The main risk factor is ultraviolet (UV) radiation, which can be particularly high in certain overseas territories. As a result, outdoor workers constitute a high-risk population.</div></div><div><h3>Objectives</h3><div>The aim of this study was to determine the proportion of skin cancers potentially attributable to occupational sun exposure in Reunion Island.</div><div>The increasing prevalence of skin cancer in recent years has led to its recognition as a significant public health concern. The primary risk factor is ultraviolet (UV) radiation, which has been observed to reach elevated levels in certain overseas territories. Consequently, individuals engaged in outdoor work are considered a high-risk population. The objectives of this study were to determine the proportion of skin cancers potentially attributable to occupational sun exposure in Reunion Island and to identify factors associated with such exposure.</div></div><div><h3>Methods</h3><div>This study covers all melanomas, cutaneous squamous cell carcinomas and a representative sample of basal cell carcinomas, diagnosed over one year in subjects who had worked for at least 3 years on the island. Telephone interviews were conducted with each subject to gather data on their exposure to sunlight during childhood, leisure activities and work. Cancers were classified as potentially attributable to work if the time spent in the sun at work was greater than the time spent in the sun during leisure time, taking into account sun protection data and the site of the skin cancer (areas exposed during work).</div></div><div><h3>Results</h3><div>Of the 329 people included, 37.7% had skin cancer(s) attributable to occupational UV exposure. Several high-risk professions were identified, including farmers. Among people surveyed, 93% reported that their employers had taken any steps to prevent sun exposure and 73% had never used sunscreen at work.</div></div><div><h3>Conclusion</h3><div>The percentage of people with skin cancer potentially attributable to occupational sun exposure in Reunion Island is considerable. Prevention and education on photoprotection in the workplace are essential. Dermal invasive melanoma developing from LM is called LM melanoma (LMM) and has the same prognosis as other melanoma subtypes. The standard treatment for LM is surgery, which offers the lowest risk of recurrence. Alternatives exist, however, including radiotherapy (RT).</div></div><div><h3>Objective</h3><div>Describe a real-life cohort of patients with LM/LMM treated with RT and assess prognostic factors for recurrence.</div></div><div><h3>Method</h3><div>This is a retrospective monocentric real-life study conducted in Bordeaux University Hospital. All LM or LMM patients treated with RT between 2009 and 2020 were included. RT was administered using a superficial or orthovoltage X-ray (kV) with a dose of 40 Gy in 10 fractions over 5 weeks.</div></div><div><h3>Results</h3><div>Ninety-one patients were included: 67 LM and 24 LMM. Sixty-four patients were female, the mean age was 77 (range: 50‑96) in LM group, and 80 (range: 56–98) in LMM group. The mean duration of post-RT follow-up was 25.1 months (range: 0–100). Fourteen recurrences were observed (15.4%): 10 in LM group (14.9%) and 4 in LMM group (16.7%). Most recurrences occurred in the irradiation field: 8/10 in LM group, and 2/4 in LMM group. Recurrences were more frequent in men (6/27 vs 8/64) and on the eyelid area (4/14) compared to others locations. The risk of recurrence was also higher in patients with at least one prior treatment failure (8/36 (22.2%) vs 6/55 (10.9%)). Dereure","doi":"10.1016/j.annder.2024.103329","DOIUrl":"10.1016/j.annder.2024.103329","url":null,"abstract":"<div><h3>Background</h3><div>In bullous pemphigoid (BP), prolonged treatment with low-dose methotrexate (MTX) may represent an effective strategy to maintain an initial clinical remission achieved by short-duration superpotent topical steroids (STS).).</div></div><div><h3>Patients and methods</h3><div>To evaluate the efficacy and safety of MTX in a real-life setting, a retrospective analysis was conducted in a reference centre for all BP patients treated between 2015 and 2020 with STS and MTX, either in monotherapy (mMTX) or in combination with oral steroids (MTX+OS). The primary endpoints were BP relapse rate and time to relapse during treatment for patients achieving initial disease control (IDC). Secondary endpoints included IDC achievement rate, relapse rate and time to relapse after MTX discontinuation, adverse effect (AE) and mortality rates, number of patients permanently discontinuing MTX owing to AE, number of patients alive and still in complete remission (CR) at the latest updates, and whether they were off treatment (cure rate).</div></div><div><h3>Results</h3><div>The BP relapse rate during treatment after achievement of IDC was significantly higher in patients treated with mMTX (32.2%) than with MTX+OS (0%) (<em>p</em> = 0.03); mean time to relapse was 10.3 months. The BP relapse rate after MTX discontinuation and the cure rate did not differ significantly between the two groups. One patient (receiving mMTX) died during follow-up and 27.8% of patients experienced at least one AE during the study. The cure rate in patients achieving IDC was 45.2% with mMTX vs. 30% with MTX+OS (<em>p</em> = 0.15).</div></div><div><h3>Conclusion</h3><div>Our study highlights the value of low-dose mMTX for treating BP in a real-life setting, both for achievement of initial response and for long-term control, with a good overall safety profile and no significant additional mortality risk in elderly subjects.
