Differentiation of nongestational from gestational trophoblastic neoplasia is crucial, the prognosis and therapy being distinct in each type. We report a case of primary ovarian, nongestational, pure choriocarcinoma in a 27-year-old woman treated with surgery and aggressive chemotherapy. {"title":"Diagnosis and management of bilateral theca lutein cysts in a normal term pregnancy.","authors":"D H Barad, M L Gimovsky, R H Petrie, E T Bowe","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Theca lutein cysts are benign neoplasms known to be associated with twins, molar pregnancy and erythroblastosis fetalis, but they are only rarely associated with a normal singleton pregnancy. Their natural course is postpartum spontaneous regression. Documented cases were noted because of abdominal pain or dystocia. This paper reports theca lutein cysts which were asymptomatic and were first noted ultrasonographically in a singleton gestation at term. In a few published cases and in a larger number of unpublished cases, apparent confusion as to the benign nature of these cysts has led to unwarranted bilateral oophorectomy. Asymptomatic theca lutein cysts undoubtably occur more commonly than they are reported. With the increasing use of ultrasonography and cesarean section, this condition probably will be observed more frequently in the future. Although these drugs have been in use for many years, recent advances have made their employment more desirable and therefore more frequent. While many of these drugs obtain the desired effect, all of them have side effects that may be either beneficial or adverse to the mother or fetus, or to both. {"title":"Three-dimensional subgross morphology of the human endometrium.","authors":"W A Schmidt, M Karon","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>A method is described for subgross, structural assessment of endometrial configuration. Human uteri from autopsy and surgical pathology were preserved in formalin and sectioned at 1-mm intervals. A procedure for staining, dehydration, and clearing was developed to achieve three-dimensional resolution of endometrial morphology. Processed specimens were sealed in translucent heavy plastic bags for the purpose of storage, easy manipulation under the dissecting microscope, and for photographic documentation. For simultaneous comparison with standard histological preparations, selected areas of sealed specimens were punched-out and paraffin sections made. This simple method can easily be applied to the future investigation and classification of focal premalignant, dysplastic, anaplastic, and atypical changes in the endometrium. Of the total population, 0.5% expressed both antigens, 8.8% expressed only hCG, and 5.5% expressed only CEA; 85% expressed neither. Intraoperative findings included a massively dilated biliary collecting system and an impacted stone at the ampulla of Vater. The bladder was stable in 53 patients and detrusor instability was detection in 59 of the incontinent women. Cystometry performed in the supine position cannot be relied upon to be sufficiently useful in the complete evaluation of various forms of instability. Involuntary detrusor contractions occurring during one or more provocative tests represent an abnormal bladder behavior pattern. An isometric contraction develops when the stream is interrupted and stable incontinent women show only a small isometric rise in 35 cases (66%). Fifty-six patients with unstable bladders presented a large isometric detrusor contraction (95%). Appearance on ultrasonography and review of the literature.","authors":"C Janus","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The term Krukenberg tumor refers to metastatic disease to the ovary and more specifically those metastases containing mucin-secreting signet cells. When this definition is used most of these tumors arise in the gastrointestinal tract although the term Krukenberg tumor has also been used more loosely to describe metastatic disease to the ovary from other sites such as breast and genital tract. On ultrasound examination, the Krukenberg tumor has been described as a complex type of mass showing both solid and cystic characteristics.
