{"title":"The effects of prefrontal leucotomy on visuoperceptive and visuoconstructive tests.","authors":"D T Stuss, D F Benson, E F Kaplan, C L Della Malva, W S Weir","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Visuoperceptive and visuoconstructive tests were administered to three groups of schizophrenics who had undergone bilateral prefrontal leucotomy approximately 25 years earlier. A nonoperated schizophrenic patient group and a normal subject group served as controls for the effects of schizophrenic symptomatology and for age and education. The results demonstrated that orbital frontal white matter lesions do not disturb simple perceptions and constructions. The nonoperated schizophrenics had significant problems on many of the tests. Only one test, the WAIS Picture Arrangement, revealed deficits in all patient groups, including the good recovery leucotomized patients. {"title":"Syncope due to right atrial myxoma.","authors":"D I Peterson","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Atrial myxomas are uncommon, but when present they often cause neurological abnormalities. Those that arise from the left atrium are a well-recognized source of cerebral emboli while those that arise in the right atrium may cause recurrent attacks of syncope. Twelve cases of atrial myxoma were treated at Loma Linda University Medical Center from 1968 to 1984. Loss of consciousness was one of the symptoms in all three of the right-sided lesions. Surgical removal of the tumor eliminated the attacks of syncope in each case. {"title":"Application of a new rating scale of brain dysfunction to monitoring rehabilitation in 65 patients with severe head injury.","authors":"A S Dekaban, C E Robinson","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Sixty-five patients with severe head injury sustained in motor vehicle accidents (MVA) are the subject of the prospective investigation. For assessment of results of a particular treatment, planning of long-range management and predicting the outcome in terms of employment or of self-care, it is essential to monitor sequentially the progress of recovery. A comprehensive semiquantitative rating Scale g was developed and its application during repeated examinations demonstrated. The Scale g is open-ended, as it allows physicians and investigators in the health field to produce their own schedules of any disability provided that seven levels of that particular dysfunction are defined according to the principles of Scale g. The progress of recovery in motor deficits, dysarthria, mental impairment, and self-care was assessed using appropriate schedules according to Scale g in 65 inpatients at their first admission to the rehabilitation center and at their discharge. The change was made evident by showing patients' advancement, if present, to the higher functional level. CT scan showed a ring enhancing lesion in the pons and CSF examination showed elevated protein and pleocytosis. A pulmonary abscess containing Nocardia was found and antimicrobial therapy produced progressive reduction in size of the pontine lesion and clinical improvement without surgical drainage. {"title":"Effect of heparin reversal following endarterectomy in an atherosclerotic animal model.","authors":"S L Giannotta, S K Hirtz, S Hyman","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>A focal atherosclerotic plaque was induced in the aortas of New Zealand White female rabbits by a balloon injury and an atherogenic diet. Under general anesthesia and systemic heparinization, endarterectomy was performed using the operating microscope. Animals were sacrificed at 5, 10, 20, and 60 min following the endarterectomy and operated aortic segments were perfused and examined using scanning electron microscopy. Each segment was compared to a similar segment of endarterectomized aorta from a normal, nonatherosclerotic rabbit. Thrombus formation including aggregated platelets, red cells, and fibrin was found to be more pronounced in the atherosclerotic segments. Ten more atherosclerotic rabbits underwent identical procedures except that heparin was reversed using protamine sulfate 5 min following the endarterectomy. When these specimens were compared to a similar atherosclerotic group without heparin reversal, it was evident that a tremendous thrombogenic process had taken place in the \"reversed\" segments.
