To compare nafarelin (NAF) with leuprolide acetate (LA) for pituitary down-regulation in IVF cycles.</p><p><strong>Materials and methods: </strong>Patients in a private IVF program who received both nasal spray and subcutaneous injections starting in the luteal phase prior to intended stimulation with any one route containing active drug in a randomized, prospective, double-blind study. {"title":"Intrauterine or oral administration of levonorgestrel in combination with estradiol to perimenopausal women--effects on lipid metabolism during 12 months of treatment.","authors":"K Andersson, E Stadberg, L A Mattsson, G Rybo, G Samsioe","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>Limited data concerning serum lipids and lipoproteins are available on the effect of HRT in perimenopausal women, who commonly have marked bleeding disturbances and may have severe climacteric symptoms. Almost all previously published data have utilized a simplified form of lipoprotein analysis, which includes an estimation and not a determination of LDL cholesterol. To delineate the role of locally administered progestogen, perimenopausal women were studied for a year.</p><p><strong>Patients and methods: </strong>40 perimenopausal women with climacteric complaints. The continuous release of low-dose levonorgestrel from an intrauterine device was used as progestogen co-medication to estradiol in a new type of continuous combined hormone replacement therapy. Women were randomized to either cyclical treatment with 2 mg of oral estradiol valerate in combination with 250 micrograms of levonorgestrel for the last ten days (Cyclo Progynova) or continuously with 2 mg estradiol valerate orally in combination with a 20 micrograms per 24 hour levonorgestrel releasing intrauterine device.</p><p><strong>Results: </strong>Reduced HDL cholesterol was initially recorded in both treatment arms and disappeared after 1 year of treatment. Triglycerides were reduced in the orally treated group, but not in the device group. No changes in LDL cholesterol were noted.</p><p><strong>Conclusions: </strong>The findings suggest that continuous combined HRT with intrauterine release of 20 micrograms levonorgestrel per 24 hours in perimenopausal women is neutral as far as lipid metabolism is concerned, since no alterations compared with pretreatment values could be noted after 12 months of treatment. Less marked lipid changes were obtained in perimenopausal women as compared with data on postmenopausal women. {"title":"Treatment of ectopic pregnancy by systemic methotrexate, transvaginal methotrexate, and operative laparoscopy.","authors":"M Yao, T Tulandi, T Falcone","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>To evaluate the efficacy of treatment of ectopic pregnancy by local administration of methotrexate, systemic methotrexate, and laparoscopy.</p><p><strong>Methods: </strong>This study was a retrospective analysis from the department of Obstetrics and Gynecology of two university-affiliated hospitals. Medical records of 40 patients who were treated by methotrexate from January 1991 to October 1994 and 66 patients who were treated surgically by laparoscopy from April 1986 to June 1994 were reviewed. Among the methotrexate group, 19 patients were treated by ultrasound-guided injection and 31 others were treated by intramuscular administration. Success rate of the primary treatment and the duration of hospitalization were examined for each group. The cases of methotrexate failure were characterized and compared with the primary laparoscopic group.</p><p><strong>Results: </strong>The success rate was similar between the local (79.8%) and the systemic (66.7%) methotrexate groups. The primary laparoscopy group had a significantly higher success rate (95.5%) than the methotrexate group (72.5%); P < .01. When patients who had laparoscopy as their primary treatment (n = 66) were compared with those who underwent surgery after failed methotrexate treatment (n = 11), the primary laparoscopic group were seen to have shorter hospitalization time, smaller tubal diameter at surgery, and higher pre-operative hemoglobin level (P < .05). More patients in the methotrexate failure group (45.5%) had hemoperitoneum of > 500 mL than those in the primary laparoscopy group (1.5%; P < .001).</p><p><strong>Conclusions: </strong>There is no difference in efficacy between local and systemic methotrexate administration. Laparoscopic treatment of ectopic pregnancy is associated with a higher success rate than methotrexate treatment. {"title":"Effects on acne of two oral contraceptives containing desogestrel and cyproterone acetate.","authors":"C Charoenvisal, Y Thaipisuttikul, S Pinjaroen, O Krisanapan, W Benjawang, A Koster, W Doesburg","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>To compare the effects of a low-dose oral contraceptive containing desogestrel (Marvelon) and an anti-androgenic preparation containing cyproterone acetate (Diane) in Oriental women suffering from acne.</p><p><strong>Methods: </strong>In an open-label, bi-center, randomized study, 32 women using Marvelon and 34 using Diane were followed for 6 treatment cycles. The measured variables were objective and subjective severity of acne, and related biochemical variables such as sex hormone-binding globulin and free and total testosterone.</p><p><strong>Results: </strong>In Center A, with both preparations a decrease in mean objective acne score was observed, reaching statistical significance with Diane (P < .05). In addition, there was a significant between group difference at cycle 6 (P < .05). In Center B, a consistent and significant decrease in mean acne score was observed with Marvelon after three and six treatment cycles (P < .05 and P < .01) and with Diane after six treatment cycles (P < .001). There were no significant between-group differences. The decrease in percentage of severe/moderate acne was statistically significant with Marvelon in Center B (P = 0.002) and with Diane in Centers A (P = 0.014) and B (P = 0.004). {"title":"Vaginal lubricants for the infertile couple: effect on sperm activity.","authors":"W H Kutteh, C H Chao, J O Ritter, W Byrd","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>To determine the effects of natural vegetable oils and vaginal lubricants on sperm motion and viability.</p><p><strong>Design: </strong>Four widely used vaginal lubricants (K-Y Jelly, Astroglide, Replens, and Touch) and two vegetable oil products that have been used as vaginal lubricants were purchased through local vendors. Sperm was obtained by masturbation without lubrication from normal, healthy donors. Lubricants were mixed with sperm from individual donors and the effects on sperm motility were determined at 1, 15, 30, and 60 minutes.</p><p><strong>Setting: </strong>Southwestern Fertility Associates of The University of Texas Southwestern Medical Center at Dallas.</p><p><strong>Main outcome measures: </strong>Sperm motility was evaluated by manual motility counts and by computer-assisted semen analysis. Sperm viability was evaluated with Hoechst 33258 dye. The effects of the various lubricants were compared with those of a spermicidal agent, Gynol II (negative control) and Ham's F-10 (positive control).</p><p><strong>Results: </strong>Commercial lubricants inhibited sperm motility by 60-100% after 60 minutes of incubation. Sperm exposed to Replens or Astroglide were nonmotile and nonviable after incubation for 60 minutes, similar to the control, nonoxynol-9 containing product Gynol II. Canola oil had no detrimental effects and was indistinguishable from Ham's F-10 in terms of sperm viability and motility.</p><p><strong>Conclusions: </strong>For couples with infertility, the use of vaginal lubricants during intercourse is not recommended. Although it appears to be a rare entity, usually secondary to a primary site of infection by Mycobacterium tuberculosis, its true incidence is not known owing, in part, to inadequate diagnostic procedures, particularly in less developed areas of the world.</p><p><strong>Patients and methods: </strong>We present our experience with 25 cases of genital TB. Of these woman, 21 were seen for infertility; 3 were postmenopausal with uterine hemorrhage, and 1 was admitted with an acute abdomen. All women were treated medically and/or surgically.</p><p><strong>Results: </strong>Only five women were found to be suitable for artificial insemination.
