The ultrasound presumptive diagnosis was that of low urinary tract obstruction. Neonatal mortality with this defect is estimated to be approximately 50% and mainly arises from kidney damage. In order to achieve further information for obstetrical management, intrauterine bladder evacuation was performed and fetal urine production rate monitored by ultrasound. Report of a fatal case.","authors":"P D Mozley","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Malignant hyperthermia has been an iatrogenic syndrome which was usually fatal. The syndrome occurs when certain physiologically active compounds act on defective skeletal and cardiac muscle cells. The syndrome consists of a soaring fever, severe acidosis, tachycardia, tachypnea, and usually myoclonic spasms. Cardiac arrhythmias, shock, bleeding disorders, and death soon follow. Malignant hyperthermia has characteristically complicated the administrations of anesthesiologists, but is being triggered by drugs used in other diagnostic and therapeutic activities. This paper reports a fatal case which followed the infusion of iodinated contrast media. The increased release of epinephrine and the production of fibrin split products seen in an iodinated contrast media reaction suggest certain commonalities between it and a malignant hyperthermia reaction which may be triggered be epinephrine and is complicated by disseminated vascular clotting and bleeding disorders. The potential for successful treatment has greatly improved with the availability of dantrolene. Serial sonographic examinations revealed a large unilocular fetal intraabdominal mass. Cystocentesis and amniocentesis under ultrasonic control were preformed as well as an amniogram to establish the diagnosis. This is the fourth reported case in the English literature of fetal urethral agenesis diagnosed during the prenatal period. Construction of a profile of this neoplasm was undertaken. The mean age of patients was 60 years, and most were postmenopausal with abdominal symptoms and a unilateral abdominopelvic mass. Only one case had bilateral tumors. The pattern of spread of this tumor is usually regional with only occasional distant metastases. Diffuse peritoneal seeding is not a characteristic feature of MBT. 56% of the patients with MBT died of their tumor, usually within 3 years of initial diagnosis. Mean survival in fatal cases was 1 year. Microscopically, foci of characteristic benign Brenner tumor may or may not be seen in individual examples of MBT. Admixture of adenocarcinoma (often mucinous carcinoma) and squamous carcinoma may also be seen. diagnostic problems can arise when a mixture of histologic patterns is present. The lesion had the characteristic mixed pseudolobular pattern with cellular areas separated by edematous stroma. A prominent, additional feature was the presence of partially or completely obstructed vessels at the hilus. A 27-year-old woman had this focal pregnancy-like change in an area of mild sclerosing adenosis. Ultrastructurally, the cells have some features that are similar to the secretory cells of normal lactation. Histologically the mass appeared to have two components, both of which were malignant. The first component was a surface squamous cell carcinoma, while the second was an infiltrating pseudosarcomatous lesion. The polypoid tumor mimics lesions which arise in the esophagus, pharynx, larynx, and mouth, and which have been variously classified as pseudosarcoma or carcinosarcoma. Treatment consisted of applications of radium followed, in 5 months, by radical abdominal hysterectomy and pelvic lymphadenectomy. The patient remains free of any clinical recurrence of the tumor after more than 2 years, suggesting a favorable prognosis. A prospective study on differential diagnosis.","authors":"F A Gonzalez,&nbsp;M Waxman","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>This study analyzes the clinicopathologic findings in patients with ectopic pregnancy (EP), and deals with the differential diagnosis of the EP, intrauterine pregnancy (IUP), and pelvic inflammatory disease (PID). We evaluated 346 patients with suspected EP. Among those, 119 patients had EP, 82 had IUP, and 55 had PID without pregnancy. The incidence of EP was 1/32.9 live births. Comparing with the other groups, the patients with EP were slightly older, gave a history of previous pregnancies, had acute abdominal pain, nausea, vomiting, dizziness, and fainting, and had direct and rebound abdominal tenderness, pain on motion of the cervix, absence of a pelvic mass, and bilateral adnexal or cul de sac fullness. Culdocentesis was accurate in 95.1% of EP cases. Salpingectomy was performed in 89.9% of the patients with EP. A retrospective study of 501 consecutive patients.","authors":"F A Gonzalez,&nbsp;M Waxman","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>We reviewed the records of 501 patients with ectopic pregnancy (EP) seen at the Kings County Hospital during a five-year period (January 1973-December 1977). Our population characteristics and anamnestic data correlated well with those reported in the literature, except for lower incidence of a previous pelvic infection. Abdominal pain, amenorrhea, and vaginal bleeding were the most common presenting symptoms. While the ultrasound studies demonstrated adnexal masses of a non-specific nature, the laparoscopy constituted an accurate diagnostic tool. Ruptured EP was found in 339 patients (67.6%). Salpingectomy only was the mode of treatment in 369 cases (73.6%). In 28.4% of the cases there a microscopic evidence of chronic inflammatory changes of the tube. There were two fatalities.
